Maternal risk factors for hypertension in pregnancy: A quantitative study on the role of demographics, obesity, and healthcare compliance.

Hypertension in pregnancy is a major contributor to maternal and perinatal morbidity and mortality. This study examined the associations between maternal risk factors and hypertension in pregnancy. A prospective observational cohort study was conducted at Wonoayu Health Center, Sidoarjo, Indonesia, from November 2024 to January 2025. Fifty-seven pregnant women with a gestational age of at least 28 weeks and a positive mean arterial pressure screening result were recruited through consecutive sampling. Data were collected from medical records, interviews, adherence cards, physical examinations,

Early preeclampsia screening and prevention: An update for general practice.

BACKGROUND: Preeclampsia is a disorder of pregnancy that causes significant adverse perinatal outcomes. Risk factor-based screening performs poorly when compared with algorithm-based approaches. The role of general practitioners in early screening for preeclampsia is crucial in predicting and preventing the condition. OBJECTIVE: This article discusses the different screening approaches and interventions available for prevention of preeclampsia, making the case for a screen-and-treat model based on individual risk calculation for all pregnant women. DISCUSSION: The Fetal Medicine Foundation (

Women's gynaecological and reproductive health in UK nephrology: gaps in confidence, training and service delivery from a UK National Survey.

BACKGROUND: Women with chronic kidney disease (CKD) face unique gynaecological and reproductive health challenges, including menstrual and menopausal disturbances, fertility issues, and increased pregnancy-related risks. Despite the clinical relevance of these issues, it is unclear how well equipped nephrologists are to address them in routine practice. METHODS: We conducted a national cross-sectional survey of UK nephrologists and renal trainees to assess their confidence, clinical experience, and perceived barriers in managing women's health within the context of CKD. The survey captured re

Aspirin in preeclampsia prevention: a narrative Review of clinical and mechanistic evidence.

BACKGROUND: Low-dose aspirin reduces preterm preeclampsia, but its mechanisms remain uncertain because circulating concentrations of unchanged aspirin are substantially lower than those used in many experimental studies. This narrative review evaluates proposed mechanisms according to their clinical relevance and compatibility with prophylactic exposure. METHODS: Evidence concerning platelet inhibition, placentation, inflammatory resolution, angiogenic regulation and epigenetic modification was narratively reviewed, with particular attention to experimental concentrations, model limitations a

Race and ethnicity-specific differences in incidence of pre-eclampsia with severe features: Assessment per socioeconomic status.

In this cross-sectional study of 18,969,287 hospital deliveries identified in Healthcare Cost and Utilization Project's National Inpatient Sample from 2016 to 2022, Black pregnant people had higher incidence rates of developing pre-eclampsia with severe features compared to White pregnant people across all the socioeconomic strata. The effect sizes for Black pregnant people comparing to White pregnant people were similar and consistent across census-level household income and insurance status. These data suggest that the development of pre-eclampsia with severe features may be driven by a comb

Increasing prescribing of acetylsalicylic acid in pregnancy: Program evaluation in Saskatchewan.

OBJECTIVE: To determine the proportion of patients with a singleton pregnancy who are at risk for preeclampsia, based on a Society of Obstetricians and Gynaecologists of Canada guideline that reclassified a body mass index value greater than 30 kg/m2 as a stand-alone indication for daily acetylsalicylic acid (ASA) prophylaxis, and to assess the impact of a single educational session on clinicians' prescribing of ASA prophylaxis. DESIGN: A pre-post study evaluated prescribing of preventive ASA for patients at risk for preeclampsia, as documented in electronic medical records (EMRs) before and

Postpartum macrophage activation syndrome with temporal dissociation of cytopenias in a patient with Kikuchi-Fujimoto disease and undifferentiated connective tissue disease: a case report.

BACKGROUND: Macrophage activation syndrome (MAS) is a life-threatening hyperinflammatory disorder; postpartum MAS is extremely rare and often misdiagnosed due to overlap with sepsis and HELLP syndrome. Kikuchi-Fujimoto disease (KFD) and undifferentiated connective tissue disease (UCTD) may increase susceptibility, but reports are scarce. This case highlights the diagnostic challenges of postpartum MAS in the setting of overlapping inflammatory conditions. CASE DESCRIPTION: The patient was a 35-year-old primiparous woman with KFD and UCTD who underwent cesarean section for preeclampsia and cer

Management of pregnancy in patients with Graves' disease secondary to alemtuzumab: a case series and literature review.

INTRODUCTION: Alemtuzumab treatment for multiple sclerosis frequently induces Graves' disease (GD), characterized by common coexistence of stimulating and blocking anti-thyrotropin receptor autoantibodies (TRAb), causing fluctuations of thyroid function. Alemtuzumab-induced TRAb-positive hypothyroidism is also common. These conditions are challenging to manage during pregnancy, considering the imperative necessity to maintain euthyroidism to minimize risks for both pregnancy and fetal/neonatal outcomes. METHODS: We reviewed the clinical course and therapeutic management of four pregnant women

Spontaneous retroperitoneal haematoma in pregnancy secondary to ruptured ovarian artery aneurysm: a case report.

Rupture of an ovarian artery aneurysm is a rare but potentially life-threatening cause of retroperitoneal haemorrhage during pregnancy. Most reported cases have occurred postpartum in multiparous women. This report describes an unusual case in a 26-year-old primigravid woman with symptom onset in the second trimester. She presented with recurrent left-sided abdominal pain from 21 + 6 weeks of gestation. Following repeated normal initial assessments, CT angiography at 31 + 6 weeks revealed a ruptured left ovarian artery aneurysm with retroperitoneal haemorrhage. She underwent a successful trans

Prenatal Cannabinoid Exposure Induces Sex-Specific Alterations in Fetal Growth and Placental Lipid Metabolism Gene Expression.

BACKGROUND: Prenatal cannabis use is becoming increasingly more commonplace. However, cannabis exposure is linked to adverse pregnancy outcomes, including gestational hypertension, preeclampsia, and preterm birth. The aim of this study was to determine the morphological and molecular effects of prenatal cannabinoid exposure on the placenta. METHODS: Pregnant Sprague-Dawley rats were exposed daily to vaporized THC (100 mg/mL) starting at gestational day (GD)5 until GD19, when dams were sacrificed and fetuses and placentas collected. Fetuses were genotyped for genetic sex, and transcriptomic an

First trimester ultrasound.

First-trimester ultrasound, performed between 11 and 14 weeks' gestation, has become a cornerstone of modern prenatal care, supporting early risk assessment and guiding subsequent pregnancy management. This review summarises current principles, clinical applications, and practical considerations in first-trimester ultrasound, including confirmation of viability, accurate pregnancy dating, determination of chorionicity in multiple pregnancies, and early anatomical assessment. The scan also enables screening for chromosomal abnormalities through measurement of nuchal translucency and evaluation

A Rare Event Causing Blindness in an Unrecognised Pregnancy: A Case Report.

Purtscher-like retinopathy (PLR) is a rare, non-traumatic microembolic angiopathy characteristically associated with systemic endothelial conditions, including pre-eclampsia. While PLR typically exhibits a self-limiting course with a highly favourable visual prognosis, severe cases result in permanent, treatment-refractory blindness. We present a catastrophic, atypical case of bilateral PLR in an unrecognised pregnancy without any antenatal care. A 31-year-old primigravida without prior antenatal screening presented with a three-day history of sudden, bilateral central blurry vision and was ne

Elevated lipoprotein(a) and adverse pregnancy outcomes.

Lipoprotein(a) [Lp(a)] is gaining attention as a potential marker for adverse pregnancy outcomes, in addition to its well-established role in cardiovascular disease. This review explores the evolving understanding of Lp(a) in pregnancy, focusing on its links to hypertensive disorders of pregnancy, gestational diabetes mellitus, fetal growth restriction, and preterm birth, while also addressing diagnostic and management challenges. During pregnancy, Lp(a) levels rise substantially, nearly doubling between weeks 10 and 35. Although this increase is universal, its clinical implications are unclea

Glycosylated Fibronectin Compared With sFlt-1/PlGF Ratio for Preeclampsia Assessment.

BACKGROUND: The ratio of sFlt-1 (soluble fms-like tyrosine kinase 1) to PlGF (placental growth factor) is an established biomarker for preeclampsia diagnostic triage. GlyFn (glycosylated fibronectin) measured by point-of-care testing is an alternative rapid assessment from finger-stick blood samples. This study was designed to test the hypothesis that the sensitivity of GlyFn is noninferior to that of the sFlt-1/PlGF ratio in women with suspected preeclampsia. METHODS: This prospective observational study evaluated the diagnostic accuracy of both tests for preeclampsia in high-risk participan

Mapping Hydrogen Sulfide Biosynthesis and Oxidation Pathways in Placental Health and Disease: a Scoping Review.

Hydrogen sulfide (H2S) plays an important role in placental function. Its bioavailability depends on the balance between enzymatic synthesis by cystathionine β-synthase (CBS), cystathionine γ-lyase (CSE/CTH) and 3-mercaptopyruvate sulfurtransferase (3-MST/MPST), and mitochondrial oxidation through sulfide:quinone oxidoreductase (SQOR), persulfide dioxygenase (ETHE1) and thiosulfate sulfurtransferase (TST). However, the extent to which H2S synthesis, oxidation and experimental modulation have been investigated in placental systems remains unclear. This scoping review mapped the placental H2S li

Hypertensive disorders of pregnancy: epidemiology, pathophysiology, therapeutic strategies, and maternal-offspring outcomes.

Hypertensive disorders of pregnancy (HDP) encompass chronic hypertension, gestational hypertension, preeclampsia, and preeclampsia superimposed on chronic hypertension, affecting about 5% to 10% of pregnancies worldwide and remaining a major cause of maternal and perinatal morbidity and mortality. Their burden is increasing with rising obesity, advanced maternal age, evolving diagnostic criteria, and marked socioeconomic and racial-ethnic disparities. This review summarises current evidence on the epidemiology, pathophysiology, biomarkers, clinical management, and long-term outcomes of HDP, hi
