The impact of vaginal bromocriptine on reducing pain and menstrual bleeding in women with adenomyosis: a randomized controlled trial.

Bromocriptine, a dopamine receptor agonist that suppresses prolactin secretion from the pituitary gland, is an inexpensive and widely accessible medication with few adverse effects. The study aim was to assesse the efficacy of vaginal bromocriptine in reducing pelvic pain, menstrual bleeding, and cycle irregularities among patients with adenomyosis. In this randomized controled trial, 64 women diagnosed with adenomyosis were randomly assigned to either an intervention group receiving vaginal bromocriptine or a routine-treatment group. The intervention consisted of 5 mg bromocriptine administer

Analysis of Hirsutism as a Symptom of Ihtibās al-Tamth (Amenorrhea) and its Relation to Polycystic Ovary Syndrome.

BACKGROUND: Hirsutism, defined as excessive terminal hair growth in females in a male pattern, is a significant clinical manifestation of increased androgen. Unani scholars such as Hippocrates, Haly Abbas, Rhazes, and Avicenna linked hirsutism with menstrual irregularities, specifically Ihtibās al-Tamth (Amenorrhea). Modern medicine also associates hirsutism with hyperandrogenism, a hallmark of polycystic ovarian syndrome (PCOS), among other conditions. OBJECTIVE: This study aims to explore the understanding of hirsutism from both Unani classical literature and modern medical perspectives, pa

Evaluation of Heavy Menstrual Bleeding in Adolescents.

OBJECTIVE: Heavy menstrual bleeding (HMB) in adolescents often manifests as “excessive bleeding” and may result in acute anemia requiring emergency treatment. The aim of this study was to evaluate the diagnostic and management options for adolescents with HMB. METHODS: Retrospective data were collected from patients’ medical records. Adolescents were classified based on the degree of anemia: Group 1 included patients with hemoglobin (Hb) levels of <8 g/dL; Group 2, Hb levels of 8-10 g/dL; Group 3, Hb levels of 10-12 g/dL; and Group 4, Hb levels of ≥12 g/dL. The impact of nutrition, exercise, and pharmacotherapy on menstrual health in adolescents with overweight and obesity.

BACKGROUND: Menstrual health is a critical marker of physiological and psychological well-being in adolescents. While undernutrition and energy deficiency are well-known causes of menstrual disruption through hypothalamic hypogonadal suppression, the impact of overweight and obesity on menstrual physiology is an increasing clinical concern as adolescent obesity rates continue to rise globally. OBJECTIVES: This review synthesizes current evidence on how excess adiposity, diet quality, physical activity, insulin sensitizers, and weight-modifying medications affect menstrual health in adolescent

Variability of menstrual cycles by age, polycystic ovary syndrome, and early-life cycle irregularity in the apple Women's Health Study.

BACKGROUND: Polycystic ovary syndrome is a common endocrine disorder, characterized by oligomenorrhea and androgen excess. Only a few studies have addressed the natural history of menstrual cycles among women with polycystic ovary syndrome and/or irregular cycles, most with limited sample size and homogenous populations. OBJECTIVE: We describe age-related differences in menstrual cycle length and regularity for those with and without a diagnosis of polycystic ovary syndrome. STUDY DESIGN: We included participants in the Apple Women's Health Study, a digital US cohort, who consented and enrol

Association of heavy menstrual bleeding with cardiovascular disease in US female hospitalizations.

BACKGROUND: Heavy menstrual bleeding (HMB) is a common menstrual disorder associated with multiple risk factors of cardiovascular disease (CVD) in women. In addition, HMB is often present with irregular menstruation (IM) which is a risk factor for CVD outcomes. However, the relationship between HMB and CVD outcomes is unexplored in the presence or absence of IM. We determined the association of HMB with multiple CVD outcomes using a nationally representative sample of female hospitalizations in the US. METHODS: All hospitalizations of females with HMB diagnosis and normal menstrual cycles fro

Clinical Phenotypes of PCOS: a Cross-Sectional Study.

This cross-sectional study examines the Doi-Alshoumer PCOS clinical phenotype classification in relation to measured clinical and biochemical characteristics of women with polycystic ovary syndrome (PCOS). Two cohorts of women (Kuwait and Rotterdam) diagnosed with PCOS (FAI > 4.5%) were examined. These phenotypes were created using neuroendocrine dysfunction (IRMA LH/FSH ratio > 1 or LH > 6 IU/L) and menstrual cycle status (oligo/amenorrhea) to create three phenotypes: (A) neuroendocrine dysfunction and oligo/amenorrhea, (B) without neuroendocrine dysfunction but with oligo/amenorrhea, and (C)

Menstrual Disorders Related to Eating Disorders.

Eating Disorders (ED) are associated with multiple physical complications that strongly affect the physical health of these young and fragile patients and can also cause significant mortality, the highest among psychiatric pathologies. Among the various organic complications, albeit still little known, the gynecological implications, up to infertility, are very widespread. Both among adolescent and adult patients, gynecological symptoms can be very widespread and range from menstrual irregularities to amenorrhea, from vaginitis to ovarian polycystosis, up to complications during the gestationa

Relationship among Nutritional Intake, Anxiety, and Menstrual Irregularity in Elite Rowers.

There is evidence showing that excessive mental stress is detrimental to the menstrual period, and it is known that many elite athletes are highly susceptible to mental anxiety. This study investigated the nutritional intake and mental anxiety of 104 relatively young elite endurance athletes aged 16 to 23 years and used a multiple logistic model to examine the factors that might be related to menstrual irregularity. Calcium intake was marginally associated with the occurrence of menstrual irregularities (odds ratio = 1.004, p = 0.030), whereas there were strong associations between body mass o

Heavy Menstrual Bleeding in Adolescent: Normal or a Sign of an Underlying Disease?

Heavy, and often irregular, menstrual bleeding (HMB) is a common gynecologic complaint among adolescents. During the first few post-menarcheal years, anovulatory cycles related to immaturity of the hypothalamic-pituitary-ovarian axis are the most common etiology for abnormal uterine bleeding and should be considered as a part of normal pubertal development rather than a disease. If an already regular menstrual cycle becomes irregular, secondary causes of anovulation should be ruled out. Inherited and acquired bleeding disorders, such as von Willebrand disease, and quantitative and qualitative

Associations of Menstrual Cycle Characteristics Across the Reproductive Life Span and Lifestyle Factors With Risk of Type 2 Diabetes.

IMPORTANCE: Menstrual cycle dysfunction is associated with insulin resistance, a key feature early in the pathogenesis of type 2 diabetes. However, the evidence linking irregular and long menstrual cycles with type 2 diabetes is scarce and inconsistent. OBJECTIVES: To evaluate the associations between menstrual cycle characteristics at different points throughout a woman's reproductive life span and risk of type 2 diabetes and the extent to which this association is modified by lifestyle factors. DESIGN, SETTING, AND PARTICIPANTS: This prospective cohort study included 75 546 premenopausal U

Menses Requires Energy: A Review of How Disordered Eating, Excessive Exercise, and High Stress Lead to Menstrual Irregularities.

PURPOSE: Functional hypothalamic amenorrhea secondary to low weight, excessive exercise, and/or high levels of stress is common among young women. Adolescence is a time in development that a positive energy balance is crucial for puberty, menarche, and regular menstruation. Disordered eating and eating disorders are the third most common chronic illness and tend to start during puberty. High-level athletes, specifically young girls participating in ballet, running, gymnastics, and figure skating, are at risk of developing hypothalamic amenorrhea from excessive exercise and inability to meet th

Menstrual patterns and self-reported hirsutism as assessed via the modified Ferriman-Gallwey scale: A cross-sectional study.

OBJECTIVE: Hirsutism, the presence of excess terminal hair in a male pattern, is a clinical marker of androgen excess in women. We used cross-sectional data from a North American preconception cohort study to evaluate the association between menstrual cycle characteristics and hirsutism. STUDY DESIGN: Women aged 21-45 years were recruited to a North American cohort of pregnancy planners. On the baseline questionnaire, participants self-reported menstrual characteristics, which included menstrual regularity, cycle length, bleed length, and bleed heaviness. Participants provided a self-rating o

Perimenarchal air pollution exposure and menstrual disorders.

STUDY QUESTION: What is the association between perimenarchal exposure to total suspended particulate (TSP) in air, menstrual irregularity phenotypes and time to menstrual cycle regularity? SUMMARY ANSWER: Exposures to TSP during high school are associated with slightly increased odds of menstrual irregularity and longer time to regularity in high school and early adulthood. WHAT IS KNOWN ALREADY: The menstrual cycle is responsive to hormonal regulation. Particulate matter air pollution has demonstrated hormonal activity. However, it is not known if air pollution is associated with menstrual

Polycystic ovary syndrome in adolescent girls.

PURPOSE OF REVIEW: Polycystic ovary syndrome (PCOS) is a common heterogeneous disorder that appears to have its origins during the peripubertal years. The diagnostic conundrum is that the typical clinical features, irregular menses and acne, occur during normal female puberty. Understanding the physiologic origins and molecular basis of the dysregulated hypothalamic-pituitary-gonadal axis in PCOS is fundamental to interrupting the distinctive vicious cycle of hyperandrogenism and chronic anovulation. RECENT FINDINGS: Newer ultrasound technology with better spatial resolution has generated con
