Placental Snowstorm Appearance in the First Trimester: Expert Assessment of the Fetal Anatomy Does Matter.

Snowstorm appearance of the placenta warrants a differential diagnosis among placental mesenchymal dysplasia, partial molar pregnancy and dichorionic twin pregnancy with a complete mole in one sac. Albeit sharing the "unusual" appearance of the placenta, the three conditions differ in terms of management and prognosis as well as serum biochemistry and association with structural and genetic fetal anomalies. In our experience, the expert sonographic assessment of fetal anatomy best performs in differentiating the conditions as early as the first trimester. We present a case in which placental a

Partial hydatidiform mole coexists with a living fetus: a case report.

BACKGROUND: Hydatidiform mole is a benign gestational trophoblastic disease (GTD) that is classified into two types: complete and partial. Partial molar pregnancy is usually presented as a missed or incomplete miscarriage in the first trimester of pregnancy. A partial molar pregnancy coexisting with living molar pregnancy fetus is a rare occurrence. CASE PRESENTATION: A dead fetus and placenta product of normal vaginal delivery arrived at our hospital. This dead fetus belonged to the first pregnancy of a 21-year-old Persian woman, without gross anomaly, male gender, and was compatible with 5-

Ocean acidification at the Toarcian Anoxic Event captured by boron isotopes in the lime mud record.

The present study investigated the effects of grip on changes in the median nerve cross-sectional area (MNCSA) and median nerve diameter in the radial-ulnar direction (D1) and dorsal-palmar direction (D2) at three wrist angles. Twenty-nine healthy participants (19 men [mean age, 24.2 ± 1.6 years]; 10 women [mean age, 24.0 ± 1.6 years]) were recruited. The median nerve was examined at the proximal carpal tunnel region in three grip conditions, namely finger relaxation, unclenched fist, and clenched fist. Ultrasound examinations were performed in the neutral wrist position (0°), at 30°wrist flex

Discrepancies Between Pathological Distinction and DNA Genotyping in the Diagnosis of Hydatidiform Moles.

Introduction The incidence of invasive moles and choriocarcinoma after uterine evacuation differs between a complete hydatidiform mole (CHM) and a partial hydatidiform mole (PHM). Accurately distinguishing between these two types is important. This study aimed to investigate diagnostic differences in hydatidiform moles using p57kip2 immunohistochemistry and DNA genotyping analysis in trophoblastic tissue. Materials and methods Twenty-four patients who underwent uterine evacuation or total hysterectomy for suspected hydatidiform moles between 2013 and 2022 were included in this study. Pathologi

Isthmic tubal ectopic pregnancy from a partial molar pregnancy: A case report and literature review.

While tubal ectopic pregnancies and molar pregnancies are relatively common, tubal molar pregnancies are exceptionally rare, with an estimated incidence of 1.5 per 1,000,000 pregnancies. Molar pregnancies are at risk for developing malignant gestational trophoblastic neoplasia (15-20 % for complete hydatidiform moles and 0.5-6 % for partial hydatidiform moles) and warrant robust diagnostic workup to determine appropriate management. Presented here is a case of an isthmic tubal ectopic pregnancy secondary to a partial hydatidiform mole; additionally, a literature review through PubMed discusses

Macroscopic Monozygotic Androgenetic/Biparental Mosaicism: Molecular Characterization and Clinical Implications.

Hydatidiform moles represent abnormal pregnancies characterized by trophoblastic hyperproliferation. However, accurate diagnosis of partial hydatidiform moles (PHM) remains challenging. We present a rare case of a monozygotic androgenetic/biparental mosaic in a 26-year-old primigravida. The patient was referred to our institution for a suspected PHM, and ultrasonography revealed a nonviable embryo-like structure alongside villous formations with focal cystic changes. Pathological examination of the evacuated tissue revealed the coexistence of normal and hydropic villi. Histological assessment

A Partial Hydatidiform Mole in an Ovarian Ectopic Pregnancy: An Exceptional Occurrence.

Background and Clinical Significance: Ovarian ectopic pregnancy (OEP) is a rare occurrence, and molar degeneration is even more exceptional. Differential diagnosis between a partial and complete hydatidiform mole is paramount as the complete type carries a higher risk of post-molar gestational trophoblastic neoplasia. Herein, we describe a case of a partial mole in an OEP (OPHM) with thorough investigations. Case Presentation: A 39-year-old woman presented at 6 weeks of amenorrhea with abdominal pain and vaginal bleeding. Ultrasound showed no intrauterine pregnancy, but an ovarian cyst suspici

Gestational Trophoblastic Disease in Bahrain: Prevalence, Risk Factors, and Outcomes.

Background Gestational trophoblastic disease (GTD) refers to a group of conditions linked to abnormal growth of trophoblastic tissue following conception. Although uncommon, early detection of GTD is vital due to the potential for progression and serious complications. The prevalence and presentation of GTD vary by region. In Bahrain, there are limited data on GTD, its risk factors, and clinical outcomes within the Bahraini population. Objectives To investigate the frequency, associated risk factors, and clinical outcomes of GTD among women in the Kingdom of Bahrain and to compare the findings

Cesarean scar pregnancy with molar pregnancy: A case report and literature review.

RATIONALE: Cesarean scar pregnancy with molar pregnancy is a rare but high-risk pregnancy complication characterized by the implantation of a fertilized egg in the uterine scar following cesarean section, accompanied by pathological manifestations of a hydatidiform mole. This paper reports a clinical case of hydatidiform mole in a cesarean scar and reviews the literature to understand its diagnosis and treatment strategies. PATIENT CONCERNS: We reported a 33-year-old woman who presented to our hospital with intermittent vaginal bleeding for over 2 months following uterine curettage. DIAGNOSE

Multiple pregnancy with complete hydatidiform mole and coexisting normal fetus: systematic review and meta-analysis of clinical outcomes from non-randomized studies.

OBJECTIVE: Complete hydatidiform mole and coexisting normal fetus (CHMCF) is a rare condition for which there is significant heterogeneity in diagnosis, counseling and management of complications. The objective of this study was to summarize the prevalence of clinical outcomes in reported cases of CHMCF. METHODS: A systematic literature search was conducted in PubMed, Embase and Scopus databases from inception until 1 October 2024. Case series and cohort studies including at least three cases of histologically confirmed CHMCF were included. A random-effects model was used for meta-analysis of

Recurrent Beta-Human Chorionic Gonadotropin (β-hCG) Elevation and Suspicion of Gestational Trophoblastic Neoplasia Following a Complete Hydatidiform Mole in a 17-Year-Old Female Patient: A Case Report.

The patients with complete hydatidiform mole (CHM) often present with markedly elevated beta-human chorionic gonadotropin (β-hCG) levels, uterine enlargement, and trophoblastic hyperplasia, and may develop complications such as preeclampsia, hyperthyroidism, or theca lutein cysts. The risk of developing persistent gestational trophoblastic neoplasia (GTN) following CHM is substantially higher than with partial hydatidiform mole (PHM). We describe the case of a 17-year-old female patient referred for evaluation of abdominal distension, abnormal vaginal bleeding, and hypertension. Imaging and la

Gestational trophoblastic diseases and neonatal choriocarcinoma.

Trophoblastic diseases include benign pre-tumor entities (hydatidiform moles) and malignancies called gestational trophoblastic tumors. Most of the latter arise after a hydatidiform mole and are referred to as post-molar trophoblastic tumors. Their diagnosis relies on elevated human chorionic gonadotrophin (hCG) levels following a mole, without the need for histological confirmation. Other forms, including choriocarcinoma, placental site trophoblastic tumor (PSTT), and epithelioid trophoblastic tumor (ETT), require histology; notably, PSTT and ETT are relatively resistant to chemotherapy and u

Predicting severe COVID-19 using readily available admission indicators: SpO2/FiO2 ratio, comorbidity index, and gender.

The focus of this study was to identify risk factors for severe and critical COVID-19, evaluate local respiratory immune responses to SARS-CoV-2 infection, and develop a prognostic tool for COVID-19 severity using accessible early indicators. Using nasopharyngeal swab samples from hospitalized patients with COVID-19 of varying severity during the first wave of the pandemic from March to May 2020 in Louisiana, we evaluated the association between COVID-19 severity and viral load, respiratory immune mediators, and demographic/clinical factors. We found that the SpO2/FiO2 ratio at triage, total c

Recent advances in rectal cancer treatment - are we on the right track?

BACKGROUND: Staging and treatment of rectal cancer have evolved over several decades with considerably fewer locoregional recurrences but no marked improved survival since systemic recurrence risks remain virtually unchanged. This development will briefly be summarised followed by a thorough discussion of two recent developments. METHODS: A systematic approach towards the literature is aimed at focusing on organ preservation and the delivery of all non-surgical treatments prior to surgery or total neoadjuvant treatment (TNT). RESULTS: Organ preservation, that is to defer surgery if the tumou

Expert Pathology for Gestational Trophoblastic Disease: Towards an International Multidisciplinary Team Meeting.

BACKGROUND: Gestational trophoblastic disease (GTD), comprising hydatidiform moles and gestational trophoblastic tumours, is extremely rare. Exact diagnosis is crucial to indicate the appropriate treatment and to prevent complications. The scarcity and variability in the number of cases available for reporting, lack of specialised training in GTD, and non-existence of refresher courses implies that the pathologist dealing with these rare and, at times, extremely challenging cases is not completely confident in their diagnosis. OBJECTIVES: The objective of this study was to explore the benefit

Short tandem repeats genotyping of gestational choriocarcinoma - our experiences.

OBJECTIVE: This short communication demonstrates how short tandem repeat genotyping can identify the origin of gestational choriocarcinoma. MATERIALS AND METHODS: The origin of gestational choriocarcinoma in our three cases was determined using the short tandem repeats genotyping technique, which involved quantitative fluorescent PCR and fragmentation analysis. RESULTS: In Case 1 despite no medical history of molar pregnancy, DNA analysis indicated that the choriocarcinoma originated from a homozygous complete hydatidiform mole. We conclude, that the patient's complete abortion 10 years prio
