Impact of country income status on the relationship between interpregnancy interval and maternal complications of pregnancy: a systematic review and meta-analysis protocol.

BACKGROUND: Both short and long interpregnancy intervals (IPIs) have been associated with adverse maternal outcomes, including uterine rupture, gestational diabetes mellitus (GDM), gestational hypertension and postpartum haemorrhage (PPH). However, the strength and consistency of these relationships remain uncertain, and there is limited information on how these associations differ according to country income status. This review is designed to examine the impact of the income status of a country on the relationship between IPI and maternal complications of pregnancy. METHODS AND ANALYSIS: CIN

Prediction of Adverse Maternal Outcome In Women With Preeclampsia By Full PIERS Model.

Preeclampsia is a pregnancy-specific disorder and one of the leading causes of maternal and perinatal morbidity and mortality in Bangladesh. Identification of women at increased risk of developing severe complications from preeclampsia is important to manage timely in appropriate settings. A fullPIERS model is a new tool that could allow real-time maternal risk stratification in preeclampsia to aid caregivers and also guide decisions around triage, transport and treatment. This prospective cohort study was conducted in the Department of Fetomaternal Medicine, Bangladesh Medical University (BMU

Chronic Hypertension and Cardiovascular Disease After Preeclampsia: A Danish Nationwide Register-Based Cohort Study Identifying Women at High Risk.

AIMS: Preeclampsia is associated with future cardiovascular disease (CVD), yet systematic approaches to identify those requiring intensified prevention are lacking. We aimed to characterise these women and identify subgroups at particularly high risk of later chronic hypertension or CVD. METHODS: Using Danish nationwide registries, we followed primiparous women with preeclampsia who gave birth during 2004-2018 until December 2022. Using Poisson regression models we estimated incidence rate ratios (IRR) of later chronic hypertension or CVD. RESULTS: Of 17,434 women with preeclampsia, 2,372 de

The inverted pyramid of care in modern-day obstetrics.

Traditional antenatal care consists of an accelerating frequency of visits with advancing gestational age, as the bulk of pregnancy complications occur in late pregnancy. The 'inverted pyramid of care' concept describes a focus on structured risk assessment in early pregnancy, to facilitate prevention and enhanced surveillance of those at increased risk of maternal, intrinsic fetal, and uteroplacental problems.Since publication of this concept in 2011, it has become clear that early pregnancy screening may be contingent, with a sub-population identified for re-screening in mid- or late pregnanc

Prenatal factors shaping adolescent cardiovascular function and cardiometabolic risk trajectories.

Cardiovascular disease remains the leading cause of mortality worldwide, with early risk markers, including elevated blood pressure, arterial stiffness, and adverse lipid profiles, often emerging during adolescence. These phenotypes are clinically important, as adolescent cardiovascular risk strongly predicts adult subclinical atherosclerosis and future cardiovascular events. Increasing evidence from developmental origins of health and disease (DOHaD) research suggests that cardiovascular outcomes in adolescence are associated, in part, with prenatal exposures. This review synthesizes current

Artificial Intelligence and Machine Learning for predicting Major Obstetric Emergencies: Current Evidence, Clinical Translation, and Future Directions.

BACKGROUND: Obstetric emergencies, including postpartum hemorrhage, preeclampsia, and preterm birth, remain major contributors to preventable maternal and neonatal morbidity and mortality worldwide. Conventional risk assessment approaches often rely on predefined clinical variables and may insufficiently capture complex nonlinear relationships among maternal characteristics, biomarkers, imaging findings, and electronic health record data. PURPOSE: This narrative review aimed to synthesize current evidence on the application of artificial intelligence and machine learning for prediction and ri

Hypertensive disorders of pregnancy may increase the risk of neurodevelopmental delay in the offspring: a systematic review with meta-analysis.

Hypertensive disorders of pregnancy (HDP) are the most frequent complication of pregnancy, posing a burden on maternal and offspring health. Evidence about the potential effect of HDP on the infants' neurodevelopment is still inconsistent. We summarize the evidence about the effect of HDP on neurodevelopmental delay in the offspring. We conducted a systematic review with meta-analysis following the PRISMA (Preferred Reporting Items for Systematic reviews with Meta-Analysis) guidelines. The search was conducted in PubMed, Web of Science, and EBSCO databases. 21 publications were selected (20 co

Hypertensive Pregnancy Disorders: From Mechanisms to Management.

Hypertensive disorders of pregnancy (HDPs) are a leading cause of maternal and perinatal morbidity and mortality globally, affecting up to 10% of pregnancies. As rates of obesity, chronic hypertension, and advanced maternal age continue to rise, the burden of HDPs is expected to escalate. This review provides a comprehensive overview of HDPs, encompassing updated classification systems, risk factors, and diagnostic approaches, including emerging biomarkers and predictive imaging tools. We highlight the complex pathophysiology involving impaired placentation, angiogenic imbalance, immune dysreg

Role of PD-L1 in the Pathogenesis of Pre-Eclampsia and Its Association with Adverse Fetal Outcomes.

OBJECTIVE: Preeclampsia is a pregnancy-specific disorder characterized by impaired maternal-fetal immune tolerance. The maternal immune system plays a crucial role in maintaining pregnancy, and its dysfunction is believed to contribute to preeclampsia. Immune checkpoint molecules such as programmed cell death protein 1 (PD-1) and its ligand, programmed death ligand 1 (PD-L1), may play a key role in this process. This study evaluated PD-L1 expression in the placentae of patients with pre-eclampsia (PE) and eclampsia (EC). We also compared PD-L1 expression with histomorphological features and fe

Barriers and facilitators of cardiovascular disease prevention services for women with prior gestational diabetes or hypertensive disorders of pregnancy: a scoping review.

AIMS: Although cardiovascular disease (CVD) screening and preventive care in the early postpartum are recommended for women with prior gestational diabetes (GDM) and hypertensive disorders of pregnancy (HDP), certain barriers limit access to such services. We conducted a scoping review to summarize evidence on the barriers and facilitators of CVD prevention services among women with prior GDM or HDP. METHODS AND RESULTS: A comprehensive search strategy was used to search articles published in three databases (Ovid Medline, CINAHL, Embase). Studies published in English or French that investiga

Female- and age-specific reference values for blood pressure and its relationship with cardiac aberrations after pre-eclampsia.

AIMS: Cardiovascular risk increases progressively increases from lower blood pressure (BP) values in females than males, questioning current universal thresholds for hypertension. In this study, we established female- and age-specific thresholds for hypertension and assessed its association with subclinical cardiac aberrations in a high-risk female group of women after pre-eclampsia. METHODS: A standardized cross-sectional cardiovascular assessment was performed among former pregnant women, including 30-minutes BP measurement and transthoracic echocardiography to assess cardiac function and r
