Postpartum health-seeking behaviours, awareness and experiences of Genitourinary Syndrome of Lactation.

INTRODUCTION: The hypo-oestrogenic state of the postpartum period leads to bothersome genitourinary symptoms in some lactating women. These symptoms have been named 'Genitourinary Syndrome of Lactation' (GSL), and include vaginal dryness, atrophy, urinary incontinence (UI), urinary tract infection (UTI), delayed wound healing and dyspareunia. Despite its prevalence, the experiences, awareness, quality of life, treatments offered and health-seeking behaviours of mothers with GSL in Australia are unknown. METHODS: A national cross-sectional survey was conducted in Australia, assessing health-se

Sexual function after urogynecologic surgery.

PURPOSE OF REVIEW: The purpose of this review is to summarize and highlight contemporary studies evaluating sexual function after urogynecologic surgery. As nearly one in five women will undergo surgery for pelvic organ prolapse (POP) or urinary incontinence, it is critical to understand the impact of surgery on postoperative sexual function. This review includes a discussion of reconstructive surgeries for POP, uterine-preserving techniques, mesh-based prolapse repairs, and anti-incontinence procedures for both stress urinary incontinence (SUI) and urge urinary incontinence (UUI) and the asso

Patient-reported Outcomes of Penile Inversion Vaginoplasty Using Post-Affirming Surgery Form and Function Individual Reporting Measure: A Retrospective Single-institution Study.

BACKGROUND: Penile inversion vaginoplasty (PIV) is a common gender-affirming procedure for transgender women, but validated, transgender-specific patient-reported outcomes remain limited. This study evaluated long-term outcomes after PIV using the Post-Affirming Surgery Form and Function Individual Reporting Measure (AFFIRM), a validated instrument assessing appearance, urologic, and gynecologic domains. METHODS: We conducted a single-institution retrospective cohort with a cross-sectional, anonymous AFFIRM survey distributed in 2024. Demographics, perioperative outcomes, and complications we

Initial Development of a Brief Mind-Body Sexual Well-Being Program for Female Colorectal and Anal Cancer Survivors: A Two-Phase Study of Survivor Needs and Treatment Preferences.

BACKGROUND: Many female colorectal and anal cancer survivors experience distressing changes to sexual well-being (e.g., dyspareunia, low libido) after treatment. Though mind-body programming has previously shown benefit for women's sexual well-being, no existing program is available to address women's sexual well-being specifically following colorectal and anal cancer treatment. OBJECTIVE: This two-phase study describes initial development of a group mind-body program for women's sexual well-being in colorectal and anal cancer survivorship. METHODS: Phase 1 includes individual interviews wit

Redefining genitourinary syndrome: A proposal to expand the GSM nomenclature to encompass the full spectrum of hypoestrogenic and hypoandrogenic urogenital disorders.

The current term "Genitourinary Syndrome of Menopause" (GSM) was introduced to replace "vulvovaginal atrophy" and to better describe a constellation of symptoms affecting the genital and lower urinary tracts due to hypoestrogenism. However, its association with menopause excludes a growing and diverse population of individuals who experience identical symptoms due to non-menopausal etiologies, including perimenopause, lactation, hormonal contraception, cancer therapies, gender-affirming hormone therapy, and other medications. This paper proposes an expanded and updated nomenclature-Genitourina

Hyperbaric oxygen treatment and toll-like receptor 5 in radioprotection of epithelial cells.

BACKGROUND: Irradiation of tumours in the pelvic area may lead to adverse effects such as vaginal fibrosis and dyspareunia. We investigated the radioprotective effects of hyperbaric oxygen (HBOT) and toll-like receptor 5 (TLR5) in epithelial cells. METHODS AND MATERIALS: Endothelial cells (HUVEC), urothelial cells (UROtsa), and cervical epithelial cells (HeLa) were irradiated with 6 Gy and subsequently underwent HBOT. Cells were counted and qPCR was performed on HeLa for the expressions of TLR5, downstream signalling molecules, oxidative stress, and cytokines. The effects of the TLR5 agonist

Quality of life in women with endometriosis in Portugal: a cross-sectional study on the roles of social support and sexual satisfaction.

BACKGROUND: Endometriosis, a chronic and often painful gynecological condition characterized by the presence of endometrial-like tissue outside the uterine cavity, significantly impacts multiple facets of women's lives, including their sexual health, psychological well-being, and overall quality of life. The heterogeneous presentation of symptoms, ranging from dysmenorrhea and dyspareunia to infertility, exerts not only a physical toll but also an emotional and social burden. This cross-sectional study explored how social support contributed to the quality of life in Portuguese-speaking women

Integrating IVF and Surgical Management in Endometriosis-Associated Infertility: A Review.

Endometriosis is a chronic, debilitating condition characterized by heterogenous clinical manifestations. It has a prevalence of 5-10% in women of reproductive age, and 30-50% of individuals with endometriosis are affected by infertility. There are multiple mechanisms through which endometriosis can lead to infertility: reduced ovarian reserve, distorted pelvic anatomy, chronic inflammation, altered immunity, dyspareunia, and/or altered hormonal and cell-mediated endometrial function. The revised American Society for Reproductive Medicine (rASRM) classification was one of the initial methods t

Revisiting the role of danazol in the contemporary management of endometriosis-related pain: a systematic review.

INTRODUCTION: Danazol is a synthetic steroid approved in 1976 for the treatment of endometriosis-associated pain. Despite its established efficacy, its clinical use has markedly declined due to androgenic and metabolic adverse effects, prompting reconsideration of its role in current practice. METHODS: A systematic review was conducted according to PRISMA guidelines and registered in PROSPERO. Six electronic databases were searched up to June 2025 for studies evaluating oral danazol in women with endometriosis. Thirty studies (1,486 patients), including 22 randomized trials, were included. Reframing endometriosis care in low-resource settings: a pragmatic 5R framework for recognition, rule-out, relief, referral and review.

Endometriosis is a chronic inflammatory gynecological condition associated with severe dysmenorrhea, chronic pelvic pain, deep dyspareunia, cyclic bowel or urinary symptoms and infertility. It is also linked to psychological morbidity, reduced quality of life and lost economic productivity. In low-resource settings, diagnostic and therapeutic delays often reflect pathway failure rather than specialist scarcity alone: symptoms are commonly normalized, first-contact recognition remains weak, differential diagnosis is inconsistent, referral pathways are poorly governed and structured follow-up is

Exploring Short-Acting Hormonal Contraceptives in Endometriosis Care-A Narrative Review and Evidence-Based Practical Considerations.

BACKGROUND: Endometriosis is a chronic, estrogen-dependent inflammatory condition affecting approximately 10% of women of reproductive age and manifesting with pelvic pain, dysmenorrhea, dyspareunia, fatigue, and infertility. First-line treatment in most patients is hormonal therapy, and among the available options, contraceptives are frequently used. Short-acting contraceptives (SACs) have been employed off-label to manage endometriosis-related symptoms, yet comparative evidence regarding optimal formulations and regimens remains limited. This narrative review, part of the Agnodice Project, a

Three- and Four-Dimensional Pelvic Floor Ultrasound in Chronic Pelvic Pain: Evidence, Technical Considerations, and a Proposed Framework.

Chronic pelvic pain (CPP) is a heterogeneous condition in which visceral, musculoskeletal, neuropathic, and pain-processing contributors often overlap. Pelvic floor ultrasound can depict morphology and movement, but its CPP-specific evidence base remains limited. This targeted narrative review summarizes direct CPP evidence; evidence from endometriosis, dyspareunia, myofascial pain, and methodological pelvic floor cohorts; and the technical distinctions among transperineal/translabial and transvaginal routes, static three-dimensional (3D) volumes, real-time four-dimensional (4D) assessment, mu
