Exposure during pregnancy to Il-1 targeted therapies and pregnancy, foetal and neonatal outcomes: a study from the French Teratology Information Service.

OBJECTIVE: To describe the outcomes of pregnancies exposed to IL-1 targeted therapies (anti-IL-1): anakinra and/or canakinumab. METHODS: We performed a descriptive observational study based on data from CRAT, the French Teratology Information Service. We included prospective and retrospective pregnancies exposed to anakinra and/or canakinumab with known outcomes. Pregnancy, foetal and neonatal outcomes were analysed. RESULTS: Between 2007 and 2023, 57 pregnant women exposed to anti-IL-1 were referred, including 27 (47.3%) treated for Still's disease. Among prospective cases, 52 foetuses (one

Comparing Medical Abortion Outcomes and Experiences Between Young and Adult Women: Evidence From Urban Cambodia.

INTRODUCTION: Self-managed abortions are increasingly common globally, but little is known about young women's experiences. METHODS: Secondary analysis of data from 1847 women accessing MA in urban Cambodia in 2018-2019 and in-depth interviews with seven women aged 15-19. Descriptive statistics compared young and adult women on self-reported abortion outcomes and experiences in clinics and pharmacies, and qualitative data contextualized quantitative findings. RESULTS: Young women were better represented among clinic clients (30.0%) compared to pharmacy clients (20.2%). Young women had compar

Preconception hypoglycemia and adverse pregnancy outcomes in Chinese women aged 20-49 years: A retrospective cohort study in China.

BACKGROUND: In addition to hyperglycemia, women with hypoglycemia identified during pregnancy have a higher risk of adverse pregnancy outcomes. However, there is limited evidence of the association between prepregnant hypoglycemia and adverse pregnancy outcomes in women without pre-existing diabetes. This study aims to explore the association between maternal preconception hypoglycemia and adverse pregnancy outcomes among childbearing-aged women in China. METHODS AND FINDINGS: This was a retrospective cohort study of the National Free Preconception Checkup Project (NFPCP), including women who

From Adhesions to Conception: A Case Study on Platelet-Rich Plasma's Role in Gynecologic Recovery.

Intrauterine adhesions (IUA) are a common gynecologic disorder usually caused by infection, surgery, or trauma. Its pathogenesis is complex and involves multiple factors such as cytokines, inflammatory responses, and tissue fibrosis. In recent years, diagnostic and therapeutic options for uterine adhesions have evolved, encompassing both traditional surgical methods and emerging biologic therapies. Platelet-rich plasma (PRP) therapy has gradually gained widespread attention in the field of obstetrics and gynecology. Its core concept is to utilize the abundant platelets in the patient's own blo

Mate Whenua-follow-up after early medical abortion: study protocol for a randomized controlled trial.

BACKGROUND: Early medical abortion (EMA) is safe and effective; an uncommon but crucial adverse outcome is ongoing live pregnancy. The best method of follow-up after EMA to detect ongoing pregnancy is a critical research gap. Few trials compare blood or urine pregnancy tests to ultrasound scans, and no trial compares these tests to each other. The aim is to evaluate the completeness of follow-up of two methods of follow-up after EMA- self-assessment with low-sensitivity urine pregnancy test result or serial serum βhCG blood tests. Secondary aims will evaluate whether self-assessment follow-up

Sepsis following hysteroscopic removal of retained products of conception: A case report and literature review.

RATIONALE: Sepsis following hysteroscopy is an rare complication, with current evidence suggesting that routine prophylactic antibiotic administration may not be warranted. However, this does not imply that we should disregard vigilance regarding the potential occurrence of severe infections post-hysteroscopy. PATIENT CONCERNS: A 27-year-old female underwent hysteroscopic resection of retained products of conception after incomplete medical abortion. After 10 hours postoperatively, the patient developed hyperthermia (39.9°C), tachycardia (137 beats/min), and hypotension (77/36 mm Hg). The blo

Efficacy of self-administration of medication abortion for first trimester of pregnancy in Ethiopia: A multicenter prospective cohort study.

Medical abortion with mifepristone and misoprostol, or misoprostol alone, can be safely administered in clinics or self-managed at home up to nine weeks of gestation. We conducted a prospective multi-center cohort study among women and girls seeking pregnancy termination at six purposively selected healthcare facilities or medication abortion dispensers in Hawassa City, Southern Ethiopia, from March 16, 2024, to January 15, 2025. We grouped the participants in two cohorts: self-administration of medication abortion (exposure group) and health-personnel provided medication abortion care (contro

Determinants of induction-to-expulsion time and adverse maternal outcomes of second trimester medical abortion in Amhara Region, Ethiopia: Prospective follow-up study.

Second-trimester medical abortion (STMA) accounts for 20-40% of abortions in Ethiopia and is associated with risks including uterine rupture, hemorrhage, infection, incomplete abortion, and prolonged hospitalization. This multicenter prospective study included 617 women undergoing STMA in three referral hospitals in the Amhara region from January to October 2024. Induction to-expulsion time and associated factors were analyzed using Cox proportional hazard modeling. Median fetal and placental expulsion times were 10 and 10.5 hours, respectively, with an 81.2% complete expulsion rate. Prolonged

Implementing telemedicine for medical abortion within the public health system: a qualitative study on implementation bottlenecks and solutions in South Africa.

BACKGROUND: Abortion in South Africa is legal, but there are still many barriers to access and high utilisation of the informal sector. Telemedicine for medical abortion is an alternative model that has been found to be a safe, effective, and acceptable option to increase access to abortion services. This study aimed to understand how key informants view telemedicine for medical abortion and how they view potential bottlenecks and solutions concerning implementation in the public sector of South Africa. METHODS: We conducted interviews between February and March 2023 with 19 key informants ac

Outcomes of medical abortion with an intrauterine device in situ: a retrospective review.

BACKGROUND: There is a lack of evidence on the outcomes of medical abortion with an intrauterine device (IUD) in situ. Guidelines recommend IUD removal prior to medical abortion, but retrieval is not always possible. This study examines the outcomes of medical abortion (< 12 weeks) in women where the IUD was removed prior to abortion and those where it remained in situ. METHODS: A retrospective review of medical abortion outcomes < 12 weeks (complete abortion, incomplete abortion and ongoing pregnancy) at a single abortion service in Scotland over five years (2020-2024). Outcomes were compare

Demographic Factors Influencing Eligibility for EMA in South Australia.

BACKGROUND: Early medical abortion (EMA) can be performed by administration of mifepristone and misoprostol in an outpatient setting prior to 63 days gestation in Australia. While this is a flexible, efficacious and safe option for abortion, it requires early identification of pregnancy and efficient access to a clinical service. Outpatient EMA with mifepristone and misoprostol was introduced in Australia in 2012 for gestations < 49 days, and extended to 63 days in 2015. METHODS: The laws governing abortion in South Australia mandate routine data collection. A retrospective cohort study was c

The Sociodemographic Benefits of Extending MS-2Step to 70 Days in Australia.

In many areas of the world, outpatient early medical abortion (EMA) is provided through mifepristone and misoprostol up to 10 weeks or 70 days gestation; however in Australia access is restricted to 63 days. A retrospective cohort study using South Australian data from 2012 to 2020 compares women undergoing abortion at less than 9 weeks with women undergoing abortion at less than 10 weeks. Currently, socioeconomic disadvantage is associated with a higher rate of presenting for abortion after 9 weeks. Extending EMA access would be of particular benefit for groups with socioeconomic disadvantage

Women's real-time pain and stress during medical abortion: An ecological momentary assessment study.

INTRODUCTION: Knowledge on women's pain and stress experiences during medical abortion is scarce and evidence mainly relies on retrospective reports, which are prone to recall bias. Hence, our objective was to study real-time self-reported pain and stress throughout the medical abortion process, their fluctuation over time, and to examine whether age, parity, gestational age, adverse childhood events (ACE), and contextual factors (e.g., analgesics intake, bleeding) were associated with pain and stress. MATERIAL AND METHODS: This one-group, single center, exploratory cohort study (conducted 20

Multivariate genome-wide association study of suicidal behaviors in >1.7 million individuals of diverse population descents.

BACKGROUND: While previous genome-wide association studies (GWAS) identified multiple risk loci for suicide ideation (SI) and suicide attempt (SA), there is still a limited understanding of the genetic predisposition underlying suicidal behaviors in diverse populations. This study aimed to conduct a large-scale investigation of the suicidality spectrum (SP) to generate new insights into its biology and epidemiology. METHODS: Leveraging ancestrally diverse participants (SI N case/control =179 881/1 013 900; SA N case/control =66 867/1 654 798) from the UK Biobank, All of Us Research Program, M
