Incidence of uterine cancer after supracervical hysterectomy.

BACKGROUND: Supracervical hysterectomy (SCH), commonly performed during sacrocolpopexy, involves removal of the uterine corpus, leaving the cervix in-situ. Significant anatomic variation exists in remnant tissue, which may develop into a uterine malignancy. No prior studies have sought to describe the future risk of developing de novo uterine cancer after SCH with benign pathology. This study aims to describe the incidence rate of uterine cancer following SCH performed for benign indication. METHODS: Retrospective cohort study of patients who underwent SCH between June 2000 and December 2022

A rare incidental finding of an occult ovarian Sertoli-Leydig cell tumor presenting with hyperestrogenism in a postmenopausal woman: Diagnostic challenges.

OBJECTIVE: Ovarian Sertoli-Leydig cell tumor (SLCT) is an exceptionally rare subtype of ovarian sex cord-stromal tumor, comprising less than 0.5% of ovarian neoplasms. While SLCT typically manifests in reproductive-age women, occurring in fewer than 10% of cases either prior to menarche or postmenopausally, it often presents with androgen secretion, resulting in virilization. However, occult lesions associated with hyperestrogenism in postmenopausal women are infrequent and may be easily overlooked. CASE REPORT: We present a rare case of SLCT in a 60-year-old postmenopausal woman who experien

An Assessment of Barriers to Care and Health Literacy: A Cross-Sectional Telephone Survey at a Federally Qualified Health Center Women's Clinic.

INTRODUCTION: Persistent barriers to healthcare disproportionately affect marginalized populations, particularly those served by federally qualified health centers (FQHCs), where patients are more likely to be non-white, low-income, and Medicaid-insured. Women in these settings face additional obstacles; missed appointments both reflect structural inequities and increase the risk of adverse health outcomes. METHODS: Patients with missed appointments at the University of California, Irvine Family Health Center’s OBGYN clinic, an FQHC in Santa Ana, were identified through EPIC. Using a convenien

Histopathological Evaluation of Endometrial Patterns in Women With Postmenopausal Bleeding and Their Correlation With Clinical and Radiological Features.

Background Postmenopausal bleeding (PMB) is a common gynaecological complaint and an important clinical indicator of underlying endometrial pathology, including malignancy. The present study aimed to evaluate histopathological patterns in women presenting with PMB and to correlate these findings with clinical and radiological parameters, particularly endometrial thickness (ET). Materials and methods This one-year observational study included 35 postmenopausal women presenting with bleeding per vaginum. Endometrial tissue obtained via dilation and curettage, pipelle biopsy, or hysterectomy was

Endometrial Endometrioid Carcinomas With Signet Ring Cells: Report of a Case Series With Detailed Clinical, Pathologic, and Molecular Analysis.

Signet ring cells are extremely rare in primary endometrial carcinomas with a limited number of reported cases. We report a series of 5 endometrioid-type endometrial carcinomas with signet ring cells, the largest reported series in the literature to date. The patients ranged in age from 53 to 89 yr (mean: 72), and all presented with postmenopausal bleeding. Four of the tumors were FIGO grade 3, and 1 FIGO grade 2, and the FIGO stages were IA, IB, IIIA, IIIB, and IIIC1. The percentage of signet ring cells ranged from 10% to 30% of the tumor, and the signet ring cells were confined to solid area

Retained Lippes Loop Intrauterine Device (IUD) in a Woman With Post-menopausal Bleeding and Chronic Pelvic Pain.

Forgotten intrauterine devices (IUDs), particularly those retained for decades, can lead to complications in postmenopausal women, including bleeding and chronic pelvic pain. The Lippes Loop IUD, widely used from the 1960s to the 1980s, is now rarely encountered. We present the case of a 68-year-old woman with a forgotten Lippes Loop IUD who presented with chronic pelvic pain and postmenopausal bleeding. The patient was uncertain whether her IUD had been spontaneously expelled or removed. During a laparoscopic hysterectomy performed for postmenopausal bleeding, an intra-pelvic Lippes Loop IUD

A Rare Variant of Ovarian Fibroma With Minor Sex Cord Elements: A Unique Expression of Stromal Differentiation.

Ovarian fibroma with minor sex cord elements is a rare and underrecognized histological variant of ovarian stromal tumors. While the bulk of the tumor resembles a classic fibroma, the presence of small foci of sexcord-like cells can create a diagnostic dilemma, particularly in limited samples. This tumor exhibits hormonal production resulting in endometrial hyperplasia, polyp, or even carcinoma. We report the case of a 56-year-old postmenopausal woman who presented with a history of intermittent postmenopausal bleeding and lower abdominal discomfort with primary infertility. There was a slowly

Postmenopausal complete hydatidiform mole: A case report.

Complete hydatidiform mole (CHM) cases occurring in the postmenopausal period are quite rare and may be delayed in diagnosis. In the present study, the case reported is of a 55-year-old postmenopausal woman (menopausal for 5 years) who presented with painless postmenopausal bleeding in March 2025, ongoing for ~1 month and was diagnosed with CHM. Transvaginal ultrasonography showed endometrial thickness of 28-mm and serum β-human chorionic gonadotropin (β-hCG) level of 187,240 mIU/ml. After ultrasound-guided curettage, the diagnosis of CHM was confirmed in histopathological examination. Since t

Temporal Clustering of Endometrial Cancer and Hyperplasia in HRT Users with Unscheduled Bleeding: A Retrospective NHS Cohort Study.

Background: Unscheduled bleeding in postmenopausal women on hormone replacement therapy (HRT) represents a common but poorly characterised clinical challenge. Whilst endometrial cancer affects approximately 9% of women with unexplained postmenopausal bleeding, existing evidence in HRT users is largely restricted to women under 60 years and short-duration regimens, leaving a critical evidence gap in contemporary all-age clinical practice. Whether the same investigative urgency is warranted for HRT users experiencing unscheduled bleeding as for women with unexplained postmenopausal haemorrhage r

Cultural, Societal, and Behavioral Contributors to Delays in Seeking Care for Postmenopausal Bleeding Among Disaggregated Populations of Black Women.

BACKGROUND: Endometrial cancer outcomes differ among Black women when examined by nativity, and timely evaluation of postmenopausal bleeding (PMB), the most common presenting symptom, may contribute to these disparities. METHODS: This qualitative study explored cultural, societal, and behavioral factors shaping PMB appraisal and anticipated care-seeking among US-born Black, Caribbean-born Black, and Haitian Creole-speaking women in South Florida, guided by the Safer-Andersen Model of Total Patient Delay. Ten focus groups were conducted with 55 Black women aged ≥50 years recruited through purp

Transforming endometrial cancer diagnosis: the case for molecular triage in abnormal uterine and post-menopausal bleeding.

Abnormal uterine bleeding is the most common symptom of endometrial cancer, but due to the low prevalence of underlying malignancy (3%), clinicians must distinguish those who can be quickly reassured from those who require urgent diagnosis and cancer treatment. The current diagnostic pathway is limited by the low specificity of transvaginal ultrasound, which leads to large numbers of invasive follow-up tests (hysteroscopy and biopsy). Transvaginal ultrasound may also miss almost a quarter of serous cancers, which have the poorest prognoses, and overall performance of this test, particularly sp

Metastatic appendiceal mucinous adenocarcinoma presenting as bilateral ovarian masses mimicking advanced ovarian cancer: a case report.

Appendiceal mucinous adenocarcinoma is a rare gastrointestinal malignancy that may metastasize to the ovaries and closely mimic advanced primary ovarian cancer, creating diagnostic and therapeutic challenges. A 61-year-old postmenopausal woman presented with postmenopausal bleeding, abdominal distension, abdominal pain, and weight loss. Imaging demonstrated bilateral adnexal masses with omental caking and ascites, suggestive of advanced ovarian malignancy. Cancer antigen 125 was elevated, and carcinoembryonic antigen (CEA) levels were markedly increased. She had a history of acute appendicitis

Can machine learning predict endometrial cancer in patients with postmenopausal uterine bleeding?

INTRODUCTION: Postmenopausal bleeding (PMB) is a common clinical symptom, with endometrial cancer (EC) accounting for about 10% of cases. This study aimed to develop machine-learning models to predict EC in women with PMB, supporting risk stratification and optimizing diagnostic pathways. MATERIAL AND METHODS: We retrospectively analysed 617 women who underwent hysteroscopy for PMB at the Meir Medical Center between 2014 and 2023. Demographic, clinical, laboratory, and imaging data were collected. Three supervised machine-learning algorithms - Random Forest, eXtreme Gradient Boosting (XGBoost

Pre-diagnosis symptoms reported among endometrial cancer survivors.

OBJECTIVES: Timely recognition of pre-diagnosis symptoms plays a key role in early diagnosis of endometrial cancer (EC). Symptom profiles may vary by conditions such as fibroids or obesity which are differentially distributed by race. We describe pre-diagnosis symptom reporting among EC survivors by race, fibroids history, and body mass index (BMI). METHODS: We calculated prevalence ratios (PR) and 95% confidence intervals (CI) for 6 menstrual and 7 non-menstrual self-reported, pre-diagnosis symptoms using Poisson regression. We assessed participants' pre-diagnosis knowledge about postmenopau

Preoperative Ultrasound Nomogram for Differentiating Polypoid Adenomyoma from Endometrial Polyps.

OBJECTIVES: Uterine polypoid adenomyoma (PA) tends to persist, recur, and carry a high risk of malignancy. However, preoperative distinction between PA and endometrial polyp (EP) remains challenging in clinical practice, often leading to suboptimal therapeutic decisions. This study aimed to compare the clinical and ultrasonographic characteristics of PA and EP and to develop a nomogram for accurate preoperative diagnosis. METHODS: We retrospectively identified women with pathologically confirmed PA diagnosed at Peking Union Medical College Hospital between January 2017 and April 2024. Patient
