Extragenital lichen sclerosus (EGLS) is often resistant to first-line ultrapotent corticosteroids, with few steroid-sparing options available. Topical roflumilast has been approved by the US Food and Drug Administration (FDA) for an expanding number of indications in dermatology, including plaque psoriasis, atopic dermatitis, and, most recently, seborrheic dermatitis. We present two cases of symptomatic EGLS, who both demonstrated clinical improvement using roflumilast 0.3% cream as early as one month of follow-up.

BACKGROUND: Vulvar lichen sclerosus (VLS), vulvar lichen simplex chronicus (VLSC) and erosive vulvar lichen planus (EVLP) are the most common vulvar lichenoid dermatoses. They are frequently misdiagnosed in clinical practice, and comparative studies of these diseases remain limited.
OBJECTIVES: To analyse the clinical characteristics and quality of life (QoL) burden of these three distressing vulvar diseases and to raise awareness of them.
METHODS: This retrospective cross-sectional study analysed clinical registration data of adult patients diagnosed with VLS, VLSC or EVLP at a single-centr

Lichen sclerosus (LS) is a chronic, lymphocyte-mediated inflammatory skin disease characterized by porcelain-white atrophic skin lesions. Localized LS is relatively common and predominantly occurs in the anogenital region. Generalized LS involving both genital and extragenital sites is clinically rare and frequently refractory to conventional therapies like topical corticosteroids. As an emerging targeted therapy, Janus kinase (JAK) inhibitors demonstrate broad therapeutic potential. The highly selective JAK1 inhibitor upadacitinib demonstrates robust efficacy across various inflammatory derma

Lichen sclerosus (LS) is a chronic inflammatory dermatosis of the anogenital region with limited therapeutic options. Emerging evidence implicates dysregulation of the Janus kinase (JAK)-signal transducer and activator of transcription (STAT) pathway in LS pathogenesis, supporting investigation of targeted therapies. We report a series of 6 patients treated with topical ruxolitinib for vulvar LS, applied twice daily. Clinical outcomes were assessed using the Clinical Scoring System (CSS) and Patient Global Assessment (PtGA) over a mean follow-up of 7.5 months. All patients reported early sympt

AIMS: The aim of the study was to examine the potential risk of vulvar lichen planus (LP), alone or associated with lichen sclerosus (LS) or lichen simplex chronicus (LSC), to evolve towards vulvar cancer (VSCC).
METHODS: We retrospectively investigated the incidence of vulvar cancer in women diagnosed with LP, alone or associated with LS and LSC, between 2007 and 2022.
RESULTS: We retrieved the data of 77 women with LP with a mean age of 63.5 ± 11.9 years old. Of these, 53 had LP, 19 had LP+LS, and 5 had LP+LSC. Ultrapotent topical corticosteroids were the first-line treatment. The mean fol

BACKGROUND: Vulvar lichen sclerosus (VLS), vulvar lichen planus (VLP), and vulvar lichen simplex chronicus (VLSC) are inflammatory dermatoses with overlapping clinical features, posing diagnostic challenges. VLS and VLP carry malignant potential, unlike VLSC. Comparative studies are limited despite frequent misclassification.
OBJECTIVE: To compare the clinical characteristics of VLS, VLP, and VLSC and identify distinguishing diagnostic features.
METHODS: A retrospective study was conducted on clinical data from patients diagnosed with VLS, VLP, or VLSC at the Beijing Hospital Vulvar Clinic (

Penile squamous cell carcinoma (PSCC) and its established precursor, penile intraepithelial neoplasia (PeIN), are classified as HPV-associated and HPV-independent. However, there is a spectrum of precursor lesions, which presents diagnostic challenges due to morphologic overlap. Immunohistochemical loss of GATA3, a tumor suppressor gene, has recently been reported in vulvar neoplasia. In this study, we evaluate the expression and diagnostic utility of GATA3 immunohistochemistry (IHC) in penile neoplasia. GATA3, p53, and p16 IHC was performed on 97 penile lesions, including lichen sclerosus (LS

BACKGROUND: Photodynamic therapy (PDT) has demonstrated broad application prospects in treating gynecological diseases such as human papillomavirus (HPV) infection, cervical intraepithelial neoplasia (CIN), and vulvar lichen sclerosus (VLS), due to its non-invasiveness, targeting capability, and low side effects. However, the bibliometric characteristics of this field remain unexplored.
METHODS: On March 26, 2026, we retrieved literature from the Web of Science Core Collection (WoSCC) database covering publications from January 1, 2010, to December 31, 2025. A total of 748 publications (545 a

The 2026 International Society for the Study of Vulvovaginal Disease terminology for vulvar intraepithelial neoplasia and squamous intraepithelial lesion requires p16 and p53 immunohistochemistry for classification into human papillomavirus-associated or human papillomavirus-independent disease. p16 and p53 are tumor suppressor proteins; block positive p16 staining is a surrogate marker for genomic integration of oncogenic human papillomavirus, while null or overexpressed p53 staining correlates with TP53 mutation. OBJECTIVES: Describe clinicopathologic features of vulvar low-grade squamous intraepithelial lesion (LSIL) and small condyloma to elaborate diagnostic criteria.
METHODS: Pathology database search identified vulvar biopsies reported as LSIL, wart, or condyloma. Exclusions included cancer specimens, concern for vulvar intraepithelial neoplasia (VIN), large condyloma, and inadequate slides or tissue blocks. After review of histopathology and p16, cases were designated as LSIL or condyloma, and clinical information was collected. Descriptive statistics and group comparisons were performed.

Lichen sclerosus is a chronic inflammatory condition that involves both anogenital and extragenital areas, primarily affecting females, and can occur at any age. Although extragenital manifestations are rare, children may present with genital involvement, extragenital lesions, or a combination of both. We report a case of an eight-year-old girl who had vulvar lesions for two years, followed by the appearance of widespread cutaneous lesions two months prior to presentation. The diagnosis of lichen sclerosus was confirmed through dermoscopic evaluation and histological examination.

BACKGROUND: Lichen sclerosus (LS) is a chronic inflammatory dermatosis predominantly affecting the anogenital region and is associated with significant impairment of quality of life. High-potency topical corticosteroids are the standard of care; however, some patients show inadequate response or experience relapse. Noninvasive physical plasma (NIPP) is an emerging therapeutic modality with antimicrobial and anti-inflammatory properties, but its role in LS remains unclear.
OBJECTIVE: To evaluate the safety, tolerability, and preliminary efficacy of NIPP as an adjunctive treatment in patients w

BACKGROUND: The introduction of second-generation antipsychotics (SGAs) over the past 2 decades generated considerable optimism that better antipsychotic treatments for schizophrenia and bipolar disorder were possible. SGAs offer several tolerability benefits over first-generation antipsychotics (FGAs), particularly with respect to extrapyramidal symptoms. However, SGAs can induce serious metabolic dysregulations, especially in drug-naive, first-episode, and child and adolescent populations, with olanzapine and clozapine having the highest propensity to cause these abnormalities. An elderly postmenopausal woman with untreated vulvar lichen sclerosus and lichen planus (LP) overlap presented with abdominal pain, constipation and urinary symptoms. Severe vulvovaginal scarring caused complete vaginal obliteration, preventing conventional speculum examination and transvaginal ultrasonography. Endorectal and abdominal ultrasonography revealed a large cystic pelvic mass. Urgent laparotomy identified extensive haematocolpos and uterine distension due to complete outflow obstruction. The patient underwent a hysterectomy, bilateral salpingo-oophorectomy and subtotal colpectomy.

BACKGROUND: Lichen sclerosus (LS) is a chronic inflammatory condition that mainly affects genital skin. It causes distressing symptoms that impact daily quality of life, as well as progressive anatomical changes and a potential risk of cancer. Vulval LS is often misdiagnosed and treatment delayed. If nonexperts can be supported to identify clinical features of LS, they may diagnose the condition sooner and commence timely treatment or refer a patient for assessment and confirmatory biopsy.
OBJECTIVES: To reach international expert consensus on clinical diagnostic features for vulval LS.

BACKGROUND: Vulval lichen sclerosus (VLS) is a chronic inflammatory condition that is frequently misdiagnosed and under-recognised. To date, qualitative research has focused on lived experience of VLS, with women attributing diagnostic delay to poor interactions with healthcare professionals (HCPs), often due to lack of knowledge. In the UK, women with VLS are most likely to present to primary care.
AIM: To establish HCPs' perspectives on the identification, management, and education of vulval skin disease, with a focus on VLS.
