The epidemiology of ectopic pregnancy: global burden and projections.

BACKGROUND: Ectopic pregnancy (EP) remains a major cause of early-pregnancy mortality, yet global long-term trends and inequalities are poorly characterized. MATERIALS AND METHODS: The age-standardized incidence rate (ASIR), age-standardized mortality rate (ASMR), and age-standardized disability-adjusted life year rate (ASDR) data of EP were sourced from the Global Burden of Disease Study 2021 (GBD 2021). We comprehensively mapped the longitudinal epidemiological trends together with the EP burden across 21 regions and 204 countries between 1990 and 2021. To further assess these trends, joinp

A challenging diagnosis of interstitial ectopic pregnancy confounded by sonographic signs of adenomyosis: a case report.

Interstitial ectopic pregnancies account for 1%-3% of ectopic pregnancies and carry a high risk of morbidity due to delayed diagnosis and potential catastrophic haemorrhage. Sonographic identification is challenging and may be further confounded by distorted pelvic anatomy, including adenomyosis, which can present with myometrial cysts or sub-endometrial microcysts that mimic a gestational sac. We describe a 36-year-old woman presenting with early-pregnancy bleeding whose initial ultrasounds suggested a possible intrauterine pregnancy due to adenomyosis-related cystic changes. Serial serum hum

Development and validation of a nomogram model for predicting postoperative complications of cesarean scar pregnancy based on clinical data.

BACKGROUND/OBJECTIVES: Cesarean scar pregnancy (CSP) is a rare but increasingly prevalent form of ectopic pregnancy, often associated with severe postoperative complications. Current research lacks robust tools to predict these complications. This study aimed to develop and validate a clinical nomogram to assess the risk of postoperative complications in CSP patients using multidimensional clinical data. METHODS: A retrospective cohort of 917 patients diagnosed with CSP between December 2015 and March 2024 was analyzed. Patients were randomly assigned to a training set (n = 689) and a validat

Comparing ovulation induction strategies for unexplained infertility: protocol for a systematic review and network meta-analysis.

INTRODUCTION: Unexplained infertility affects about 30% of couples seeking help for infertility, yet the optimal ovulation induction strategy remains largely unclear. Letrozole, clomiphene citrate and gonadotropins are widely used, alone or in combination, with or without intrauterine insemination (IUI), but evidence of comparative effectiveness and safety is inconsistent. Most reviews are restricted to pairwise comparisons or mixed infertility populations. This protocol describes a systematic review and network meta-analysis (NMA) to compare ovulation induction strategies specifically in unex

Retroperitoneal Ectopic Pregnancy: Technical Management.

BACKGROUND: Retroperitoneal ectopic pregnancy is a rare form of ectopic pregnancy, accounting for less than 1% of all cases. This location carries a high risk of massive hemorrhage due to its proximity to major retroperitoneal vessels. CASE PRESENTATION: A 24-year-old female patient, G1P1, presented with a 2-week history of abdominal pain, which had exacerbated in the last 24 hours, accompanied by fever. Laboratory tests revealed elevated serum β-hCG levels (89,331 IU/L), anemia (Hb 8.1 g/dL), and leukocytosis. Abdominal ultrasonography and computed tomography (CT) identified a heterogeneous

Influence of In Vitro Fertilization and embryo transfer procedures on the risk of ectopic pregnancy.

PURPOSE: Ectopic pregnancy remains the leading cause of death in the first trimester of pregnancy. It is surprisingly more common in cases of in vitro fertilization (IVF) (2-5%) than in spontaneous pregnancy (1-2%). The aim of this study was to analyze the influence of demographic and IVF parameters and embryo transfer methods on the risk of ectopic pregnancy. METHODS: This was a retrospective study including all embryo transfers obtained by IVF with or without intracytoplasmic sperm injection (ICSI), over a nine-year period, and resulting in pregnancy (ßhCG >100 IU/L). The population was sep

The letrozole use in reproductive medicine: Beyond aromatase inhibition - a comprehensive review.

Letrozole and other aromatase inhibitors are increasingly recognized as first-line ovulation induction (OI) medications, offering an efficient and physiologic approach to ovarian stimulation that enhances outcomes in reproductive medicine. By selectively inhibiting aromatase and maintaining lower peripheral estrogen levels, letrozole supports mono- or bi-follicular development while reducing the risk of supraphysiologic estradiol exposure seen with traditional gonadotropin regimens. These pharmacological characteristics have contributed to its expanding use not only in OI but also in various a

Emergency department care for early pregnancy loss: A scoping review of patient experiences and compassionate clinical practices in the United States.

BACKGROUND: Early pregnancy loss (EPL), including miscarriage and ectopic pregnancy, affects a significant proportion of pregnancies and often leads patients to seek urgent care in the emergency department (ED). Despite this, the quality of care received varies and individuals often have negative experiences. This scoping review provides a comprehensive overview of published research examining ED care for EPL, specifically focusing on patient experiences and compassionate clinical practices. METHODS: The PRISMA-ScR checklist was utilized for the review. A literature search was performed to id

Bilateral haemorrhagic cysts on ultrasound with an intraoperative diagnosis of spontaneous bilateral unruptured ectopic pregnancies: A case report with management challenges in a rural setting.

Spontaneous bilateral ectopic pregnancy is extremely rare, with an estimated incidence of 1 in 200,000 spontaneous pregnancies. Diagnosis is challenging, particularly in rural and remote settings where clinicians may have limited experience with rare presentations and reduced access to advanced imaging and specialist support. This report presents the case of a 36-year-old woman (gravida4, para 3) who presented to a rural hospital with vaginal bleeding and pelvic pain at 4 + 5 weeks of gestation. Serial quantitative beta-human chorionic gonadotropin levels increased from 962 IU/L to 6545 IU/L o

Kynurenine pathway activation in ectopic pregnancy: diagnostic value of 3-hydroxykynurenine and 3-hydroxyanthranilic acid; a prospective study.

BACKGROUND AND AIMS: Ectopic pregnancy (EP) is a leading cause of maternal morbidity and mortality in the first trimester, accounting for nearly 9% of pregnancy-related deaths. An accurate biochemical marker for early detection is still unavailable. The kynurenine pathway, the primary route of tryptophan metabolism, is involved in immune tolerance, oxidative stress, and placental development. This study aimed to evaluate kynurenine metabolites as potential biomarkers for EP. METHODS: In this prospective single-center study, 106 pregnant women were recruited between January and June 2025, incl

Turning pain into purpose: a qualitative investigation into the psycho-social impacts associated with ectopic pregnancy loss among women in the United Kingdom.

Despite the profound effects ectopic pregnancy has been demonstrated to have on women's mental health, this type of pregnancy loss has received little attention compared to other forms. Ectopic pregnancies are the leading cause of maternal death in early pregnancy and often require urgent medical intervention, which overshadows the psycho-social impacts associated with pregnancy loss. We aimed to undertake a detailed investigation into women's experiences and associated psychosocial impacts of ectopic pregnancy. Using a qualitative research design, semi-structured interviews (N = 20) were cond
