Ophelia Syndrome: Acute Stupor as a Primary Manifestation of Hodgkin Lymphoma Diagnosed by Fluorodeoxyglucose Positron Emission Tomography.

Ophelia syndrome is a rare form of paraneoplastic limbic encephalitis associated with Hodgkin lymphoma, mediated by autoantibodies targeting the metabotropic glutamate receptor type 5 (mGluR5). It is characterized by a spectrum of neurological and psychiatric manifestations. Impaired consciousness progressing to stupor as the presenting feature is uncommon and, combined with a non-diagnostic structural MRI, can substantially delay diagnosis and treatment. We report a 59-year-old male with recently diagnosed classical Hodgkin lymphoma who developed acute unexplained stupor after recovery from a

Eco-Evolutionary Thinking in Metastatic Breast Cancer: A Clinician's Primer on Why Tumors Outsmart Us and How to Fight Back Smarter.

Metastatic breast cancer (MBC) remains incurable because of tumor evolution. Despite advances in targeted therapies-CDK4/6 inhibitors, PIK3 inhibitors, HER2-directed agents, and antibody-drug conjugates-median overall survival is modest, with resistance emerging in nearly all patients. From an evolutionary perspective, standard continuous maximum tolerated dose (MTD) therapy may create intense selective pressure that reduces treatment-sensitive cells and, in some incurable and resistance-prone settings, could permit competitive release of resistant clones. While MTD remains the guideline-endor

Review article: Immunosuppression in Decompensated Autoimmune Hepatitis Cirrhosis - To Suppress or Not to Suppress?

BACKGROUND: The management of decompensated autoimmune hepatitis (AIH) cirrhosis is one of the most challenging and controversial scenarios in hepatology. Clinicians face a high-stakes dilemma: whether to initiate immunosuppression to halt inflammatory activity or prioritize liver transplantation (LT) referral to avoid treatment futility and life-threatening infections. AIMS: To critically appraise the evidence regarding immunosuppression in decompensated AIH cirrhosis and propose a biologically grounded, risk-stratified framework for clinical decision-making. METHODS: This review synthesize

Case Report: Hereditary ATTR-CA presenting with recurrent severe abdominal cramps: a diagnostic challenge.

BACKGROUND: Hereditary transthyretin cardiac amyloidosis (ATTR-CA) is a rare autosomal dominant systemic disorder characterized by amyloid deposition in the peripheral nerves, myocardium, kidneys, and gastrointestinal tract. The clinical manifestations of ATTR-CA are nonspecific, resulting in high rates of misdiagnosis and underdiagnosis. CASE PRESENTATION: We report the case of a 58-year-old woman who presented with recurrent severe abdominal pain and echocardiographic evidence of ventricular hypertrophy. Initial cardiac magnetic resonance imaging at an outside institution suggested left ven

Hyperbaric Oxygen Therapy for Diabetic Lower-Limb Wounds in the Caribbean: A Retrospective Cohort Study.

Hyperbaric oxygen therapy (HBOT) is an established adjunctive treatment for selected diabetic foot ulcers, although evidence regarding its effectiveness remains heterogeneous. We conducted a retrospective, single-center, observational study of adult diabetic patients receiving HBOT (2.5 ATA, 100% oxygen) for a lower-limb wound at the only hyperbaric center in Guadeloupe, a French Caribbean territory, between January 2020 and June 2023. Among 257 patients treated with HBOT, 71 diabetic patients met inclusion criteria (mean age 63.1 ± 12.7 years; 69% male). Peripheral arterial disease and osteit

Disappearing Colorectal Liver Metastases: Should the Treatment Sequence Be Reconsidered for Selected Lesions?

Disappearing liver metastases (DLMs) remain a challenging problem in the management of colorectal liver metastases (CRLMs). Although modern systemic therapy may produce complete radiological responses, radiological disappearance does not reliably indicate complete pathological response. Conversely, mandatory treatment of every original metastatic site may cause unnecessary parenchymal sacrifice when a lesion is no longer identifiable on hepatobiliary contrast-enhanced magnetic resonance imaging or intraoperative ultrasonography. Current strategies-including treatment of the presumed site, sele

Adjunctive Hyperbaric Oxygen Therapy for a Refractory Sickle Cell-Associated Leg Ulcer: A Case Report.

Sickle cell leg ulcers (SCLUs) are a chronic and debilitating complication of sickle cell disease (SCD), often associated with severe pain, impaired mobility, delayed healing, and recurrence. Their management remains challenging because ulcer formation and persistence are driven by multiple overlapping mechanisms, including hemolysis-associated vasculopathy, microvascular dysfunction, chronic inflammation, venous disease, and local tissue hypoxia. Despite standard measures such as local wound care, compression therapy, infection control, analgesia, and optimization of SCD-directed treatment, m

Delayed innominate vein perforation and massive hemothorax following left-sided PICC placement: A case report.

RATIONALE: Peripherally inserted central catheters are common venous access devices; however, they can cause rare and life-threatening mechanical complications. Delayed vascular perforations often present with nonspecific findings, making early diagnosis challenging. Herein, we report a case of delayed innominate vein perforation, highlighting the importance of careful interpretation of serial chest radiographs. PATIENT CONCERNS: An 82-year-old woman with acute ischemic stroke underwent bedside left-arm placement of a peripherally inserted central catheter. The initial postprocedural chest ra

Innovative Compounded Topical Powder for Post-Digital Amputation Care: A Clinical Case Study in Wound Healing.

Diabetes is a leading global cause of mortality and disability, with foot complications often resulting in amputation. Digital (toe) amputations are particularly common due to neuropathy, ischemia, and infection, and are frequently followed by delayed healing. This case describes a 66-year-old diabetic male who underwent multiple toe amputations on his right foot. Despite successful surgery, wound healing was slow. A compounded topical powder was prescribed, containing phenytoin, piperacillin, tazobactam, misoprostol, aloe vera, and beta glucan in a novel powder-to-hydrogel, film-forming topic

Endoscopic Management of Biliary Strictures After Liver Transplantation: Current Evidence and Emerging Approaches.

Liver transplantation is a definitive treatment for end-stage liver disease and can improve quality of life and long-term prognosis. However, postoperative biliary complications remain an important limitation to transplant outcomes, and biliary stricture is among the most prevalent. Patients may present with jaundice, abdominal pain, recurrent cholangitis, and abnormal liver function, while delayed treatment may progress to biliary cirrhosis or graft failure. Endoscopic retrograde cholangiopancreatography (ERCP) has become the first-line therapeutic approach because it is minimally invasive, r

Pancytopenia as the Initial Presentation of Celiac Disease.

Celiac disease is an immune-mediated enteropathy precipitated by gluten in genetically susceptible individuals. Although diarrhea, weight loss, and malabsorption are classical features, extraintestinal presentations are increasingly recognized. Pancytopenia as the initial manifestation of adult celiac disease is rare and often delays diagnosis. We report a 39-year-old woman from Assam, Northeast India, who presented with chronic diarrhea, weight loss, and progressive weakness, and was found to have severe pancytopenia (hemoglobin 3.9 g/dL, leukocytes 1,200/mm3, and platelets 23,000/mm3). Nonpharmaceutical interventions and postpandemic resurgence: the shifting epidemiology of influenza, respiratory syncytial virus, and adenovirus in Hangzhou, Zhejiang Province (2020-2024).

Acute respiratory infections caused by influenza A/B (Flu A/Flu B), respiratory syncytial virus (RSV), and adenovirus (ADV) remain a major global health burden. The COVID-19 pandemic profoundly altered respiratory virus transmission through nonpharmaceutical interventions (NPIs). Samples from patients with respiratory tract infections at Hangzhou Hospital of Traditional Chinese Medicine from January 2020 to October 2024 were collected. ADV, Flu A, Flu B, and RSV were detected to evaluate the impact of NPIs on viral epidemiology. Statistical analyses were performed using SPSS software and Graph

Mitochondrial Dysfunction: A Critical Link Between Maternal Diet and Offspring Metabolic Health.

Background/Objectives: The developmental origins of health and disease (DOHaD) theory suggests that intrauterine and early postnatal life represents a critical window for programming lifelong health trajectories and disease susceptibility in offspring. Maternal nutritional imbalance during this period is closely associated with obstetric complications and an elevated risk of metabolic disorders in children. As central metabolic hubs, mitochondria constitute a critical axis linking adverse in utero exposure to metabolic defects in offspring across generations. Methods: In this narrative review,

Non-Invasive Skin Imaging Techniques for Diagnosing and Monitoring Mycosis Fungoides: A Comprehensive Review.

Mycosis fungoides, the most common primary cutaneous T-cell lymphoma, presents notable diagnostic challenges. In its early stages, it may resemble inflammatory dermatoses both clinically and histologically, causing delays in diagnosis. These factors can lead to treatment delays, allowing the disease to progress and adversely affecting the patient's quality of life. To address this issue, the use of skin imaging techniques like dermoscopy, reflectance confocal microscopy, optical coherence tomography, and high-frequency ultrasound has increased in recent years. These techniques assist in diagno

The Role of the KLF Family in T-Cell-Mediated Regulation of Cardiovascular Diseases: Molecular Mechanisms and Therapeutic Prospects.

Cardiovascular diseases are increasingly recognized as immune-inflammatory disorders in which adaptive immunity shapes tissue injury, repair, and long-term remodeling. T cells are central to these processes because they integrate antigen recognition, lineage-defining transcriptional programs, tissue trafficking, cytokine production, and immunological memory. In this Review, we synthesize current evidence on the Krüppel-like factor (KLF) family as a transcriptional framework linking T-cell biology to cardiovascular disease. KLF2 primarily regulates T-cell quiescence and trafficking, KLF10 suppo

Chronic pruritus as the presenting symptom of Hodgkin lymphoma: A case report.

A 25-year-old woman presented to the emergency department (ED) with acute chest tightness and dyspnea superimposed on a one-year history of diffuse, treatment-refractory pruritus that had been managed as eczema at two prior visits. She also reported night sweats and unintentional weight loss. Laboratory studies showed marked leukocytosis, transaminitis, critical hypokalemia and hypomagnesemia, and an elevated lactate dehydrogenase. No lymphadenopathy was palpable on examination. Chest radiography and computed tomography revealed a large, locally invasive right upper lobe mass with mediastinal,

Management of Melanoma in Advanced Age: A Comprehensive Literature Review of Tumor Biology, Systemic Therapies and Geriatric Challenges.

Background: Melanoma disproportionately affects older adults, who account for a growing share of cases as populations age. Its incidence rises with age, and the number of ultra-octogenarians (≥85 years) is expected to triple by 2050, particularly in high-incidence regions such as Australia, North America and Europe. Moreover, older adults frequently present with substantial comorbidities, making management complex and complicating treatment decisions. Methods: This comprehensive review summarizes evidence concerning the clinicopathological characteristics, treatment patterns, systemic therapy
