Baxdrostat (Baxfendy®), a first-in-class aldosterone synthase inhibitor approved for adults with uncontrolled hypertension.

Despite the availability of multiple antihypertensive therapies, many patients remain unable to achieve adequate blood pressure control. Excess aldosterone is a key contributor to resistant and uncontrolled hypertension. Baxdrostat (Baxfendy®), a first-in-class selective aldosterone synthase (CYP11B2) inhibitor, was approved on May 15, 2026, for use in adults with inadequately controlled hypertension receiving other antihypertensive medications. Unlike mineralocorticoid receptor antagonists, baxdrostat reduces aldosterone biosynthesis while preserving cortisol production, offering a distinct m

The Iron Chaperone PCBP2 Controls Systemic Iron Homeostasis by Regulating Iron Flux in Red Pulp Macrophages.

Systemic iron homeostasis critically depends on macrophages for the recycling of iron from senescent erythrocytes. The iron chaperone poly(rC)-binding protein 2 (PCBP2) is considered to direct intracellular iron trafficking. However, its specific role in macrophage-mediated iron redistribution remains unclear. We show that iron deficiency induces PCBP2 expression in macrophages of mice and humans. Mice lacking PCBP2 in macrophages develop features of iron-restricted erythropoiesis, including reduced bone marrow iron, altered peripheral erythrocytes, and suppressed hepcidin levels. Despite medu

The theoretical basis of reservoir pressure in arteries.

The separation of measured arterial pressure into a reservoir pressure and an excess pressure was introduced over 20 years ago as a heuristic hypothesis. Since then it has gained some traction through epidemiological studies that show that various measures of reservoir and excess pressure are independent risk factors for cardiovascular disease. We demonstrate that a two-time asymptotic analysis of the 1-D conservation equations in each artery coupled with the separation of the arteries into inviscid and viscid arteries, based on their resistance or drag coefficients, results in a formal deriva

Severe Anemia and Neutropenia Associated with Copper Deficiency in Three elderly Adults Residing in Nursing Homes.

This report describes three cases of severe anemia and neutropenia associated with copper deficiency. All patients were octogenarian women who presented with cognitive impairment and resided in nursing homes. The rapid response to low-dose oral copper supplementation indicates that their copper deficiency was caused by a poor intake rather than the malabsorption of copper. This case series demonstrates that severe copper deficiency can occur in the absence of typical underlying conditions, such as gastrointestinal surgery or an excessive zinc intake. Our experience suggests incorporating serum

Targeted Catalytic Anti-Inflammatory and Antioxidant Therapy for Aortic Dissection Using Biomimetic Nanomaterials.

Aortic dissection (AD) is a life-threatening cardiovascular disease with an extremely high mortality rate. Excessive reactive oxygen species (ROS) production and inflammatory responses in diseased vascular regions are central drivers of AD pathogenesis, leading to destruction of the aortic wall structure and fatal hemorrhage. However, conventional clinical therapies for AD cannot effectively eliminate excessive ROS at lesion sites while simultaneously exerting anti-inflammatory effects. To address these limitations, we developed a biomimetic, inflammation-targeting nanoparticle, ZIF-8@Res@PEG-

Immunotherapy-Based Therapies in Pediatric Hodgkin Lymphoma: Potential to Narrow the Gap in Long-Term Survival.

Pediatric advanced-stage Hodgkin lymphoma (HL) is highly curable, yet survivors treated with historical regimens face substantial late morbidity and excess mortality. Immunotherapy-based frontline therapies improve disease control, but late toxicities remain unknown. We developed a simulation model to project lifetime outcomes after contemporary brentuximab vedotin- or nivolumab-containing regimens using trial data (COG AHOD1331 and SWOG S1826), Childhood Cancer Survivor Study data, and national databases. We assessed how frontline and relapse therapy changes could influence long-term survival

Insulin levels in prepubertal patients with isolated premature pubarche: The INPREPP study.

We aimed to determine insulin levels and the homeostasis model assessment of insulin resistance (HOMA-IR) index in a cohort of children with isolated premature pubarche, to analyze the potential influence of adiposity, and to compare these parameters with those of a reference population. This study included 50 prepubertal patients with isolated premature pubarche between the ages of 5 and 8 yr (girls) and up to 9 yr (boys). Anthropometric parameters, abdominal circumference, body composition assessed by bioelectrical impedance analysis, and bone age were evaluated. Blood glucose, lipid profile

Effect of antenatal breast milk expression or postnatal dietary intervention on breastfeeding outcomes and human milk composition in women with excess weight: protocol for the Ready, Set, Nourish study, a randomized controlled trial.

BACKGROUND: Human milk is recommended as the ideal source of nutrients intake for infants. It provides optimal nutrition and critical health benefits for both infants and mothers. However, women with excess weight are less likely to initiate and sustain breastfeeding. This study aims to assess how a prenatal and a postnatal intervention can improve breastfeeding outcomes, human milk composition, and infant growth in women with excess weight. METHODS: This three-arm randomized controlled trial will include 180 pregnant women with pre-pregnancy BMI ≥ 25 kg/m2 recruited from central Arkansas. Th

Aldosterone-producing adrenocortical carcinoma presenting with severe, rapidly progressive hypokalemia: a case report and literature review.

Although previous studies have described steroidogenic enzyme expression in aldosterone-producing adrenocortical carcinoma (APAC), detailed immunohistochemical evaluations of CYP11B2 at the protein level and its clinicopathological relevance remain limited. Here, we report a rare case of APAC with a comprehensive immunohistochemical analysis focusing on CYP11B2 expression. An 82-year-old woman presented with limb weakness and severe hypokalemia (serum potassium: 1.9 mmol/L). Endocrinological evaluation revealed markedly elevated plasma aldosterone levels and suppressed renin activity, indicati

Study design, rationale, and baseline characteristics of ATTAIN-HYPERTENSION, a phase 3 trial of the oral GLP-1 receptor agonist orforglipron for hypertension.

BACKGROUND: Up to three-quarters of primary hypertension is epidemiologically attributed to excess adiposity. Incretin-based therapies are consistently associated with reduced blood pressure (BP) in clinical trials; however, no prior trial has investigated incretin-based therapies for hypertension treatment with BP as its primary efficacy outcome. METHODS: ATTAIN-HYPERTENSION is a phase 3 master protocol directing two identical, randomized, placebo-controlled studies. Participants with BMI≥25kg/m2 and systolic BP (SBP)≥140 mmHg or diastolic BP (DBP)≥90 mmHg, independent of antihypertensive tr

Excess body weight at diagnosis is associated with inferior ALL outcomes in male adolescents in the ALL-BFM 2000 study.

Body weight at diagnosis may influence outcomes in childhood acute lymphoblastic leukemia (ALL), although current evidence remains conflicting. Both, underweight and overweight/obesity at diagnosis can negatively affect outcomes. Here, we present data from the ALL-BFM 2000 trial on excess body weight at diagnosis and inferior outcome, especially among male patients aged ≥10 years with ALL. We analyzed 3920 patients with B-cell precursor (BCP)-ALL and 674 patients with T-cell ALL (T-ALL), aged 1 to 17 years. Outcome was assessed by weight category (underweight, normal weight, and overweight/obe

Locomotion-specific metabolic demands in 30-s anaerobic efforts.

PURPOSE: This study aimed to examine whether energy system contributions differ across locomotion modalities during 30-s all-out exercise. METHODS: Sixteen trained male team-sport athletes completed a Wingate Anaerobic Test (WAnT), a 30-s continuous jumping test (CJ30), and a 30-s all-out running test (RUN30) in randomized order. Aerobic, glycolytic, and phosphagen energy contributions were estimated using the PCr-La-O2 method based on oxygen uptake, blood lactate accumulation, and excess post-exercise oxygen consumption. RESULTS: Total energy expenditure was higher in RUN30 (130.8 ± 18.0 kJ

Blood-brain barrier opening with focused ultrasound reduces neuromelanin in a non-human primate model.

Age-related accumulation of neuromelanin (NM) has been implicated in the intrinsic vulnerability of catecholaminergic neurons in Parkinson's disease. Postmortem studies in Parkinson's disease patients have revealed elevated intracellular and extracellular NM in the substantia nigra pars compacta, whereas experimental enhancement of NM synthesis in animal models disrupts neuronal homeostasis and accelerates neurodegeneration. Low-intensity focused ultrasound combined with microbubbles (LIFU-MB) induces focal opening of the blood-brain barrier. It has been shown to influence protein aggregation

Foeniculum vulgare and metformin combination improve ovarian function in a rat model of polycystic ovary syndrome: targeting AMH imbalances, folliculogenesis disruption, fibrosis, and abnormal vascular density.

Polycystic ovary syndrome (PCOS), a major cause of female infertility, is accompanied by elevated anti-Mullerian hormone (AMH), ovarian hypervascularity, and excessive collagen deposition. While metformin is a well-established treatment, no study has yet explored whether Foeniculum vulgare (Fennel)-a medicinal plant with documented anti-PCOS benefits, administered alone or in combination-can attenuate these hormonal and structural abnormalities; this study provides the first integrated evaluation of both agents on ovarian damage in rats. Thirty-three female Wistar adult rats were divided into

Early to mid-gestational testosterone excess leads to adverse lung programming in sheep offspring.

Prenatal testosterone (T) excess programs sex-specific cardiovascular dysfunction in sheep offspring. Given the close reciprocal relationship between the cardiopulmonary system, we hypothesized that prenatal T excess would also adversely program the offspring's pulmonary system in a sex-specific manner from fetal life into adulthood. Pregnant ewes received intramuscular injections of T propionate (100 mg) or a vehicle control (C) twice weekly from gestational days (GD) 30-90 (term 147 days). Offspring lungs were collected at GD120 (n=6-7 per group), and at 24 months of age (n=4-6 per group) fo

Beta-adrenergic receptor mediated inhibitory control of CD16+ monocyte inflammation is associated with neurobehavioral response to social exclusion and peripheral arterial stiffness a function of HIV.

BACKGROUND: Growing evidence suggests the experience of social exclusion is associated with increased risk for cardiovascular disease (CVD), however the psycho-neuro-immune mechanisms involved remain unclear. Peripheral blood mononuclear cells (PBMCs) play an important role in the etiology of peripheral artery disease. Previous research has uncovered beta-adrenergic receptor inhibitory control (BARIC) of inflammation is blunted in PBMCs from individuals at greater risk for CVD. Chronic HIV disease fits within this paradigm given these individuals living with the virus are more likely to report

Efficacy and safety of erythrocytapheresis in chronic mountain sickness: a randomised controlled trial protocol (ESCAPE-CMS).

INTRODUCTION: Chronic mountain sickness (CMS) is a clinical syndrome affecting more than 4 million individuals living at high altitudes, primarily characterised by excessive polycythaemia. Current CMS consensus recommends therapies targeting haematocrit (HCT), such as phlebotomy or isovolemic haemodilution. However, the clinical benefit of HCT-reduction strategies in CMS has yet to be established. This trial aims to evaluate whether erythrocytapheresis, regarded as one of the most precise and effective approaches for HCT-reduction, could improve clinical outcomes in patients with CMS. METHODS

Mitophagy in pathogenesis and therapeutic implications for infection.

Mitophagy, a critical selective autophagy pathway targeting dysfunctional mitochondria, is involved in cellular homeostasis and quality control under various stressors. During infection, invading pathogens cause mitochondrial abnormalities that trigger mitophagy. Most bacteria and viruses can exploit mitophagy as a pro-pathogenic process to escape host antiviral responses during infection. Emerging evidence suggests that restoring mitophagy is beneficial for mitigating excessive inflammation, such as in sepsis, whereas mitophagy blockade can activate host defense during bacterial and viral inf

Non-Proliferative Immune Restoration by Euglena gracilis EK Powder in Cyclophosphamide-Induced Immunosuppressed Mice.

Cytotoxic chemotherapy frequently induces immunosuppression, impairing the innate cellular and humoral immune responses. Therefore, immunomodulatory agents that restore immune function without indiscriminate leukocyte expansion are required. This study examined whether Euglena gracilis EK powder (EKP), a paramylon-rich β-1,3-glucan preparation, supports immune recovery under cyclophosphamide (CPA)-induced immunosuppression. Male BALB/c mice received EKP (80, 200, or 500 mg/kg/day for 21 days) under a dual-temporal CPA design: acute phase (days 7 and 9) and established phase (days 14 and 17). T
