A Guide to Screening for Autoimmune Diseases in Patients With Vulvar Lichen Sclerosus.

BACKGROUND: The aetiology of vulvar lichen sclerosus (VLS) remains unknown. However, there is evidence that in addition to a genetic predisposition, autoimmunity contributes to the pathogenesis. OBJECTIVES: The objective of this study was to determine the prevalence of autoimmune disease and positive autoantibody serology in patients with VLS. METHODS: A VLS database in Sydney, Australia, was retrospectively reviewed. A diagnosis of VLS was required for inclusion in the study. Data collected included demographics, comorbidities including any personal history of autoimmune disease, family his

Challenges With Diagnosis of Labial Agglutination Due to Lichen Sclerosus.

OBJECTIVE: Labial adhesion (LA) lacks a clear etiology but is associated with low estrogen levels and lichen sclerosus (LS). Genitourinary Syndrome of Menopause is a contributor due to low estrogen, needing surgical resection if symptoms persist after topical estrogen use. Early diagnosis and treatment of LS can decrease the risk of development of LA. The objective is to investigate LS prevalence as an etiological factor and evaluate the necessity for enhanced biopsy rates in LA patients. METHODS: TrinetX provided data from 80 global health care centers, focusing on patients with LA using ICD

Coexistence of Lichen Sclerosus and Morphea in a Single Vulvar Lesion.

Lichen sclerosus (LS) and morphea are chronic inflammatory disorders with overlapping clinical and histopathological features, often complicating diagnosis. While the co-occurrence of LS and morphea as distinct lesions in the same patient is well documented, their overlap within a single lesion is rare, especially in genital regions. Herein, we present a case of a 60-year-old female with coexisting LS and morphea in the genital region, exhibiting features of LS while demonstrating histological characteristics of both. This case underscores the diagnostic challenges and provides further support

Effectiveness of nanofat in treatment-refractory penile lichen sclerosus: results from a pilot retrospective single-center case series.

BACKGROUND: Penile lichen sclerosus (PLS) is a chronic inflammatory skin disorder that may lead to progressive architectural distortion of male genitalia, with a dramatic impact on quality of life and a high unmet therapeutic need. In keeping with vulvar disease, nanofat grafting could represent a viable option in this setting. METHODS: Patients aged 18 years or above with biopsy-proven, medical and/or surgical treatment-resistant PLS were offered nanofat and were evaluated at baseline, 1 and 24 months. RESULTS: Fourteen male patients with a median age of 46 years (interquartile range [IQR]:

Trends and Developments in Vulvar Lichen Sclerosus Research.

BACKGROUND: Despite the prevalence of vulvar lichen sclerosus (VLS) as a chronic skin disease in clinical settings, there is a notable absence of a comprehensive bibliometric analysis summarizing the existing literature on this topic. The aim of this study is to offer clinicians, researchers, and other interested parties an up-to-date overview of the research status and emerging trends in VLS through the use of bibliometric analysis. METHODS: The present research employed a thorough examination of the Web of Science Core Collection to identify publications pertaining to VLS within the time fr

AID Expression in Vulvar SCC Arising From Lichen Sclerosus et Atrophicus: A Retrospective Study.

Lichen sclerosus et atrophicus (LSA) is a chronic, progressive inflammatory disorder with a risk of malignant transformation to squamous cell carcinoma (SCC). This study investigated the role of activation-induced cytidine deaminase (AID) in UV-independent SCC arising from LSA. We retrospectively analyzed 19 female patients, including 5 SCC cases. AID expression was significantly associated with SCC development, pruritus, and shorter disease duration. AID was detected in SCC lesions and adjacent LSA tissues, mainly in basal cells. Reduced AID expression was observed in higher-stage tumors, sug

TGF-β drives myofibroblast activation and inflammatory mediator production in vulvar lichen sclerosus.

OBJECTIVE: Vulvar lichen sclerosus (VLS) is a chronic inflammatory skin condition characterized by whitening of the external genitalia, debilitating pruritus (itching), and pain. As disease advances, loss of labia, burying of vulvar anatomy (ie, clitoris, urethra, vaginal opening), and vulvar cancer can occur, yet mechanistic understanding of these events remains limited. VLS lesions appear visibly similar to scars and are frequently referred to as such. Therefore, we investigated the role of fibrosis in the VLS disease mechanism with the goal of identifying targets for new therapeutic and dia

Diagnostic gaps in vulvar diseases from referral to final diagnosis in a specialized center: analysis of pathways and recommendations for enhancing future diagnostic accuracy.

BACKGROUND: Vulvar diseases have a serious impact on women's quality of life and can be challenging to diagnose. Correct diagnosis is, however, mandatory for appropriate treatment. Therefore, the aim of this study was to evaluate diagnostic quality regarding the classification of disease symptoms, the suspected diagnosis of referring doctors, the agreement of suspected diagnosis with final diagnosis and the latencies from onset of symptoms to final diagnosis, thereby identifying potential resources for improvement. METHODS: Retrospective data from the electronic medical charts of 325 patients

Addressing ignored and neglected: Nonvenereal vulvar dermatoses.

INTRODUCTION: Vulval dermatoses may present with varied manifestations ranging from asymptomatic to chronic disabling conditions. Due to the warm, moist, and frictional environment of the vulva and its frequent exposure to irritating substances such as urine, feces, repeated use of napkins, and vaginal secretions, the classic appearance of common dermatoses is modified. AIM AND OBJECTIVE: To study the pattern of nonvenereal disease of female external genitalia. MATERIALS AND METHODS: The study involved 50 female participants over a 1-year period, with informed consent obtained from all. A co

Gynecologic Cancers: Vulvar Cancer.

Vulvar cancer is one of the rarer gynecologic cancers. Previously, it was thought to affect only postmenopausal women, but a recent epidemiologic review reveals that younger premenopausal females are being affected at higher rates because of human papillomavirus infection. Family physicians are uniquely positioned as a common initial point of contact for many reproductive-age patients and can provide valuable patient education on the need for human papillomavirus vaccination and importance of addressing risk factors early. Early and prompt recognition of vulvar pathology is crucial because the
