AA and Pregnancy.

AA is a crucial fatty acid in pregnancy, especially for the growth and development of the fetus and its central nervous system (CNS). As already discussed in the previous chapters, AA regulates inflammation, immune response, and cell signaling. AA has a modulatory action on gene expression, serves as a mechanotransducer, and regulates cell membrane fluidity. By regulating cell membrane fluidity, AA modulates the expression and binding of several receptors located on the cell membrane. Eicosanoids derived from AA have both beneficial and harmful effects on pregnancy. For instance, (i) AA metabo

AA in Hypertension and Preeclampsia.

Human essential hypertension is driven by a network of interacting nutritional, metabolic, inflammatory, and endothelial mechanisms in which AA and other PUFAs play a critical role. Beyond excess sodium intake, population data supports the role of inadequate calcium, potassium, and magnesium intake and low antioxidant vitamin status in shaping blood pressure regulation. These nutrients influence vascular smooth muscle tone and act as cofactors that support Δ6 and Δ5 desaturase activities, thereby governing tissue availability of AA, EPA, and DHA and downstream formation of vasodilator and anti

Assisted Reproductive Technology as Predictor for Preeclampsia: A Systematic Review of Current Clinical Prediction Models.

Assisted Reproductive Technology (ART) increases the risk of preeclampsia (PE), yet no specific prediction model tailored to the ART population exists. This review evaluates prediction models incorporating ART as a predictor, focusing on risk of bias, performance, generalizability and clinical applicability. A systematic MEDLINE search up to July 2023 identified models predicting PE that included ART as a predictor. Exclusion comprised studies on outcomes after PE onset, univariable models and validation studies of previously validated models. Risk of bias, applicability and generalizability w

Early Pregnancy Exposure to Per- and Polyfluoroalkyl Substances and Hypertensive Disorders of Pregnancy: A Cohort Study in China.

Maternal exposure to per- and polyfluoroalkyl substances (PFAS) has shown inconsistent associations with hypertensive disorders of pregnancy (HDP). This prospective cohort study examined the associations between early pregnancy serum PFAS concentrations and HDP risk, evaluating potential effect modification by fetal sex among 2,984 pregnant women in Henan Province, China (2018-2021). Sixteen PFAS were measured in early pregnancy serum samples, with nine retained for analysis based on detection rates ≥70%. Multivariable logistic regression models estimated odds ratios (ORs) and 95% confidence i

Early preeclampsia screening and prevention: An update for general practice.

BACKGROUND: Preeclampsia is a disorder of pregnancy that causes significant adverse perinatal outcomes. Risk factor-based screening performs poorly when compared with algorithm-based approaches. The role of general practitioners in early screening for preeclampsia is crucial in predicting and preventing the condition. OBJECTIVE: This article discusses the different screening approaches and interventions available for prevention of preeclampsia, making the case for a screen-and-treat model based on individual risk calculation for all pregnant women. DISCUSSION: The Fetal Medicine Foundation (

Prenatal Cannabinoid Exposure Induces Sex-Specific Alterations in Fetal Growth and Placental Lipid Metabolism Gene Expression.

BACKGROUND: Prenatal cannabis use is becoming increasingly more commonplace. However, cannabis exposure is linked to adverse pregnancy outcomes, including gestational hypertension, preeclampsia, and preterm birth. The aim of this study was to determine the morphological and molecular effects of prenatal cannabinoid exposure on the placenta. METHODS: Pregnant Sprague-Dawley rats were exposed daily to vaporized THC (100 mg/mL) starting at gestational day (GD)5 until GD19, when dams were sacrificed and fetuses and placentas collected. Fetuses were genotyped for genetic sex, and transcriptomic an

Elevated lipoprotein(a) and adverse pregnancy outcomes.

Lipoprotein(a) [Lp(a)] is gaining attention as a potential marker for adverse pregnancy outcomes, in addition to its well-established role in cardiovascular disease. This review explores the evolving understanding of Lp(a) in pregnancy, focusing on its links to hypertensive disorders of pregnancy, gestational diabetes mellitus, fetal growth restriction, and preterm birth, while also addressing diagnostic and management challenges. During pregnancy, Lp(a) levels rise substantially, nearly doubling between weeks 10 and 35. Although this increase is universal, its clinical implications are unclea

Glycosylated Fibronectin Compared With sFlt-1/PlGF Ratio for Preeclampsia Assessment.

BACKGROUND: The ratio of sFlt-1 (soluble fms-like tyrosine kinase 1) to PlGF (placental growth factor) is an established biomarker for preeclampsia diagnostic triage. GlyFn (glycosylated fibronectin) measured by point-of-care testing is an alternative rapid assessment from finger-stick blood samples. This study was designed to test the hypothesis that the sensitivity of GlyFn is noninferior to that of the sFlt-1/PlGF ratio in women with suspected preeclampsia. METHODS: This prospective observational study evaluated the diagnostic accuracy of both tests for preeclampsia in high-risk participan

Hypertensive disorders of pregnancy: epidemiology, pathophysiology, therapeutic strategies, and maternal-offspring outcomes.

Hypertensive disorders of pregnancy (HDP) encompass chronic hypertension, gestational hypertension, preeclampsia, and preeclampsia superimposed on chronic hypertension, affecting about 5% to 10% of pregnancies worldwide and remaining a major cause of maternal and perinatal morbidity and mortality. Their burden is increasing with rising obesity, advanced maternal age, evolving diagnostic criteria, and marked socioeconomic and racial-ethnic disparities. This review summarises current evidence on the epidemiology, pathophysiology, biomarkers, clinical management, and long-term outcomes of HDP, hi

Perinatal Factors Associated With Standardized Blood Pressure Percentiles for Children Aged 6 to 12 Years: The BOSHI Study.

Elevated childhood blood pressure (BP) is associated with adulthood cardiovascular mortality. However, evidence of an association between maternal and perinatal factors and standardized childhood BP percentiles in Japan remains limited. In this study, we examined this association by using data from the BOSHI study, which included children's home BP measurements. Among 175 mother-offspring pairs, the mean maternal age and children's birth weight were 32.4 years and 3031 g, respectively; the median age of the children at follow-up was 7 years; and hypertensive disorders of pregnancy occurred in

Blockade of gastrin-releasing peptide receptor in the PVN alleviates maternal vascular and placental dysfunction in preeclamptic rat.

Preeclampsia (PE) is characterized by placental ischemia-induced systemic inflammation, oxidative stress, and maternal vascular dysfunction. We previously reported that upregulated gastrin-releasing peptide (GRP) and its receptor (GRPR) within the hypothalamic paraventricular nucleus (PVN) of PE rats contribute to the progression of PE. However, whether such central upregulation contributes to the peripheral organs damage in PE remains unknown. This study investigated the protective effects of the GRPR antagonist ICI-216140, administered via PVN microinjection, on mesenteric arteries and place
