Group 4 pulmonary hypertension due to choriocarcinoma after molar pregnancy: case report and review.

A 46-year-old Mexican woman developed type 4 pulmonary hypertension (PH) due to tumor embolism from choriocarcinoma (CC), which is an uncommon presentation of this malignancy. She presented with precordial pain, dyspnea, and rapid functional decline. Imaging revealed obstructive thrombi in the pulmonary arteries, initially suggestive of operable chronic thromboembolic PH (CTEPH). A bilateral subsegmental pulmonary thromboendarterectomy was performed, the standard treatment for CTEPH, aimed at removing obstructive material and restoring hemodynamic stability. However, histopathological examinat

STR genotyping diagnosis of hydatidiform moles: an assessment of 2871 consecutive products of conceptions.

This study evaluated the incidence of hydatidiform moles in a tertiary care setting and examined the efficacy of short tandem repeat genotyping (STR) as a primary ancillary diagnostic tool for molar gestations. Consecutive products of conceptions undergoing genotyping were analyzed. A total of 2871 cases were analyzed, comprising 1693 in-house and 1178 consultation specimens. Patient ages ranged from 10 to 57 years (mean 32, median 33). Laser microdissection was performed in 138 specimens. STR genotyping yielded a definitive diagnosis in 99.5% of cases (2857/2871), classifying them as 282 comp

Placental mesenchymal dysplasia: A rare case report and literature review.

RATIONALE: Placental mesenchymal dysplasia (PMD) is a rare placental disorder that poses diagnostic challenges and is often misdiagnosed as a trophoblastic tumor. While PMD is associated with fetal growth restriction, intrauterine fetal death, and pregnancy-induced hypertension, it does not involve malignant trophoblastic disease. Accurate diagnosis is crucial for optimizing maternal and fetal outcomes. PATIENT CONCERNS: A 30-year-old female, gravida 3, para 1, with an uncomplicated medical history, presented with early pregnancy bleeding, which was managed with oral progesterone. Ultrasound

Novel mutations of maternal effect gene thyroid hormone receptor interactor 13 involved in biparental complete hydatidiform mole.

RATIONALE: Biparental complete hydatidiform mole (BiCHM) is a rare form of molar pregnancy, frequently associated with familial recurrence, whose mechanism was historically unclear. It is classically diploid with biparental inheritance, but manifests as a complete hydatidiform mole (CHM). Cytogenetic studies suggested a link to maternal mutations in imprinted genes NOD-like receptor family, pyrin domain containing 7 or Kelch helper domain containing 3-like within the oocyte. This case reports a novel gene association. PATIENT CONCERNS: A 43-year-old woman presented with a history of recurrent

Twin Pregnancy With Discordant Pathology: A Case of Molar Gestation and Viable Co-twin After Assisted Conception.

Complete hydatidiform mole with a coexisting viable twin is an exceptionally rare condition. It poses significant maternal risks, including hemorrhage, early-onset preeclampsia, and progression to gestational trophoblastic neoplasia. The use of assisted reproductive technology (ART) has increased the detection of such complex gestations, yet optimal management remains controversial. We present the case of a 23-year-old primigravida who conceived a dichorionic diamniotic twin pregnancy following frozen embryo transfer. Early ultrasound demonstrated one normally developing gestational sac with a

Invasive Mole of the Uterus: A Case Report.

This case presents a 31-year-old patient who was admitted due to secondary amenorrhea and high beta-human chorionic gonadotropin (hCG) levels. During hospitalization, a hydatidiform mole was identified. Initially, fractional curettage, hysteroscopy, and laparoscopy were performed. Due to the ineffectiveness of these methods, it was decided to perform a supracervical hysterectomy. Histopathological examination confirmed the presence of an invasive hydatidiform mole. hCG levels were monitored until they dropped to <1.2 mIU/ml. This case shows that surgical treatment can be an effective method fo

Whole-genome paternal uniparental disomy identified through prenatal single-nucleotide polymorphism-based cell-free DNA screening.

OBJECTIVE: Prenatal single-nucleotide polymorphism (SNP)-based cell-free DNA (cfDNA) screening can identify genome-wide paternal uniparental disomy (GW-UPDpat), including cases with complete hydatidiform mole with a coexisting fetus (CHMCF), those with placental mesenchymal dysplasia (PMD) and those with a mosaic/chimeric GW-UPDpat syndrome. Our objective was to review laboratory data and pregnancy outcome for SNP-based cfDNA screening tests with results compatible with GW-UPDpat and a normal cell line. METHODS: This was a retrospective study of all cfDNA screening results from a single comme

A rare concomitant association: partial hydatidiform mole with preeclampsia without severe features, hyperthyroidism, torsion of a theca-lutein cyst and choriocarcinoma with pulmonary metastases.

Gestational trophoblastic disease (GTD) encompasses a spectrum of premalignant and malignant conditions arising from trophoblastic tissue. This case highlights an unusual and severe progression of a partial hydatidiform mole complicated by preeclampsia, hyperthyroidism, torsion of a theca-lutein cyst, and ultimately metastatic choriocarcinoma. The patient's course illustrates the importance of early recognition, close monitoring, and multidisciplinary management. This rare and instructive presentation underscores the malignant potential of partial moles. Case: A 36-year-old patient who present

Evaluation of false positive and false negative errors in targeted next generation sequencing.

BACKGROUND: Next-generation sequencing (NGS) has become an indispensable diagnostic tool across various diseases. However, sequencing and analysis errors remain major barriers to clinical implementation. In cancer diagnostics, detecting low-level somatic variants is particularly challenging due to tumor heterogeneity and contamination from normal cells. RESULTS: We assess targeted next-generation sequencing (T-NGS) performance using reference-standard DNA mixtures of homozygote hydatidiform mole and heterozygote blood DNA at varying ratios, analyzed by certified NGS providers. Analytical sens

Turner's Syndrome in Discordant Dizygotic Twins: Biological Origins and Twin Relations/Twin Research Reviews: Prevention of Premature Twin Birth; Twin Gestation with Hydatidiform Mole; Update on Feingold Syndrome Twins; Qualitative MZ Twin Difference Studies/Media: Identical Twins Turn 100 Years of Age; Twins in Famous Families; Celebration of Yorùbá Twins of Nigeria; Identical Artistic Partners; Rare Conjoined Twins Separated.

A pair of dizygotic (DZ) twins discordant for Turner syndrome are discussed with reference to the biological origins of the condition and the effects of discordance on the twin relationship. There is little research on how having an atypical twin influences the life events and goals of the typical twin. Next, timely reviews of research on preventing premature twin birth, a twin gestation with hydatidiform mole, an update on Feingold syndrome twins discussed in a previous issue of this journal, and qualitative monozygotic twin difference studies are presented. The final portion of this article
