The impact of fresh versus cryopreserved testicular sperm on in-vitro fertilization outcomes in couples with elevated sperm DNA fragmentation: a retrospective cohort study.

BACKGROUND: Elevated sperm DNA fragmentation (SDF) is associated with male factor infertility, recurrent pregnancy loss, and poor reproductive outcomes. An option for management includes the utilization of testicular sperm; however, the comparative reproductive efficacy of fresh versus cryopreserved testicular sperm men with elevated SDF remains unclear. Therefore, the objective of this study was to identify differences in reproductive outcomes between fresh and cryopreserved testicular sperm in patients with elevated SDF who are undergoing intracytoplasmic sperm injection (ICSI) after testicu

Machine learning-based prediction of IVF/ICSI outcomes in male factor infertility highlighting couple-level BMI.

BACKGROUND: Most clinical prediction models for assisted reproductive technology focus primarily on female ovarian reserve markers and often under-represent male factors and the metabolic status of both partners. Additionally, traditional parametric models may have limited ability to capture nonlinear patterns within reproductive data. This study aimed to develop and validate a machine learning (ML)-based model to predict clinical pregnancy outcomes in couples with male factor infertility undergoing IVF/ICSI, and to explore model interpretability using Shapley Additive exPlanations (SHAP). Metabolomic analysis of follicular fluid in women with unexplained infertility.

Unexplained infertility (UI) affects ∼10% of infertile couples, yet standard diagnostic protocols fail to identify a cause. Follicular fluid (FF), which supports oocyte development, contains metabolites that may reflect underlying molecular disturbances. In this exploratory study, we investigated the FF metabolome of women with UI and compared it with controls to explore metabolic alterations associated with UI. FF was collected during oocyte retrieval from 20 women undergoing IVF (ten with UI, ten with male factor infertility), matched for age, BMI, stimulation, and fertilization protocols. M

A comprehensive review about sperm-oocyte interactions and key activation factors of fertilization in mammals.

Mammalian fertilization involves the migration of spermatozoa through the female reproductive system. Early embryonic development is a consequence of several steps and signaling pathways being activated, as well as biochemical and morphological modifications of spermatozoa that enable them to penetrate the membrane of mature oocytes. There are some crucial steps known to clearly explain the process of fertilization, starting with hyperactivation of spermatozoa, mutual recognition, and binding of gametes mediated by receptors located on the surface membranes of both gametes. The final step is f

Defining associated factors for total fertilization failure and embryo development arrest in intracytoplasmic sperm injection cycles.

BACKGROUND: Total fertilization failure (TFF) and embryo developmental arrest (EDA) remain clinically challenging outcomes in intracytoplasmic sperm injection (ICSI) cycles. Although ICSI is widely and effectively used to overcome fertilization disorders, fertilization failure and embryo development arrest continue to occur in a subset of patients. This study aimed to identify baseline clinical characteristics and ovarian reserve parameters associated with the risk of TFF and EDA. METHODS: In this retrospective cohort study, 1,846 ICSI cycles performed between January 2016 and December 2019 at

Long-term patency and pregnancy after vasovasostomy: a comprehensive review.

BACKGROUND: Vasovasostomy (VV) is often pursued by men seeking natural conception after vasectomy. Microsurgical VV is associated with high patency rates (~95%) and moderate pregnancy rates (~40%), according to existing literature. However, long-term outcome data remain limited. This review evaluates patency and pregnancy rates at ≥12 months following microsurgical VV. MATERIALS AND METHODS: A comprehensive literature search was conducted via PubMed using the terms "vasovasostomy," "patency rate," and "pregnancy rate." Studies were included if they reported patency or pregnancy outcomes ≥12 m

Diet, lifestyle factors, and human fertility: what we know, what we wish we knew, and what we may never know.

Persistent downward trends in birth rates and semen quality across the globe have understandably raised interest in understanding the key drivers of human fertility. Among these, interest in factors that are potentially modifiable and subject to an important degree of volitional control, such as diet, exercise, or substance use, has been considerable. Unfortunately for those interested in this topic, either to guide their own behavior or to provide medical advice to those seeking care, the level of detail sought is often misaligned with the level of evidence available at any point in time. Sperm capacitation ability, as evaluated by Cap-Score™, has little impact on ICSI success.

PURPOSE: Men with low capacitation ability have reduced conception through intercourse, IUI, and conventional IVF and are often referred for ICSI. This study evaluated whether reduced capacitation ability, as measured by Cap-Score, impacts ICSI outcomes. METHODS: Retrospective review of 199 men undergoing ICSI at a single clinic (2018-2023). Semen analysis included Cap-Score. Fertilization and euploidy rates were compared for low vs normal Cap-Scores and abnormal vs normal semen analyses. Following frozen embryo transfer, Cap-Scores and semen analysis outcomes were compared between positive a

"Infertility is mostly for women": University students' knowledge and perceptions of risk factors associated with infertility in Ghana.

Understanding knowledge and perceptions of infertility risk factors is crucial for promoting fertility preservation. While prior studies have explored infertility awareness among university students based on gender, limited research examines both genders within a single study. This study investigates university-aged males' and females' knowledge and perceptions of infertility risk factors, identifying gender-specific risks. Conducted at the University of Ghana, the study employed a descriptive cross-sectional qualitative design, involving semi-structured interviews with 21 male and 18 female s

Male Factors Associated With Success Following Assisted Reproduction Technology Among Infertile Couples in Ghana.

Assisted reproductive technologies (ARTs) are increasingly utilized in Ghana, as treatment options for couples with infertility, including male factor infertility. While extensive research has focused on female determinants of ART success, comparatively fewer studies have systematically examined the contribution of male partner characteristics beyond routine semen analysis to clinical pregnancy outcomes following ART. The paucity in research is more pronounced in Ghana. This was a cross-sectional study, conducted at the Chosen Hospital and Fertility Centre between January 2024 and March 2025.

Association of sperm DNA fragmentation index with semen quality and ART outcomes: real-world evidence from a retrospective cohort.

BACKGROUND: Sperm DNA fragmentation index (DFI) has emerged as a potential marker of male fertility, but its clinical value in assisted reproductive technology (ART) remains debated. This study aimed to investigate the association between sperm DFI and conventional semen parameters, embryological outcomes, and pregnancy outcomes in ART, using a large real-world dataset. RESULTS: A total of 1,784 ART cycles performed at a single center between January 2020 and January 2022 were analyzed. Patients were categorized into normal (< 30%) and high (≥ 30%) DFI groups. Compared with the normal DFI grou

The role of sperm DNA fragmentation testing in predicting IVF and PGT-A outcomes.

This retrospective study evaluated whether sperm DNA fragmentation (SDF), measured by DNA fragmentation index (DFI), predicts IVF outcomes in 124 couples undergoing IVF with PGT-A (Preimplantation Genetic Testing for Aneuploidy) between 2015 and 2023. SDF was assessed using the Halosperm kit and categorized into low (≤ 15%, n = 81), medium (15-30%, n = 34), and high (≥ 30%, n = 9) DFI groups. Fertilization was performed via IVF or ICSI, and embryos were analyzed with PGT-A using next-generation sequencing. High DFI was associated with advanced paternal age, reduced motility, lower fertilizatio

Practice patterns for expanded carrier screening among reproductive endocrinology and infertility physicians.

PURPOSE: Expanded carrier screening (ECS) is a commonly used preconception screening test, especially in fertility practices given available interventions to mitigate risks of genetic disease among offspring. However, little is known about the implementation of ECS by Reproductive Endocrinology and Infertility (REI) physicians. This study assessed ECS practices among REI physicians in the United States (U.S.) and physician or clinic factors associated with requiring patients' ECS to result prior to initiating any fertility treatment (in vitro fertilization or intrauterine insemination). Impact of fertility treatments on headache disorders: a systematic review with an overview of treatment modalities.

INTRODUCTION: Infertility represents a global health concern and assisted reproductive technologies expose an increasing number of women and men to intensive hormonal interventions. At the same time, migraine and other headache disorders are highly prevalent in the reproductive age group and hormonal fluctuations are well-established headache triggers. The impact of fertility treatments on headache remains unclear and guidance for clinical management is lacking. METHODS: This systematic review followed PRISMA guidelines and was pre-registered in PROSPERO. PubMed and Embase were searched up to
