Obstetric and thrombotic outcomes among pregnant patients with myeloproliferative neoplasms: a propensity score matched real-world analysis.

Pregnancies complicated by BCR::ABL1-negative myeloproliferative neoplasms (MPNs) are associated with increased maternal and thrombotic risk; however, large real-world comparative data remain limited. We performed a multicenter retrospective propensity score-matched cohort study using the TriNetX database to evaluate maternal, obstetric, and thrombotic outcomes among pregnant patients with MPN compared with matched non-MPN controls. Baseline demographics, comorbidities, and antithrombotic medication use were balanced after matching (standardized mean difference < 0.1). Among 4,467,127 pregnanc

Maternal PM2.5 exposure induces preeclampsia-like phenotypes in mice through placental angiogenic dysfunction and lncRNA-RUNX1 dysregulation.

Preeclampsia (PE) is an idiopathic gestational disorder defined by placental structural abnormalities, systemic inflammatory activation, angiogenic factor imbalance, and endothelial dysfunction. While existing epidemiological studies have established a correlational association between maternal fine particulate matter (PM2.5) exposure and elevated PE risk, the specific underlying molecular mechanisms driving this association have not been fully elucidated. Using a C57BL/6J mouse model of maternal PM2.5 exposure via intranasal instillation (3 mg/kg body weight, every other days from gestational

3D Placental Bed Perfusion Ultrasound: Development and Internal Validation of Trimester-Specific Prediction Models for Early- and Late-Onset Preeclampsia.

This study aimed to develop trimester-specific and combined prediction models for early-onset preeclampsia (EOPE) and late-onset preeclampsia (LOPE) based on maternal clinical characteristics, uterine artery pulsatility index (UtA-PI), and 3D placental parameters, identify the optimal models, and construct static nomograms. A total of 1602 singleton pregnant women were prospectively enrolled from October 8, 2020 to May 31, 2025. All maternal clinical data were collected and participants were followed up until delivery. UtA-PI and 3D placental ultrasound parameters, including placental volume (

Refractory Acquired Thrombotic Thrombocytopenic Purpura in the Third Trimester of Twin Pregnancy: A Case Report.

BACKGROUND: Acquired thrombotic thrombocytopenic purpura (TTP) is a rare, life-threatening condition triggered by pregnancy, often in the third trimester, and frequently mimics preeclampsia or HELLP syndrome due to shared features like thrombocytopenia. Refractory cases in twin pregnancies, described in this case, pose unique diagnostic and therapeutic challenges, requiring advanced interventions. CASE PRESENTATION: A 29-year-old woman carrying dichorionic-diamniotic twins presented in the third trimester with palpitations, dyspnea, oral bleeding, numbness, chest heaviness, fatigue, edema, an

Evolving Fetal Biometry in Fetal Intrahepatic Portosystemic Shunt With Late-Onset Preeclampsia and Focal Umbilical Cord Hypercoiling: A Case Report.

Fetal intrahepatic portosystemic shunt may complicate fetal growth assessment, particularly when maternal and umbilical cord abnormalities coexist. We report a pregnancy first evaluated at our center at 26 + 3 weeks for suspected severe early-onset fetal growth restriction, with estimated fetal weight below the 1st percentile and abdominal circumference below the 3rd percentile. Prenatal ultrasound and fetal magnetic resonance imaging findings were consistent with fetal intrahepatic portosystemic shunt. Subsequent serial assessments showed improved fetal biometry. Routine umbilical artery and

Prenatal exposure to hypertensive disorders and adolescent brain white matter development.

BACKGROUND: Hypertensive disorders of pregnancy (HDP) are associated with altered in utero neurodevelopment, yet long-term impact on adolescent white matter architecture remains unclear, particularly by HDP subtype and sex. While preeclampsia/eclampsia (P/E) is clinically more severe, pregnancy-related hypertension (PR-HTN) may involve longer in-utero exposure to hemodynamic stress. This study examined how distinct HDP exposures influence longitudinal white matter measures in adolescents. METHODS: Using a longitudinal cohort with two MRI timepoints (baseline ages 9-11 years; 2-year follow-up

Development and External Validation of Mini PIERSdynamic Model for Predicting the Adverse Maternal Outcome of Preeclampsia in Low-Resource Settings.

BACKGROUND: Preeclampsia remains a leading cause of maternal mortality, particularly in low-resource settings. We aimed to develop a dynamic prediction model accommodating repeated measurements for predicting adverse maternal outcomes in preeclampsia. METHODS: We utilized data from 7283 women with preeclampsia from the PIERS studies (Preeclampsia Integrated Estimate of Risk). Data from 6548 women were randomly split into a training set (80%) and an internal validation set (20%), while an independent cohort of 735 women was used for external validation. The outcome was a composite of maternal

Gestational Choriocarcinoma With Inferior Vena Cava Tumor Thrombus Formation.

Gestational choriocarcinoma with inferior vena cava (IVC) tumor thrombus formation is exceedingly rare. We report a case initially presenting with recurrent nausea and vomiting in a 52-year-old woman. Laboratory studies revealed elevated serum β-hCG levels, and ultrasonography demonstrated a pelvic mass extending from the parametrial area into the IVC. Pathological examination of a pelvic mass biopsy confirmed the diagnosis of choriocarcinoma. Further assessment identified pulmonary hypertension and emboli within small pulmonary artery branches. The patient received EMA-CO chemotherapy and ach

Repeat administration of magnesium for an exacerbation of postpartum preeclampsia: a national survey.

OBJECTIVE: Magnesium (Mg) infusion is the first-line agent for seizure prophylaxis in patients with preeclampsia with severe features (PESF). However, no guidance exists regarding utility of an additional course of Mg for patients with an exacerbation of PESF (defined as an acute worsening of clinical status) who already completed an initial course of Mg. We aimed to determine the national trend among academic institutions for administration of a second course of Mg in patients with an exacerbation of PESF. STUDY DESIGN: A REDCap survey, developed to identify institutional protocols and antic

Expression patterns of circulating microRNA in confirmed early-onset and late-onset pre-eclampsia cases.

PURPOSE: The aim of our study was to investigate selected pre-eclampsia (PE)-related miRNAs in clearly defined early- and late-onset PE (eoPE, loPE) cohorts and compare them with healthy controls. METHODS: This is a single-center, pilot study comparing pregnancies with eoPE (n=10) and loPE (n=8), each with separate healthy controls (n=10; n=8). Serum miRNA (miR-) expression levels of selected PE-associated miRNAs (miR-210-3p, miR-518b) were quantified by real-time PCR. RESULTS: Similar expression profiles (delta CT ± standard deviation) were observed between loPE vs. controls: miR-210-3p (8.

Progress and Practice of Insulin Resistance in Pregnancy: From Pathogenesis to Management.

Evidence suggests that the degree of insulin resistance in pregnancy (IRiP) seems to affect pregnancy outcomes regardless of the presence of gestational diabetes. A physiological moderate increase in IRiP is a protective factor for fetal development, but excessive enhancement can lead to maternal metabolic disorders and placental dysfunction, increasing the risk of gestational diabetes and adverse pregnancy outcomes, as well as having adverse effects on the long-term health of the offspring. This review briefly explores the key factors influencing IRiP, including pregnancy-specific and non-pre

Analysis of urinary kidney-specific cell-free DNA in preeclampsia.

AIM: To assess the feasibility of a novel methodology for quantifying kidney-specific cell-free DNA (ks-cfDNA) and explore whether levels differed between individuals with preeclampsia (PE) and normotensive pregnant controls (NP). BACKGROUND: Cell-free DNA (cfDNA) is released during cell death and circulates in bodily fluids, including urine. cfDNA quantities reflect the degree of organ injury. As PE is associated with hypertension and kidney injury, we hypothesised that urinary ks-cfDNA would be elevated in PE. METHODS: In this single-centre prospective case-control study urine samples were
