ImmunoCAP-Based Detection of Alpha-Gal Syndrome: A Reproducible Diagnostic Protocol for Human Patients.

Alpha-GAL syndrome (AGS) is a type of allergy to mammalian meat and other foods or pharmaceuticals made from mammalian substrates. It is an IgE-mediated reaction caused by sensitization to the polysaccharide galactose-α-1,3-galactose (alpha-GAL) found in tick saliva after a previous tick bite. Alpha-GAL is present on proteins of mammals except primates. Sensitization to alpha-GAL in tick saliva predisposes to cross-reactivity following exposure (oral or non-oral) to glycated galactose-α-1,3-galactose proteins, regardless of their origin. Symptoms of varying severity (from dyspepsia to anaphyla

Protocols for Diagnosis and Management of Arthropod Bite Allergies in Humans.

Allergic diseases have increased significantly in recent decades, with a higher incidence of severe manifestations such as anaphylaxis. This increase is attributed mainly to environmental factors associated with the Western lifestyle that alter the epithelial barrier, promoting sensitization to allergens, such as those from arthropods. Hymenoptera (bees and wasps) are the most implicated arthropods. The sting introduces a venom composed of highly sensitizing proteins that are capable of triggering reactions ranging from local and mild to potentially fatal systemic reactions. The incidence is h

Untargeted UHPLC-HRMS Lipidomic Profiling of Ticks for Alpha-Gal Syndrome Research.

Lipids are a fundamental group of biomolecules that play diverse roles in biological systems, including energy storage, membrane architecture, and intra- and extracellular signaling. They are classified into eight major categories, and more than 50 000 distinct lipid structures are currently reported in the Lipid Maps Structure Database (LMSD). Given their importance in cellular metabolism and physiology, accurate identification and quantification of lipids are essential. In ticks, lipid metabolism is closely associated with physiological processes related to survival, pathogen transmission, a

The Zebrafish Model for Alpha-Gal Syndrome.

Zebrafish (Danio rerio) has emerged as a robust vertebrate model for studying immune and allergic disorders, including food allergies. The alpha-Gal syndrome (AGS) is a tick-borne allergy characterized by an IgE-mediated delayed hypersensitivity to the oligosaccharide galactose-α-1,3-galactose (α-Gal), which is present in tick saliva and mammalian-derived foods and drugs. This chapter describes the experimental procedures used to establish and analyze the validated zebrafish model for AGS. Adult zebrafish were intramuscularly injected with Ixodes ricinus tick saliva and maintained on a mammali

Proteomics Analysis for Discovery of Biomarker of Tick Allergy.

The alpha-Gal syndrome (AGS) is an allergic disease with IgE-mediated delayed hypersensitivity to the oligosaccharide galactose-α-1,3-galactose (α-Gal) present in tick saliva and mammalian-derived foods and drugs. The incidence of AGS is increasing worldwide and thus requires the identification of serum biomarkers and tick biomolecules for the development of preventive, treatment, and control interventions. In this chapter, we describe step-by-step methods for proteomics analysis of human serum samples for AGS-related biomarker identification and characterization, characterization of molecular

Protective Pathogen Infection Against Tick-Induced Allergy in Zebrafish.

The α-Gal syndrome (AGS) links tick bites with delayed anaphylaxis to mammalian meat consumption. Recent work in zebrafish shows that the Ixodes ricinus salivary metalloprotease (MET) and allergen-like p23 trigger AGS-like responses, whereas subsequent infection with the mycobacterial fish pathogen, Mycobacterium marinum, skews immunity towards a protective Th1 profile. Here we provide a step-by-step protocol to: (i) reproduce tick-induced allergy in adult zebrafish, (ii) superimpose water-borne mycobacterial infection, and (iii) quantify clinical, serological, and molecular outcomes. The meth

Clinical Protocol for Evaluating Ectoparasite-Induced Allergies with Emphasis on Alpha-Gal Syndrome.

Alpha-gal syndrome (AGS) is an ectoparasite-associated IgE-mediated hypersensitivity in which reactions occur after exposure to galactose-alpha-1,3-galactose (alpha-Gal) in mammalian foods, biologics, medications, and other mammalian-derived products. The delayed onset after oral exposure and the imperfect correlation between sensitization and clinical allergy complicate diagnosis. This chapter provides a practical clinical protocol for evaluating suspected ectoparasite-induced allergy with emphasis on AGS. The protocol integrates structured history taking, tick and other ectoparasite exposure

Mortality and disability after hospitalisation for injuries in low-income and middle-income countries: a cohort study.

BACKGROUND: Mortality and disability after injuries are high in low-income and middle-income countries (LMICs). Improving outcomes for injured patients requires knowledge of their associated factors. We aimed to assess post-injury mortality and disability and their associations with access to health care across LMICs with different injury burdens, health-care systems, and contexts. METHODS: Hospitalised patients in Ghana, Pakistan, Rwanda, and South Africa with moderate-to-severe injuries were recruited and followed up until 3 months after discharge or death. Injured patients of any age who w

Loss of EGFR activity in skeletal stem/progenitor cells is associated with impaired fracture healing in aged mice.

The cellular and molecular events responsible for fracture healing becoming delayed with aging remain unclear. Epidermal growth factor receptor (EGFR) signaling has been reported to play a critical role in bone regeneration. However, knowledge of its specific function in skeletal stem cells during aging-induced fracture delayed union remains scant. In the present study, we first demonstrated that EGFR activity in skeletal stem/progenitor cells decreased as mice aged and thus resulted in delayed fracture healing. To further investigate whether the EGFR signaling can be targeted as a potential t

Pre-hospital time and risk of perforation in patients with acute appendicitis in 112 countries (AlliGatOr): an international, prospective, observational cohort study.

BACKGROUND: Acute appendicitis is the most common surgical emergency worldwide. Concern that delayed treatment might increase perforation can lead to unnecessary surgery and high rates of negative appendicectomy, where a normal, non-inflamed appendix is removed. However, whether perforation is related to pre-hospital time (symptom onset to first surgical assessment) has not been explored in contemporary global data. METHODS: We conducted an international, multicentre, prospective, observational cohort study (AlliGatOr) of patients undergoing emergency appendicectomy for suspected acute append

Advances in tuberculosis treatment: novel regimens, new drug pipelines, host-directed therapies, and updated management guidelines.

Tuberculosis treatment has undergone its most profound transformation since the launch of the standardised DOTS strategy in 1994. Since 2020, a series of pivotal randomised trials, including TB-PRACTECAL, ZeNix, Nix-TB, endTB, BEAT-TB, SHINE, and Study 31/A5349, have redefined the management of both drug-susceptible and drug-resistant tuberculosis. These studies have enabled shorter, fully oral regimens with improved efficacy and safety across adult and paediatric populations, including people with HIV, and have driven major updates to WHO treatment guidelines. Despite these advances, tubercul

A Conditional Knockout Mutant Approach to Study Essential Genes in Chlamydia in Cell Culture and In Vivo.

Identification and characterization of essential Chlamydia-specific factors are vital for the generation of effective therapeutics and vaccines. Despite delays in the development of genetic tools for Chlamydia, significant advancements in Chlamydia genetic manipulation have been made over the last decade and a half, now allowing for the targeted interruption or deletion of chromosomal genes. However, because of the obligate intracellular nature of the bacteria and its unique biphasic developmental cycle, it remains challenging to inactivate essential genes and study the function of their encod

CPK10 phosphorylates zinc-sensing transcription factors bZIP19/23 to establish time-delayed negative feedback in plant zinc homeostasis.

Zinc (Zn) homeostasis is essential for plant growth; yet, how zinc-deficiency-triggered calcium (Ca) signals are decoded into homeostatic adjustments remains elusive. Here, we identify the calcium-dependent protein kinase 10 (CPK10) as a key decoder of zinc-deficiency-triggered Ca2+ signals. Under zinc-deficient conditions, both the protein level and the kinase activity of CPK10 increase. Subsequently, CPK10 interacts with and phosphorylates the zinc-sensing transcription factors bZIP19 and bZIP23, thereby repressing their DNA-binding and transcriptional activities. Zrt/Irt-like protein (ZIP)

Centrosymmetric Molecular Strategy With Quadruple Boron-Nitrogen Heteroarenes Enabling Efficient Ultra-Narrowband Emission.

Realizing ultra-narrowband multiple-resonance thermally activated delayed fluorescence (MR-TADF) with long-wavelength emission remains a formidable challenge due to the increased vibronic coupling and excited-state relaxation with the extension of π-conjugated framework. In this contribution, a proof-of-concept of centrosymmetric molecular strategy fused with quadruple boron-nitrogen heteroarenes is proposed to develop ultra-narrowband long-wavelength MR-TADF emitters. Thus, two MR-TADF emitters, called DCz-MR and DAn-MR, are prepared and characterized. Compounds DCz-MR and DAn-MR exhibit the

Leveraging existing grid infrastructure for rapid US renewable and storage deployment.

Rapid electricity demand growth, grid connection delays, and supply chain constraints create a serious risk of supply shortages and rising electricity costs in the next few years. This paper studies the strategy of adding colocated renewables and storage at existing power plants to take advantage of spare grid capacity. Using satellite imagery, we assess solar and wind potential near 3,890 U.S. fossil and renewable plants. By 2030, we find that existing fossil plants could host up to 597 GW of economically viable solar and wind without exceeding the existing interconnection capacity of each si

Scientists disrupt less but more persistently with career aging.

Disruption describes the extent to which a scientific work overturns the prior knowledge on which it builds and leads to a new line of research. A scientist's tendency to produce disruptive work has been found recently to decline over the career. However, the disruption index captures only how strongly a paper overturns prior work and ignores whether that disruption is later accepted or overturned by subsequent research. The persistent disruption (PD) framework adds this second perspective, identifying work that disrupts prior research and is then consolidated rather than displaced by later wo

Unwarranted Delegation to Artificial Intelligence of Prior Authorizations for Select Medicare Treatments.

The Centers for Medicare & Medicaid Services (CMS) seeks to leverage the unprecedented capabilities of artificial intelligence (AI) through its newest Innovation Center approach known as the Wasteful and Inappropriate Services Reduction Model (WISeR). WISeR will redefine traditional Medicare claim approval operations through AI-driven prior authorization requirements imposed on beneficiaries seeking certain medical services. WISeR exposes beneficiaries to improper claim denials and delayed or forgone care. However, its greatest flaw is that it cannot be legally classified as a "model" at all.

CgFre2 contributes to ferric reductase activity, growth and pathogenicity in Colletotrichum gloeosporioides.

Ferric reductases are oxidoreductases that catalyze the reduction of environmental Fe³ ⁺ to bioavailable Fe² ⁺ and mediate cellular iron uptake and metabolism. However, the specific biological functions of ferric reductases in Colletotrichum gloeosporioides have not been fully characterized. In this study, 15 ferric reductase-like proteins were screened from the genome annotation of C. gloeosporioides, and CgFre2 was selected for further functional characterization. Targeted deletion of CgFre2 resulted in reduced mycelial growth and a sharp decrease in conidiation. Further observation of conid
