Tenecteplase for Acute Ischemic Stroke: From Molecular Bioengineering to Phase III Pivotal Trials.

Intravenous thrombolysis with alteplase is a primary reperfusion therapy for acute ischemic stroke, yet its clinical utility is constrained by a short half-life, continuous infusion requirements, and susceptibility to plasminogen activator inhibitor-1. Tenecteplase, a bioengineered variant of alteplase, was developed to address these limitations through a prolonged half-life, increased fibrin specificity, and enhanced resistance to plasminogen activator inhibitor-1. These structural modifications enable single-bolus administration and reduce systemic fibrinolytic disturbance. In this review, w

Placental endothelial ferroptosis induced by neutrophil NETosis leads to intrauterine growth restriction in dengue virus infection.

Studies on dengue virus (DENV) infection during pregnancy show that symptomatic dengue is associated with adverse fetal outcomes, but the mechanisms remain unclear. Our previous research indicated that neutrophils contribute to intrauterine growth restriction (IUGR) by damaging placental vasculature, yet the molecular mechanisms underlying this microvascular injury remain poorly defined. To address this, this study examined placental microvascular ferroptosis by bulk and single-cell transcriptomic reanalyses, immunofluorescence staining, and Western blotting in DENV-2-infected E18.5 placentas

Finerenone-Empagliflozin Combination Therapy in Apparent Resistant Hypertension: A CONFIDENCE Trial Post Hoc Analysis.

BACKGROUND: Apparent treatment-resistant hypertension (aTRH) is common in chronic kidney disease (CKD) with type 2 diabetes and predicts worse outcomes. Spironolactone, the established fourth-line agent, is limited by hyperkalemia in CKD. OBJECTIVES: This analysis assessed whether finerenone plus empagliflozin lowers blood pressure (BP) meaningfully in aTRH, and whether aTRH modifies these agents' efficacy and safety. METHODS: Post hoc analysis of CONFIDENCE (NCT05254002), a double-blind randomized trial of finerenone, empagliflozin, or both in CKD with albuminuria and type 2 diabetes. aTRH

FATP5 loss in hepatocellular carcinoma cells orchestrates tumor-promoting microenvironment by inducing passive long-chain fatty acid accumulation within tumor-associated endothelial cells.

Hepatocellular carcinoma (HCC) is characterized by severe immunosuppression and metabolic reprogramming, yet the spatial dynamics of lipid flux between malignant and stromal cells remain poorly understood. Here, we identify a profound, type-specific metabolic rewiring in HCC driven by the widespread downregulation of fatty acid transport protein 5 (FATP5). FATP5 deficiency in malignant cells selectively attenuates the uptake of long-chain fatty acids (LCFAs), creating an extracellular lipid surplus that is passively sequestered by neighboring FATP3-expressing tumor-associated endothelial cells

Panax notoginseng Saponins Accelerate Diabetic Wound Repair by Minimizing Mitochondrial Dysfunction and Inhibiting Pyroptosis of Macrophage via Regulating Ephx2/11,12-EET Axis.

Impaired diabetic wound (DW) healing is mainly driven by excessive inflammatory response, while Panax notoginseng saponins (PNS), the core bioactive components of Panax notoginseng, show pro-healing potential in diabetic ulcers with unclear mechanisms. Here, we established a streptozotocin-induced DW mouse model, and combined untargeted metabolomics with in vitro and in vivo validations to decipher PNS's therapeutic mechanism and active ingredients. In vivo, PNS dose-dependently accelerated wound closure, enhanced collagen deposition, and improved glucose tolerance and insulin sensitivity in d

Intranasal Delivery of an Antioxidant Nanoassembly for Cerebral Ischemia-Reperfusion Injury via Drd2/β-arrestin2/ERK Signaling.

Cerebral ischemia-reperfusion injury (CIRI) is caused by reduced cerebral blood flow, leading to excessive oxidative stress and neurological deficits. Curcumin (Cur) is a promising therapeutic agent, but its antioxidant activity is limited by low bioavailability and poor brain penetration. Herein, an intranasal delivery strategy using a tetrahedral framework nucleic acid (tFNAs)-Cur nanoassembly (tFNAs-Cur) was employed for CIRI treatment. tFNAs-Cur was synthesized through self-assembly, achieving a high Cur loading efficiency of 87% and exhibiting potent scavenging activity against reactive o

Nutritional and metabolic mechanisms linking low protein and low amino acid diets to fatty liver in laying hens.

Fatty liver is a common metabolic disorder in laying hens and is strongly influenced by dietary protein and amino acid supply. Low protein or low amino acid diets disrupt the balance between nitrogen and energy intake, enhancing hepatic de novo lipogenesis. Severe dietary protein and amino acid deficiency also impairs hepatic apolipoprotein synthesis and secretion. These metabolic alterations promote hepatic triglyceride accumulation and increase susceptibility to fatty liver. Although nutrient-sensing mechanisms in birds remain incompletely defined, evidence from mammalian studies suggests th

How to investigate abnormalities in selenium plasma concentrations in humans - Diagnostic algorithms.

BACKGROUND: Selenium is an essential trace element involved in antioxidant defence, thyroid hormone metabolism, immune regulation and cellular homeostasis. Both selenium deficiency and selenium excess are associated with clinically significant disease; however, interpretation of abnormal plasma selenium concentrations is challenging because selenium status is influenced by nutritional, inflammatory, environmental, genetic and systemic factors. OBJECTIVE: To critically synthesise the available evidence on the causes, laboratory assessment and clinical interpretation of abnormal plasma selenium

Gut microbiome composition and cardiometabolic risk among healthcare workers: findings from the EXPONIT study.

BACKGROUND AND AIMS: The gut microbiome plays an important role in metabolic regulation and has been linked to cardiometabolic disorders. Circadian-related factors (night shift work, sleep quality, chronotype) may influence gut microbiome characteristics and cardiometabolic health. We aimed to investigate the associations between gut microbiome diversity and composition and cardiometabolic outcomes, and evaluate whether these associations differ by work schedule, sleep quality, and chronotype. METHODS AND RESULTS: Fecal samples were taken on a working day, and gut microbiota composition was a

Normal-weight obesity and adverse health outcomes in women: a systematic review and meta-analysis of observational studies.

BACKGROUND: Normal body mass index (BMI) can coexist with excess adiposity, a phenotype termed normal-weight obesity (NWO). Female-specific evidence remains fragmented; we synthesized associations between NWO and female adverse health outcomes. METHODS: This was a systematic review and meta-analysis of observational studies. PubMed/MEDLINE, Embase, Scopus, and Web of Science were searched from inception to May 28, 2026, for eligible female-specific studies comparing NWO with normal-weight lean/non-obese controls. Odds ratios (OR) and other effect ratios were synthesized separately; continuous

Sucrose induces Cardiac Hypertrophy and Renal Perivascular Fibrosis in a Novel Mouse Model of Hypertension.

BACKGROUND: Excess sugar consumption increases the risk for hypertension. We previously reported pilot data demonstrating that male mice drinking 2% sucrose water develop hypertension. However, the sucrose effects on left ventricular hypertrophy, kidney fibrosis, or sex difference in hypertension remain unclear. In the present study, we characterized the cardiac and renal pathology in a larger cohort including female mice to establish a novel sucrose-induced mouse model of hypertension associated with cardiovascular remodeling. METHODS: Eight- to nine- week-old C57BL6 mice were given drinking

Apparent mineralocorticoid excess mimicking Bartter syndrome in infancy.

OBJECTIVES: Apparent mineralocorticoid excess (AME) syndrome is a rare monogenic disorder caused by loss-of-function variants in the HSD11B2 gene encoding 11β-hydroxysteroid dehydrogenase type 2 (11β-HSD2). Deficient enzyme activity results in cortisol-mediated activation of the mineralocorticoid receptor, leading to low-renin hypertension. Because affected infants frequently present with polyuria and hypokalemic metabolic alkalosis, AME syndrome is often misdiagnosed as Bartter syndrome, resulting in significant diagnostic delay. CASE PRESENTATION: A 13-month-old girl presented with polyuria

Pilates and obesity: A practical and promising option.

Pilates is a type of mind-body fitness that may lead to significant improvements in body composition, physical fitness, cardiometabolic health, and mental health. However, these findings are supported by very limited and often mixed or scarce evidence. In general, Pilates training may promote exercise engagement of people with larger bodies in a physically and mentally comfortable environment, thereby promoting their health, performance, and well-being. Research suggests that integrating Pilates training as a component of a comprehensive, tailored, supervised exercise program for individuals w

[Sepsis-induced coagulopathy].

Sepsis-induced coagulopathy (SIC) represents a frequent and prognostically highly relevant complication of sepsis, characterized by a dysregulated interaction between inflammation, endothelial activation and coagulation. Pathophysiologically, the excessive host immune response to pathogen-associated molecular patterns (PAMP) and damage-associated molecular patterns (DAMP), activation of the complement cascade, formation of neutrophil extracellular traps (NET) and disruption of the endothelial glycocalyx collectively promote a prothrombotic and antifibrinolytic microenvironment, resulting in mi

Association of clonal hematopoiesis of indeterminate potential with cardiovascular mortality in cardiovascular-kidney-metabolic syndrome: a large prospective cohort study.

BACKGROUND: Cardiovascular-kidney-metabolic (CKM) syndrome represents a progressive pathophysiological continuum. While clonal hematopoiesis of indeterminate potential (CHIP) is known to increase the risk of CKM disease onset and adverse prognoses, its overall and stage-specific impacts on cardiovascular mortality within the CKM framework, alongside the circulating proteomic mechanisms mediating this excess risk, remain largely unknown. METHODS: Participants from the UK Biobank with available CHIP data across the CKM syndrome continuum were included. The primary outcome was cardiovascular mor

Effect of Sleep Hygiene on Metabolic Syndrome in an Adult Rural Population in India: Protocol for a Cross-Sectional Study.

BACKGROUND: Metabolic syndrome (MetS) refers to a group of conditions including high blood pressure, excess weight, high blood glucose, and abnormal cholesterol and triglyceride levels that together substantially increase the risk of heart disease and diabetes. Poor sleep hygiene is associated with a higher likelihood of type 2 diabetes, while sleep duration can affect the development of various MetS components. Approximately 25% of adults worldwide currently have MetS, and this prevalence is projected to increase to 50% by 2035. However, there is a distinct lack of evidence about the specific
