OBJECTIVE: To develop and validate a diagnostic nomogram model for predicting atypical endometrial hyperplasia (AEH) and endometrial carcinoma (EC) in women aged 40-60 years with abnormal uterine bleeding (AUB), incorporating detailed bleeding patterns and clinical parameters.
METHODS: This retrospective cohort study included 1,920 patients aged 40-60 years with AUB who underwent hysteroscopic evaluation across four hospital branches from 2021 to 2024. Variables were screened via univariate logistic regression followed by LASSO regression. A multivariate logistic regression model was construc

A 51-year-old woman presented with two years of postmenopausal spotting, mainly postcoital. Although vaginal atrophy was considered, prior use of vaginal estrogen at another center had not improved her symptoms. She had a history of persistent human papillomavirus (HPV) 16 infection and abnormal cytology. Initial colposcopy showed CIN 1 but one year later, a biopsy revealed CIN 2, and loop electrosurgical excision procedure (LEEP) was performed with negative margins. At 6-month follow-up, HPV positivity and low-grade squamous intraepithelial lesion persisted, with CIN 2 on colposcopy. Despite

Adenofibroma of the cervix is an extremely rare benign Mullerian mixed tumor, which contains both benign epithelial and mesenchymal tissue. The WHO 2020 classification of the female genital tract does not even include adenofibroma as a separate entity. A postmenopausal lady in her mid-50s presented with bleeding per vagina. On examination, there were multiple cervical polyps. Histopathology of cervical polyps suggested adenofibroma of the cervix due to the absence of mitotic figures and cellular atypia. Sometimes even well-differentiated adenosarcomas can be misdiagnosed as adenofibroma. Immun

Postmenopausal bleeding is an alarming symptom that needs careful consideration. Endometrial polyps co-existing in the background of endometrial hyperplasia/endometrial cancer is well-established. However, it has been very sparsely reported in the background of atrophic endometrium. The authors report a unique case of concurrent atypical endometrial polyp with cervical intraepithelial neoplasia (CIN) in the background of an atrophic endometrium. A 53-year-old multiparous, postmenopausal lady of Asian ethnicity presented with recurrent episodes of bleeding per vaginum. She did not give any hist

OBJECTIVE: To evaluate the presence of gynecologic pathologies accompanying adenomyosis among the hysterectomy specimens performed for various gynecologic indications.
METHODS: This multicenter retrospective cohort study included women who underwent hysterectomy for various gynecologic indications and were diagnosed with adenomyosis at tertiary care centers. Eligible women were classified into two groups based on histopathologic results: Group 1-patients with only adenomyosis (n = 102) and Group 2-patients with adenomyosis and other gynecologic pathologies (n = 1353). Demographic and clinical

Hysterectomy is the most common gynaecological surgery done in females worldwide as it provides definitive cure to wide range of gynaecological diseases, both benign and malignant. The indications to perform this major surgery should always be justified and pathology should be proved. Histopathological examination is essential to justify the appropriateness of the hysterectomy. The cross sectional descriptive type of observational study was carried out to correlate preoperative indications of abdominal hysterectomy with histopathological findings. This study was conducted in the department of

BACKGROUND: Professional society guidelines endorse the use of ultrasound to evaluate an initial episode of postmenopausal bleeding, with endometrial sampling reserved for patients with an endometrial thickness exceeding 4 mm. However, the data supporting this ultrasound-based approach are derived from cohort studies with minimal racial and ethnic diversity.
OBJECTIVE: This study aimed to evaluate rates of adequate endometrial visualization, prompt guideline-concordant care, and identify risk factors for lack of timely indicated sampling in a multi-institutional cohort of patients who underwe

INTRODUCTION: Minimal serous uterine cancers (MUSCs) represent a distinct subset of endometrial malignancies characterized by early-stage presentation and unique histopathological features. These tumors typically manifest as intraepithelial or superficial serous carcinomas confined to endometrial polyps without myometrial invasion or lymphovascular involvement. MUSCs are predominantly diagnosed in postmenopausal women and are associated with favorable prognosis.
CASE PRESENTATION: The present case report briefs the finding of a 60-year-old woman presenting to the gynecological outpatient depa

Papillary serous carcinoma is a rare and aggressive subtype of endometrial cancer with a strong predilection for extrauterine spread. Although metastasis typically involves the peritoneum, lymph nodes, and lungs, isolated splenic metastases are exceedingly rare. We report the case of a 66-year-old woman initially diagnosed with papillary serous endometrial carcinoma following an episode of postmenopausal bleeding. She underwent surgical staging with hysterectomy, bilateral salpingo-oophorectomy, and pelvic lymphadenectomy, followed by adjuvant chemotherapy. Two years later, she presented with

Villoglandular adenocarcinoma (VGA) of the cervix is a rare tumour with very few reports of cytological diagnosis on a cervical smear. Since the usual prognosis of VGA is more favourable than that of a conventional cervical adenocarcinoma, a pre-operative diagnosis is essential for appropriate therapeutic decisions. We report the clinical, cytological, and histopathological features of a case of VGA in an elderly woman. A 60-year-old female presented with postmenopausal bleeding and was found to have a hypertrophied cervix. Conventional smears from the cervix demonstrated features of atypical

BACKGROUND: Endometrial tuberculosis (TB) is a rare form of extrapulmonary TB, particularly uncommon in postmenopausal women. Its atypical presentation, characterized by nonspecific symptoms, often leads to misdiagnosis, particularly when it is confused with malignancies. Moreover, peritoneal tuberculosis, although rare, can further complicate the diagnostic process due to its clinical manifestations that resemble those of various cancerous conditions. The coexistence of both endometrial and peritoneal TB in the same patient is particularly unusual and presents a significant diagnostic challen

Endometrial carcinoma (EC) is the most common gynecological cancer among women, and in more than 90% of cases, the initial manifestation of the disease is postmenopausal bleeding. Unfortunately, despite early diagnosis and treatment, EC often recurs. Among the many serum tumor markers studied, human epididymis protein 4 (HE4) and cancer antigen 125 (CA125) show the most promise as tools for EC diagnosis, prognosis, and monitoring.

Primary extranodal marginal zone lymphomas (EMZL) of the fallopian tube are rare. The few published case reports suggest a causal relationship between chronic inflammation and tubal EMZL, with associated abnormal macroscopy noted intraoperatively. We present a woman in her 50s with postmenopausal bleeding and endometrial hyperplasia who underwent a laparoscopic total hysterectomy and bilateral salpingectomy. Macroscopy of the specimen was unremarkable. Microscopy of the right fallopian tube showed dense infiltrate of monomorphous lymphocytes with mild nuclear membrane irregularity. Immunohisto

Background and objective Gynecological disorders are prevalent among elderly women due to ageing and hormonal changes in the post-reproductive period. However, there is limited evidence in Sri Lanka to guide the provision of optimal care for geriatric women with these conditions. Therefore, this study aimed to describe the gynecological disorders among elderly women. Methods A hospital-based descriptive cross-sectional study was carried out among 327 women aged 60 years and above who attended the gynecology and urology clinics at the Teaching Hospital Jaffna, Jaffna, Sri Lanka. An interviewer-

Metastatic melanoma is the most life-threatening skin cancer due to its significant tendency to metastasize and its substantial resistance to therapeutic interventions. Melanomas spread aggressively, particularly to lymph nodes, the brain, and lungs, and can also involve the adrenal gland, liver, bones, and small intestine. Metastasis to the endometrium is exceedingly rare, with only a handful of documented cases. Fluorodeoxyglucose (FDG) positron emission tomography/computed tomography (PET/CT) is an excellent modality for detecting metastases. A meta-analysis showed that PET/CT has superior

The lifetime risk of ovarian cancer is 1 in 75, and it is often diagnosed at an advanced stage due to the lack of effective screening. Epithelial ovarian cancer (EOC) is the most common type of ovarian cancer in older patients, with borderline ovarian tumors (BOTs) accounting for 15% of EOC cases. BOTs are usually of low malignant potential. Since platinum-based chemotherapy has not been shown to improve survival rates, cytoreductive surgery (CRS) is recommended as a treatment option. Additionally, endometriosis affects 10.5% of patients with EOC. It often progresses to clear cell and endometr

Perivascular epithelioid cell tumor of the uterus is a rare mesenchymal neoplasm with variable malignant potential and nonspecific clinical and radiologic features, often mistaken preoperatively for leiomyoma. This report describes the case of a postmenopausal woman with seven months of persistent postmenopausal bleeding, pelvic pain, and symptomatic anemia. Pelvic ultrasound and magnetic resonance imaging demonstrated a markedly enlarged, irregular uterus with multiple masses interpreted as fibroids. The patient underwent total abdominal hysterectomy with bilateral salpingo-oophorectomy for r

Non-islet cell tumor hypoglycemia as a result of insulin-like growth factor (IGF)-2 secretion is rare. A 59-year-old woman was referred with postmenopausal bleeding due to endometrial hyperplasia. Serum testosterone, estradiol, and adrenal androgens were elevated with suppressed gonadotropin concentrations. Cross-sectional imaging demonstrated a large left adrenal mass. The patient subsequently presented acutely with hypoglycemia. During a supervised fast, symptomatic hypoglycemia occurred within 5 hours. Serum samples drawn prior to hypoglycemia correction revealed an elevated IGF-2:IGF-1 rat

BACKGROUND: Crimean-Congo hemorrhagic fever (CCHF) is a common cause of tick-borne hemorrhagic infections. The aim of this study is to report a case of a postmenopausal woman presenting with vaginal bleeding.
CASE REPORT: A 52-year-old woman was admitted to the emergency department with high fever, weakness, loss of appetite, and vaginal bleeding. The patient was engaged in animal husbandry and had no history of tick bites. She had entered menopause 1 year prior. Laboratory tests revealed bicytopenia, abnormal kidney and liver function tests, and impaired coagulation parameters. The diagnosis
