BACKGROUND: Proteinuria in pregnancy is often attributed to preeclampsia, but primary glomerular diseases such as focal segmental glomerulosclerosis can also present during pregnancy, complicating diagnosis and management.
CASE PRESENTATION: A 29-year-old gravida 2, para 1 African woman presented at 25 weeks of gestation with generalized edema and turbid urine. Initial investigations revealed nephrotic-range proteinuria (7.2 g/24 hours), hematuria with red blood cell casts, and deteriorating renal function, with serum creatinine rising from 1.8 to 5 mg/dL within 3 days. Due to worsening renal

Preeclampsia is a multisystem vascular disorder characterized by new-onset hypertension and proteinuria after 20 weeks of gestation. Preeclampsia affects 2% to 5% of pregnancies and is a major contributor to maternal and perinatal morbidity and mortality resulting from aberrant vascular and immunologic responses. While the exact mechanisms of why preeclampsia arises remain unclear, our current understanding suggests that inadequate vascular adaptations are a contributing factor to the disease. In this review, we focus on how immune system changes affect the onset and pathogenesis of preeclamps

This study aimed to determine the association between proteinuria severity and maternal/neonatal outcomes among preeclamptic women with hypertension with proteinuria. We retrospectively assessed 721 women at 11 institutions. All participants had singleton pregnancies and delivered at ≥22 gestational weeks. They were diagnosed with preeclampsia (excluding superimposed preeclampsia) after 20 gestational weeks and had results from a spot urine protein/creatinine ratio test at delivery. The severe proteinuria significantly increased the frequency of preterm delivery in the both women of severe and

BACKGROUND: Hypertensive disorders of pregnancy (HDP) are a leading cause of maternal mortality, particularly in low-resource settings. While lactate dehydrogenase (LDH) has been proposed as a prognostic biomarker, its clinical utility is limited by a lack of consensus on a standardized threshold for interpretation. This study aimed to evaluate the predictive performance of antenatal LDH for adverse maternal outcomes and establish a clinically applicable cutoff for risk stratification.
METHODS: This prospective cohort study included 336 pregnant women with HDP admitted to a national tertiary

BACKGROUND: Atypical presentation of preeclampsia and eclampsia refers to cases where symptoms may not follow the typical signs of high blood pressure and proteinuria rendering them difficult to diagnose leading to potential delays in treatment. Hence our aim was to study the atypical presentations of preeclampsia and eclampsia and their possible outcomes. METHODS: The study was a descriptive cross-sectional hospital-based study conducted from November 2022 to April 2023 at Al Saudi Hospital in Khartoum, Sudan. The study focused on pregnant women with atypical presentations of preeclampsia and

BACKGROUND Isolated fetal and neonatal ascites is uncommon and poses substantial diagnostic and management challenges due to its diverse etiologies, which may include genitourinary or gastrointestinal malformations, infections, cardiac anomalies, and genetic disorders. Mirror syndrome, or Ballantyne syndrome, is a rare maternal complication typically associated with hydrops fetalis and characterized by maternal edema that "mirrors" the fetus and placenta, often mimicking preeclampsia. Its occurrence in conjunction with isolated fetal ascites is exceptionally rare, making recognition and manage

Background: Preeclampsia (PE) is a multicomplex disorder occurring during pregnancy, characterized by the onset of hypertension and proteinuria, or hypertension accompanied by organ dysfunction (such as impaired liver function, renal insufficiency, pulmonary edema, or cerebral and visual impairment), with or without proteinuria in the latter half of pregnancy or postpartum. It impacts around 5% of all pregnancies, resulting in considerable fetal and maternal mortality and morbidity. A properly regulated inflammatory response is crucial for achieving a successful pregnancy; nevertheless, an exc

The triad of blood pressure measurement, urinalysis, and edema assessment has constituted the cornerstone of prenatal care for over eight decades and has been consistently recorded at each examination in Japan's Maternal and Child Health Handbook. Historically, these examinations were prioritized to facilitate the early diagnosis and treatment of pregnancy toxemia-now termed hypertensive disorders of pregnancy-which once represented a leading cause of maternal mortality. The triad has undeniably played a pivotal role in the early diagnosis, prediction, and treatment of hypertensive disorders o

Preeclampsia accounts for the majority of nephrotic syndrome in pregnancy. However, primary renal diseases, although not as common, can also be the cause. The authors report a case of a 33-year-old pregnant woman at 22 weeks of gestation who presented with significant proteinuria, hypoalbuminemia, hyperlipidemia and discrete pitting edema. After the initial workup, a kidney biopsy was performed to diagnose minimal change disease (MCD), which revealed extensive podocyte foot process effacement. Treatment with IV methylprednisolone followed by oral prednisone led to remission, but a relapse occu

Systemic lupus erythematosus (SLE) is a chronic autoimmune disease that primarily affects women of reproductive age. Most patients are diagnosed prior to conception; however, new-onset SLE during pregnancy is rare and can mimic common obstetric complications, delaying diagnosis and treatment. Such overlap increases the risk of maternal morbidity and adverse pregnancy outcomes (APOs). A primigravida adolescent in her third trimester presented with severe preeclampsia, HELLP syndrome, and nephrotic-range proteinuria. Postpartum, she developed hemorrhage requiring a modified B-Lynch suture in the

INTRODUCTION: There has been a lack of novel medication classes approved for reducing blood pressure (BP) in hypertensive patients. Endothelins, powerful vasoconstricting peptides, have been at the forefront of experimental hypertension research since they were discovered in 1988. The recent PRECISION trial demonstrated the efficacy of aprocitentan, a novel endothelin receptor antagonist, in lowering blood pressure in patients with resistant hypertension (RH). This trial was the driving force behind the approval of aprocitentan in 2024 for the treatment of resistant hypertension.

AIMS AND OBJECTIVES: To compare pregnancy outcomes among pre-eclamptic women with nephrotic and non-nephrotic range proteinuria.
METHODS: This prospective cohort study was conducted from May 2021 to December 2022 at a tertiary care university hospital in South India. We included a total of 278 women diagnosed with preeclampsia, 56 with and 222 without nephrotic range proteinuria in a 1:4 ratio, respectively. The relative risks (RR) of core maternal and perinatal adverse outcomes for preeclampsia among the two groups were compared. Using logistic regression analysis, we calculated the odds rat

INTRODUCTION: Preeclampsia, a leading cause of morbidity during pregnancy, is associated with glomerular endotheliosis, fibrin deposition, and thrombotic microangiopathy and is characterized by edema, proteinuria, and acute kidney injury. Preeclampsia has been described on a background of glomerular disease membranous nephropathy, IgA nephropathy, and focal segmental glomerulosclerosis, but biopsy studies have also described the de novo diagnosis of glomerulopathy as thrombotic microangiopathy, endotheliosis or collapsing glomerulopathy in the setting of preeclampsia.

Due to the shortage of pediatric hospital beds in general and due to the large annual burden of hospital admissions for common respiratory tract infections, such as influenza or RSV [respiratory syncytial virus] in particular, it can be expected that hospitalization of children with suspected or confirmed SARS-CoV‑2 infections will face hospitals with an even more tense situation than usual in the winter 2020/2021. This tenuous shortage may touch various aspects but in the first place it will affect isolation and cohorting. In addition, this shortage will not only apply to acutely ill children

Preeclampsia (PE) is a severe gestational disorder characterized by hypertension and proteinuria, with a subset of cases exhibiting an immune-driven phenotype marked by placental overexpression of proinflammatory cytokines and chronic inflammatory damage, profoundly impacting fetal development. To elucidate the pathophysiology of this PE subtype, we established an inflammation-driven PE mouse model via lipopolysaccharide (LPS) intraperitoneal injection, systematically evaluating histopathological changes in maternal heart, liver, lung, kidney, and placenta, and integrating transcriptomic profi

Gestational diabetes mellitus (GDM) is a known risk factor for gestational hypertension which further progress toward conditions like proteinuria, dyslipidemia, thrombocytopenia, pulmonary edema leading to Preeclampsia (PE). Pregnancy can be a challenging time for many women, especially those diagnosed with GDM and PE. Thus, the current prospective study investigates the association of OGTT glucose levels with systolic and diastolic blood pressure and lipid profile parameters in pregnant women diagnosed with GDM and PE. A total of 140 pregnant women were stratified into GDM (n = 50), PE (n = 4

BACKGROUND: Most patients with signs or symptoms (s/s) of suspected preeclampsia are not diagnosed with preeclampsia. We sought to determine and compare the prevalence of s/s, pregnancy outcomes, and costs between patients with and without diagnosed preeclampsia.
METHODS: This retrospective cohort study analyzed a large insurance research database. Pregnancies with s/s of preeclampsia versus a confirmed preeclampsia diagnosis were identified using International Classification of Diseases codes. S/s include hypertension, proteinuria, headache, visual symptoms, edema, abdominal pain, and nausea

Hemolysis, elevated liver enzyme levels, and low platelet count (HELLP) syndrome is one of the most severe complications of hypertensive disorders of pregnancy. HELLP syndrome occurring before 22 gestational weeks (GWs) is extremely rare, and patients prevalently exhibit underlying maternal diseases or fetal abnormalities. Here, we report the case of a pregnant woman who had HELLP syndrome at 20 GWs without any obvious underlying maternal diseases or fetal abnormalities. A 38-year-old pregnant woman was referred to Kobe University Hospital from another hospital at 19 + 5/7 GWs for hypertension

OBJECTIVES: Mirror syndrome (MS) is a condition characterized by the presence of maternal, fetal, and placental edema and is reversible through delivery or pregnancy termination. As fetal hydrops itself may be amenable to treatment, we sought to determine outcomes for MS primarily managed by fetal therapy through a narrative review of the literature and cases managed at our fetal center.
STUDY DESIGN: PubMed, Embase, Web of Science, Scopus, and Google Scholar databases were searched through January 2024 using key words: mirror syndrome, Ballantyne's syndrome, fetal hydrops, maternal hydrops,
