Heterotopic pregnancy is on the rise with the use of assisted conception and commonly involves the fallopian tube. Ovarian heterotopic pregnancy is rare, with fewer than 40 reported cases in PubMed/Medline; cases of ovarian ectopic pregnancy after intrauterine insemination are even rarer, with only seven published reports. We report here a case of ovarian heterotopic pregnancy following intrauterine insemination; this could possibly be the first such report. BACKGROUND: Ovarian pregnancy (OP) is a rare ectopic pregnancy, in which it is very difficult to achieve preoperative diagnosis. Ovarian blood supply in OP increases which will lead to intra-abdominal bleeding, hazarding women's lives. Surgical exploration should be conducted once cases of OP are suspected.

OBJECTIVE: To investigate the clinical presentation of women with primary ovarian pregnancy diagnosed in recent years and to compare it to tubal pregnancy.
STUDY DESIGN: Seven women treated for primary ovarian pregnancy from 2002-2013 were retrospectively identified and compared to 42 women with tubal pregnancies (involving either tubal rupture or tubal abortion) operated on during the same period. In the ovarian pregnancy group the pathology examination confirmed primary ovarian pregnancy according to the Spiegelberg criteria.

We here report a case of left ovarian pregnancy passively detected in the General Reference Hospital outpatient clinic in Dilolo, Democratic Republic of Congo. The diagnosis was confirmed by ultrasound; the patient underwent surgery with uneventful postoperative course. Pregnant women and healthcare personnel should be aware of the importance of proper clinical and echographic monitoring in pregnancy for early diagnosis of abnormal implantations.

We present a rare case of ovarian pregnancy that occurred in a woman who underwent in vitro fertilization (IVF) after bilateral salpingectomy. The patient presented with abdominal pain and a positive pregnancy test. Ovarian pregnancy was diagnosed owing to a suspicious mass detected on ultrasound. She underwent laparoscopy to confirm the diagnosis, which was subsequently verified by histopathological examination. The question that comes in mind is: How did the pregnancy get there? After a review of the literature, we have found a few possible explanations for the mechanism of this rare event.

OBJECTIVES: To report a rare case of ovarian tumor with an unusual presentation; an ovarian pregnancy luteoma with massive ascites and elevated CA125 after ovulation induction therapy.
CASE PRESENTATION: A 26-year-old pregnant woman complained lower abdominal distension. Ultrasound imaging showed a solid tumor in the right adexna and massive ascites. The blood test showed elevated serum level of CA125 and androgens. The patient underwent the right salpingo-oophorectomy, and then the results of blood test were normal and ascites disappeared.

Primary ovarian ectopic pregnancy is a rare entity and is associated with rupture very early in the gestation. We present a case of ovarian pregnancy in a primi gravida, which ruptured relatively late in the first trimester. The patient did not have any predisposing factors for ovarian pregnancy. Ovarian pregnancy is one of the rarest types of extrauterine pregnancy. Its preoperative diagnosis remains a challenge since it presents quite similarly to tubal pregnancy and complicated ovarian cysts. BACKGROUND: Ectopic pregnancy is one of the ginecobstetric conditions that contribute to increased maternal mortality by 10 to 15% of cases. Ovarian ectopic pregnancy is one of the rare cases and represents up to 3% of ectopic pregnancies.
CASE: Female with 33 years old with lower abdominal pain, and pallor. Transvaginal ultrasound showed a live product with 15.3 weeks in left ovary and moderate hemoperitoneum. Exploratory laparotomy was performed with left salpingo-oophorectomy; postoperative course was satisfactory, discharged on the fourth day.
