PURPOSE: Gestational trophoblastic neoplasia (GTN) is a highly curable malignancy classified using the International Federation of Gynecology and Obstetrics (FIGO)/WHO scoring system into low-risk (score 0-6) and high-risk (score ≥ 7) categories. Although single-agent chemotherapy is standard for low-risk GTN, patients with FIGO scores of 5-6 exhibit disproportionately high rates of chemoresistance, raising concerns about current classification and treatment strategies.
METHODS: This retrospective analytic study included 162 patients with low-risk GTN treated at a tertiary referral center in

Gestational trophoblastic disease (GTD) typically occurs in women of reproductive age and is exceedingly rare in postmenopausal women. Hydatidiform mole is the most common form of GTD. The recommended management is total hysterectomy, as women over the age of 50 years have a higher risk of malignant transformation. Here, we report the case of massive molar pregnancy in a 52-year-old postmenopausal woman, complicated by hyperthyroidism and other comorbidities, with cultural and belief-related barriers influencing treatment decisions.

Complete hydatidiform mole with coexisting fetus (CHMCF) is an exceptionally rare and clinically challenging obstetric condition. Its diagnosis and management require a high index of suspicion and a multidisciplinary approach. This report presents the case of a woman in her 30s who presented for a routine prenatal examination at 15 weeks of gestation. Ultrasonography revealed a placenta with a mixed echogenicity area exhibiting a characteristic honeycomb-like appearance, alongside a coexisting viable fetus with normal biometry. Serum β-human chorionic gonadotropin (β-hCG) was markedly elevated

A hydatidiform mole (HM) is the most common form of gestational trophoblastic disease (GTD). Differentiating hydatidiform moles (HMs) from non-molar pregnancies and distinguishing complete HMs (CHMs) from partial HMs (PHMs) remains challenging due to overlapping morphological features and a high rate of misclassification. This study aimed to evaluate reliable immunohistochemical markers for improving diagnostic accuracy and addressing the limitations of current molecular techniques. We retrospectively analyzed 64 cases of HMs and hydropic abortions (HAs), diagnosed in women aged 17-36 years be

Gestational trophoblastic diseases (GTD) comprise a heterogeneous group of disorders arising from genetic anomalies occurring during fertilization in twin pregnancies and often may be associated with assisted reproductive techniques. An exceedingly rare presentation of GTD is a twin pregnancy hydatidiform mole with a co-existing fetus, condition which may be an important cause of complications for the mother and the fetus. A 36-year-old woman (G2, P0, A1) underwent a friendly controlled ovarian stimulation (COS) followed by intrauterine insemination (IUI) for assisted reproductive purposes, re

Twin pregnancy with a complete hydatidiform mole and a co-existing fetus (CHMCF) is a rare situation and may occur after using assisted reproductive technologies (ARTs). These pregnancies are associated with severe maternal and fetal complications, and their management is challenging. Ultrasound (US) and magnetic resonance imaging (MRI) are commonly used tools for the diagnosis of suspected molar pregnancy. However, they are not 100% sensitive or specific, and pregnancy could end with a healthy baby. Histopathological examination is considered the gold standard for confirming the diagnosis of

The distinction between choriocarcinoma and residual trophoblastic cell proliferation from a complete hydatidiform mole/invasive mole (CHM/IM) without villi is challenging on curettage materials. We investigated whether SALL4 immunostaining could help differentiate various gestational trophoblastic diseases. Placental site nodules (PSN; n=10), atypical PSN (APSN; n=8), placental site trophoblastic tumors (PSTT; n=9), epithelioid trophoblastic tumors (ETT; n=5), gestational choriocarcinomas (n=31), partial hydatidiform moles (PHM; n=13), CHM/IM (n=47), and nonmolar products of conception (POC)

Molar pregnancy is an uncommon diagnosis in perimenopausal women and poses a high risk of complications, including hemorrhagic shock and malignant transformation. We report the case of a 51-year-old woman who presented with severe abdominal pain and profuse vaginal bleeding. Imaging revealed a markedly enlarged uterus with a large intracavitary mass, and serum β-hCG levels were elevated. The patient developed hemorrhagic shock and underwent an emergency hemostatic hysterectomy. Histopathological examination confirmed a complete hydatidiform mole with early invasive features, without choriocarc

BACKGROUND Gestational trophoblastic diseases (GTDs) are a group of benign and malignant tumors that arise from placental tissue. Ectopic pregnancies most commonly occur within the fallopian tubes. The estimated incidence of ectopic gestational trophoblastic diseases (GTDs) is approximated at 1.5 per 1 000 000 pregnancies, which makes it a very rare condition. The diagnosis is challenging since the clinical presentation often mimics conventional ectopic pregnancy, but management approaches are significantly different. CASE REPORT A 41-year-old woman (G7 T4 P0 A2 L4) presented to the Emergency

Tubal molar pregnancy is extremely rare, and less than 200 cases have been reported in the literature. The incidence is approximated at 1.5 per 1 000 000 pregnancies. We report a case of ruptured tubal pregnancy in a woman, whose postoperative histopathology diagnosis showed partial molar pregnancy in the ruptured fallopian tube. The presence of abnormal, non-polar trophoblast proliferation, which is circumferential with vacuolation, along with sheets of pleomorphic extravillous trophoblast, is the main diagnosing feature, and it carries the risk of malignant potential.

AIMS: Specific identification of a hydatidiform mole (HM) and subclassification of a complete hydatidiform mole (CHM) or partial hydatidiform mole (PHM) are critical. This study aimed to reappraise the diagnostic performance of ultrasonography and histology with a refined diagnosis.
METHODS: This was a retrospective, multicentre cohort study of 821 patients with histologically suspected HM specimens. Refined diagnostic algorithms with p57 immunohistochemistry and short tandem repeat (STR) genotyping were performed and used as the true standard for assessing the diagnostic performance of the o

BACKGROUND: Amongst complete hydatidiform moles, 15% to 20% will progress to postmolar neoplasia. Scientists have long searched for routinely applicable prognostic markers that can efficiently predict the risk of postmolar neoplasia.
OBJECTIVE: To assess the prognostic value of atypical extravillous trophoblast foci within complete hydatidiform moles on the development of postmolar gestational trophoblastic neoplasia.
STUDY DESIGN: Between January 2017 and December 2022, a retrospective multicenter study was conducted in the Belgian Gestational Trophoblastic Diseases Registry (French-speakin

Co-presentation of complete mole with live pregnancy is very rare, few cases with live birth were reported. The diagnosis requires a high index of suspicion, which is important to set the management plan antenatal, intrapartum, postnatal and neonatal. Our patient conceived during follow-up for previous partial molar pregnancy. Early pregnancy scan showed a gestational sac with cystic spaces. Ultrasound scans by fetal medicine specialists revealed a normally developing fetus with partly multicystic placenta. She was delivered by caesarean section at 31 weeks due to abdominal pain and increasing

BACKGROUND: Placental mesenchymal dysplasia (PMD) is a placental abnormality resembling partial hydatidiform moles without trophoblastic proliferation. Although many PMD cases involve androgenetic/biparental mosaicism or chimerism, we recently reported that approximately 30% of cases retain biparental genomes (BiPMD) and exhibit aberrant methylation at multiple imprinted differentially methylated regions (DMRs). This resembles multilocus imprinting disturbances (MLIDs) and biparental hydatidiform moles (BiHMs), which are associated with pathogenic variants in subcortical maternal complex (SCMC

BACKGROUND: Complete hydatidiform mole with a coexisting fetus (CMCF) is a rare form of twin pregnancy associated with high maternal and perinatal risks, posing complex diagnostic and therapeutic challenges. The standard approach ranges from termination of pregnancy to conservative management to fetal viability. Recent evidence suggests that spontaneous regression of molar tissue may occur, supporting conservative treatment in selected cases. We present a new case and discuss the clinical consequences of this phenomenon.
CASE: We report a 29-year-old woman with a history of a previous molar p

BACKGROUND: The hydatidiform mole presents as either a complete or partial mole. They are differentiated by morphology, histopathology, karyotype, and the risk of malignancy. Partial hydatidiform moles are the only type of trophoblastic gestational disease associated with the presence of a fetus (with or without positive cardiac response). However, early intrauterine fetal death often occurs with triploidy. Therefore, a partial hydatidiform mole is often misdiagnosed as an incomplete abortion. This case is unique due to the rare occurrence of a partial hydatidiform mole complicated by severe p

INTRODUCTION: Gestational Trophoblastic Disease (GTD) encompasses a spectrum of placenta-related disorders with significant health risks. Despite the varying reported prevalence rates in East Africa, no comprehensive meta-analysis has been conducted to synthesize the overall burden and predictors of GTD in this region.
METHODS: Following the PRISMA guidelines, we conducted a systematic review and meta-analysis of studies from PubMed, Cochrane Library, and Google Scholar (search period: 2000-2024). We assessed heterogeneity (I2 and Q tests), pooled estimates using a random-effects model, and e

BACKGROUND: Gastrointestinal bleeding due to metastasis of an invasive mole to the small intestine is very rare. Most reported cases of metastatic invasive mole are diagnosed after surgery, and lack rich illustrations, which leads to insufficient understanding by clinicians, misdiagnosis, and unnecessary surgeries.
CASE SUMMARY: A 22-year-old female patient presented with bloody stool and elevated human chorionic gonadotropin. The transvaginal gynecological ultrasound ruled out pregnancy. Upper gastrointestinal endoscopy and colonoscopy were performed, but no bleeding focus was detected. The

Gestational choriocarcinoma (GCC) is a malignant and aggressive tumor composed of neoplastic trophoblasts rarely arising months after a normal gestation or after an hydatidiform mole (HM). Histologically, its main diagnostic features are a trimorphic population of trophoblast cells and an absence of chorionic villi. Recently, extremely rare cases of GCC diagnosed in molar and in placenta specimens have been described and accepted as early forms of GCC. We report two cases of GCC diagnosed in a term placenta and in a complete HM (CHM) and underline the importance of recognizing such a rare earl
