Peripheral Nerve Stimulation Reduces Opioid Use and Facilitates Early Ambulation Following Fibular Flap Harvest: A Retrospective Study.

BACKGROUND: Fibular free flap (FFF) reconstruction is the gold standard for mandibular and maxillary defects, but donor-site pain remains challenging. Multimodal analgesia (MMA) relies on opioids, which risk dependence, systemic side effects, and delayed recovery. Regional nerve blocks reduce pain but have inconsistent efficacy owing to variable drug metabolism, potential effects on flap perfusion, and motor blockade. Temporary percutaneous peripheral nerve stimulation (TPNS) is a promising alternative that selectively modulates sensory pathways while preserving motor function. METHODS: This

Advance care planning in people with intellectual disabilities, more than 'to resuscitate or not to resuscitate': A qualitative interview study with general practitioners and family caregivers.

BACKGROUND: Advance care planning for people (ACP) with intellectual disabilities requires timely and carefully communication between family caregivers and general practitioners. OBJECTIVES: Despite its recognised importance, little is known about how these stakeholders experience advance care planning and decision-making processes, particularly regarding goals of care and do not resuscitate (DNR) orders, highlighting a critical gap in ensuring person-centred palliative care for this population. METHODS: A qualitative design was used to investigate how Belgian general practitioners and famil

A Bridge to Comfort: Nurses Driving Early Palliative Consults in the Intensive Care Unit.

This project established a protocol that enabled all intensive care unit (ICU) patients to receive a palliative care (PC) screening. Those who met criteria received timely access to PC in the ICU setting. Delayed access to PC can result in untreated symptoms, lack of understanding of care preferences, unwanted invasive procedures, and underutilization of hospice care. A Plan-Do-Study-Act framework was utilized to establish this protocol. The multidisciplinary team sought to increase PC consults in the ICU by 30% using a nurse-driven standardized screening tool. The PC Referral Criteria establi

ALYREF binds to poly-ADP-ribose at double strand breaks impairing repair in tumoral cells.

DNA-RNA hybrids cause genome instability by promoting transcription-replication conflicts. However, as they are stimulated at double-stranded breaks (DSBs), they can also impact DSB repair. We investigated whether the RNA chaperon ALYREF and its RNA helicase partner UAP56/DDX39B, both of which regulate RNA biogenesis and R-loops, also act at DSBs. We found that ALYREF, but not UAP56/DDX39B, is recruited to DSBs. Recruitment is partially dependent on transcription and hybrids, and fully dependent on polyADP-ribose (PAR). Consistently, we show that ALYREF, but not UAP56/DDX39B, binds PAR in vitr

Reactive diffusion of selenium anions into clay-rocks: A sorption in batch and through-diffusion investigation.

Selenium-79 is a critical radionuclide in the safety assessment of high-level radioactive waste in deep geological repositories in clay-rich formations, due to its long half-life and speciation. It exhibits three relevant oxidation states in anionic forms: selenate(+VI), selenite(+IV), and selenide(-II) whose mobility through clay-rocks needs to be evaluated. To this end, batch and diffusion experiments were performed in the French Callovo-oxfordian clay-rocks under anoxic conditions and at ultra-trace concentrations, using the 75Se isotope. Our findings reveal that selenate is weakly retained

Antifungal potential of Capsicum annuum var. glabriusculum on Moniliophthora roreri in response to fruit maturity and shading during fruit growth.

Moniliophthora roreri has significantly impacted cacao production in Latin America, including Mexico. Extracts of Capsicum annuum can inhibit its growth, although their effectiveness depends on fruit maturity and environmental conditions. In this study, the in vitro efficacy of methanolic extracts of immature and mature fruits of C. annuum var. glabriusculum from plants grown under 0, 35, and 70% shade was evaluated for its ability to inhibit mycelial growth (MG) and sporulation of M. roreri. The most effective treatment was selected and used in a field trial involving young fruits of three ca

Influence of x-ray beam quality on retrospective bone dose estimation in a murine model: experimental investigation of EPR signal kinetics and computational modeling.

In the context of radiological accidents, retrospective dosimetry methods such as electron paramagnetic resonance (EPR) spectroscopy and Monte Carlo simulation are used for dose reconstruction. EPR analysis of bone biopsies allows experimental assessment of absorbed dose. However, several factors may affect EPR dose estimation. The time delay between irradiation and sampling can lead to underestimation of absorbed dose to bone, and the influence of beam quality needs investigation. Therefore, this study aims to evaluate the influence of X-ray beam quality on bone EPR signal kinetics and the ab

Metabolic features of enantiornithines (Avialae: Ornithothoraces): evidence supporting a semi-precocial/precocial developmental trajectory.

The enantiornithes, a group of Mesozoic birds, have morphological features suggesting a precocial mode of development; however, their detailed physiological characteristics, particularly hatchling metabolic rates, have yet to be explored. Here, we retrodicted metabolic rates for four Lower Cretaceous enantiornithines: one hatchling, one adult from Las Hoyas, one early post-hatching juvenile and one post-juvenile individual from the Jehol Biota, using phylogenetic comparative analyses and a comprehensive dataset of extant birds spanning the altricial-precocial spectrum. By examining vascular de

Long-term episodic memory alterations and the role of perceived stress in post-COVID-19 condition.

Post-COVID-19 condition (PCC) is frequently associated with cognitive impairments, but the integrity of long-term episodic memory remains poorly understood. This study examined immediate, short-term, and very long-term episodic memory in individuals with PCC and evaluated whether emotional disturbances predict long-term recognition outcomes. Forty-one participants (30 PCC; 11 non-PCC) completed a verbal cued-recall task and an immersive virtual reality free-recall object-location memory (OLM) task immediately after learning and the following day. Nine months later, three recognition tasks (ver

Additive Manufacturing of Metabolically Active Living Materials with Enhanced Biocontainment.

Additive manufacturing of engineered living materials (ELMs) enables the spatial patterning of living cells or organisms within three-dimensional (3D) polymer matrices. However, effective biocontainment, defined as the ability to retain cells within 3D-printed matrices during the application period, remains a key limitation for ELMs requiring physical confinement. While synthetic biology provides cell-specific biocontainment strategies, materials-based approaches can enable generic cell retention capabilities. Therefore, we applied a materials-based strategy that combines pre-printing formulat

Estimated incidence and number of new cases with prenatal alcohol exposure and fetal alcohol spectrum disorders in Finland 1990-2025.

BACKGROUND: Fetal alcohol spectrum disorders (FASD) arising from prenatal alcohol exposure (PAE) are the leading preventable cause of neurobehavioral disorders. Early pregnancy is particularly vulnerable to ethanol toxicity, yet alcohol use often continues until pregnancy recognition. In Finland, national incidence estimates of PAE and FASD remain limited. OBJECTIVE: To estimate the incidence of any PAE and heavy PAE in Finland between 1990 and 2025, and to model the annual number of children with FASD born in Finland. METHODS: We developed a mathematical modelling framework integrating stud

Design, synthesis, and biological evaluation of isatin-based benzenesulfonamides as carbonic anhydrase II/VII inhibitors with anticonvulsant activity.

Epilepsy remains a challenging neurological disorder, with a significant proportion of patients exhibiting resistance to current antiseizure medications. Herein, we report the rational design, synthesis, and biological evaluation of a new series of isatin-thiazolidine-2,4-dione hybrids bearing a pendant benzenesulfonamide moiety as potential antiepileptic agents targeting epilepsy-associated carbonic anhydrase isoforms II and VII. The molecular design integrates key pharmacophoric features shared by established anticonvulsants, including dual aromatic systems, hydrogen-bond donor/acceptor func

Diffusion of inert and reactive tracers: Overview of the DIR field experiment in the Callovo-Oxfordian clay rock (France).

The geological disposal of radioactive waste is under investigation in several countries, especially in Europe. In this field, clay-rich sedimentary rocks are promising natural barriers to limit and delay the release of radionuclides toward the biosphere, due to their diffusion-driven transport and retention properties. In addition to laboratory studies, field studies in Underground Research Laboratories (URL) represent a significant value to strengthen safety demonstrations. In this context, the DIR experiment (Diffusion of Inert and Reactive tracers) was developed and implanted in the Meuse/

"Something was happening, and on the other hand, it wasn't": Relational knowledge formation in intrafamilial child sexual abuse.

BACKGROUND: Intrafamilial child sexual abuse (IFCSA) frequently occurs in the presence of significant others who face complex challenges in recognizing the abuse. While research has increasingly examined disclosure and meaning-making in IFCSA contexts, comparatively little is known about how survivors come to know, interpret, and sustain knowledge of abuse within relational environments characterized by silencing and non-recognition. OBJECTIVE: This study examines how adult survivors of IFCSA come to know abuse that occurred in the presence of significant others who did not recognize it, and

Crosslinking-Dependent Structure-Transport-Function Relationships in Gelatin-Hyaluronic Acid Hydrogels for Wound-Fluid Management.

Effective wound-fluid management is essential in medical applications, as dressings must absorb, retain, and regulate excess blood, exudate, and other biological fluids. Gelatin-hyaluronic acid (Gel/HA) hydrogels are promising dressing materials because they provide a hydrated, tissue-mimetic environment and combine the biological functions of both polymers. Their performance depends strongly on the crosslinking strategy, which regulates network architecture, fluid transport, and mechanical integrity. Three Gel/HA networks were prepared by physical association (Network I), glutaraldehyde-media
