A synthetic compound, 2a, 2b dihomo 15 (S) 15 methyl prostaglandin F 2 alpha methyl ester given 8-hourly by intramuscular injection was used to terminate pregnancies in 72 patients with a missed abortion, intrauterine fetal death, molar or anencephalic pregnancies. Treatment was successful in all cases. A 4 mg dose of loperamide given orally at 15 to 30 minutes before the injection of prostaglandin analogue prevented diarrhoea as a side effect.

Twenty patients with either missed abortions or intrauterine fetal death were induced with prostaglandin (PGE2) vaginal suppositories. The uterus is surprisingly responsive to PGE2 in cases of intrauterine death, yielding a short treatment-delivery interval. Blood coagulopathies which can develop after intrauterine death can be avoided through early diagnosis and treatment. Eighteen patients completely delivered and there were no serious complications. Fetus papyraceus is a rare condition occurring in both monochorionic and dichorionic multiple gestations. Assocated with case presented were two interesting and uncommon findings: velamentous insertion of the umbilical cord of the blighted fetus and a congenital abonormality of the surviving fetus. This case is the first known intrauterine monitoring of a pregnancy involving a fetus papyraceus with serial plasma estriol and human placenta lactogen determination.

Prostaglandin is being used increasingly as an abortifacient, especially in infans mortuus, fetus mortuus, interruption of pregnancy, missed abortion and hydatidiform mole. The most commonly used form of application is extraamniotic instillation. Interruption of pregnancy during acute leukosis of pregnancy is an extension of the list of indications. One such case is reported. The reliable and rapid onset of action and the small loss of blood suggest that prostaglandins may be the drug of choice.

Ten placentae with massive subchorionic thrombohaematomas are reported. This lesion, which is composed of maternal clotted blood, separates the chorionic plate of the placenta from the villous chorion and is traversed by the main cotyledonary branches of the fetal circulation. Eighteen patients hospitalized for excessive ovarian hyperstimulation syndrome are reported. In 14 cases the ovarian hyperstimulation was induced by human menopausal -onadotropins and in 4 cases by combined treatment with clomiphene and HCG. Pregnancy associated alpha2-globulin serum levels have been measured in abortion-cases. In cases of incomplete abortion with placental destruction and missed abortion this alpha2-globulin level significantly decreased, but no changes could be found when the placenta remained intact. The prognostic value of this pregnancy protein is discussed.

In a series of 425 consecutive patients examined by sonar in the first half of pregnancy 176 ultimately aborted. On analysis of the sonar and post-abortion findings it was found that the aborted pregnancies fell into five clearly defined groups; blighted ova or anembryonic pregnancies, missed abortions, hydatidiform moles and early and late live abortions. This study confirms essentially the results of other investigators who found a good correlation between fetal risk and maternal AFP level. This adds an immuno-chemical technique to well-known methods of monitoring risk-pregnancies (ultra-sound, HPL, estrogenes) which enables us to measure a specific fetal metabolic product, alpha-feto-protein, in amniotic fluid, umbilical blood and maternal serum.
