Murine Typhus Mimicking Preeclampsia with Severe Features.

Background Murine typhus, a flea-borne rickettsiosis endemic to southern Texas, can cause headache, thrombocytopenia, and transaminitis that overlap with the criteria for preeclampsia with severe features. Case A 22-year-old gravida 5 with gestational hypertension presented at 32 5/7 weeks of gestation with 5 days of fever and headache. Worsening thrombocytopenia, transaminitis, and refractory headache were initially attributed to preeclampsia with severe features, and magnesium sulfate was administered. Persistent fever prompted empiric doxycycline for suspected murine typhus; defervescence o

[Atypical hemolytic uremic syndrome following a late miscarriage: a case report].

Atypical hemolytic uremic syndrome (aHUS) is a rare thrombotic microangiopathy (TMA) characterized by hemolysis, thrombocytopenia, and renal impairment, most commonly driven by complement system dysregulation. Differentiating aHUS from other conditions, such as HELLP syndrome and thrombotic thrombocytopenic purpura (TTP), is critical, particularly in pregnancy.

High ambient temperatures and hypertensive disorders of pregnancy: a systematic review and meta-analysis protocol.

BACKGROUND: Exposure to high ambient temperatures, commonly referred to as heat, has been consistently associated with adverse birth outcomes, particularly preterm birth. There is less consensus with regard to links with adverse maternal outcomes, such as hypertensive disorders of pregnancy (HDP). HDP are a leading cause of maternal mortality and morbidity, whose aetiology remains poorly understood. In the context of global warming driven by climate change, clarifying this potential association is of importance for global health. OBJECTIVE: The proposed systematic review and meta-analysis aim

Clinical performance of obstetric antiphospholipid syndrome classification criteria: an ambispective cohort study.

OBJECTIVES: To evaluate the clinical performance of the 2006 Sydney and 2023 ACR/EULAR classification criteria for obstetric antiphospholipid syndrome (OAPS), compare subsequent pregnancy outcomes across classifications and develop a supplementary classification method to further identify clinically relevant non-criteria OAPS (NC-OAPS) cases missed by the Sydney-derived NC-OAPS classification. METHODS: This single-center ambispective cohort included 1741 women of reproductive age enrolled from January 2020 to October 2024. Expert consensus was used to classify participants into OAPS, NC-OAPS

Race and ethnicity-specific differences in incidence of pre-eclampsia with severe features: Assessment per socioeconomic status.

In this cross-sectional study of 18,969,287 hospital deliveries identified in Healthcare Cost and Utilization Project's National Inpatient Sample from 2016 to 2022, Black pregnant people had higher incidence rates of developing pre-eclampsia with severe features compared to White pregnant people across all the socioeconomic strata. The effect sizes for Black pregnant people comparing to White pregnant people were similar and consistent across census-level household income and insurance status. These data suggest that the development of pre-eclampsia with severe features may be driven by a comb

Postpartum macrophage activation syndrome with temporal dissociation of cytopenias in a patient with Kikuchi-Fujimoto disease and undifferentiated connective tissue disease: a case report.

BACKGROUND: Macrophage activation syndrome (MAS) is a life-threatening hyperinflammatory disorder; postpartum MAS is extremely rare and often misdiagnosed due to overlap with sepsis and HELLP syndrome. Kikuchi-Fujimoto disease (KFD) and undifferentiated connective tissue disease (UCTD) may increase susceptibility, but reports are scarce. This case highlights the diagnostic challenges of postpartum MAS in the setting of overlapping inflammatory conditions. CASE DESCRIPTION: The patient was a 35-year-old primiparous woman with KFD and UCTD who underwent cesarean section for preeclampsia and cer

Management of pregnancy in patients with Graves' disease secondary to alemtuzumab: a case series and literature review.

INTRODUCTION: Alemtuzumab treatment for multiple sclerosis frequently induces Graves' disease (GD), characterized by common coexistence of stimulating and blocking anti-thyrotropin receptor autoantibodies (TRAb), causing fluctuations of thyroid function. Alemtuzumab-induced TRAb-positive hypothyroidism is also common. These conditions are challenging to manage during pregnancy, considering the imperative necessity to maintain euthyroidism to minimize risks for both pregnancy and fetal/neonatal outcomes. METHODS: We reviewed the clinical course and therapeutic management of four pregnant women

A Rare Event Causing Blindness in an Unrecognised Pregnancy: A Case Report.

Purtscher-like retinopathy (PLR) is a rare, non-traumatic microembolic angiopathy characteristically associated with systemic endothelial conditions, including pre-eclampsia. While PLR typically exhibits a self-limiting course with a highly favourable visual prognosis, severe cases result in permanent, treatment-refractory blindness. We present a catastrophic, atypical case of bilateral PLR in an unrecognised pregnancy without any antenatal care. A 31-year-old primigravida without prior antenatal screening presented with a three-day history of sudden, bilateral central blurry vision and was ne

Maternal Delivery Outcomes Associated With Acute Sickle Cell Crisis: A National Inpatient Analysis.

BACKGROUND: Pregnancies complicated by sickle cell disease (SCD) carry substantial maternal and perinatal risk, but the specific association between acute sickle cell crisis (SCC) and adverse obstetric outcomes at delivery remains incompletely characterized. This study evaluated whether acute sickle cell crisis was associated with hypertensive disorders of pregnancy and cesarean delivery among delivery hospitalizations involving SCD. METHODS: We performed a retrospective cross-sectional analysis of the 2021 Healthcare Cost and Utilization Project National Inpatient Sample (HCUP-NIS). Delivery

A Rapidly Progressive Case of Crimean-Congo Hemorrhagic Fever in Pregnancy Presenting with HELLP-Like Features and Resulting in Maternal and Fetal Death.

Crimean-Congo hemorrhagic fever (CCHF) is a life-threatening viral infection that may mimic hemolysis, elevated liver enzymes, and low platelet count (HELLP) syndrome during pregnancy. We describe a 30-year-old woman at 23 weeks' gestation who presented with fever, myalgia, thrombocytopenia, and severe transaminase elevation. Initial suspicion of HELLP syndrome was reconsidered in the presence of high fever, rural exposure, and rapidly progressive coagulopathy. Crimean-Congo hemorrhagic fever was confirmed by polymerase chain reaction. Despite intensive supportive therapy and plasmapheresis, f

Exudative Retinal Detachment in Preeclampsia. Case Reports.

Preeclampsia, eclampsia and HELLP syndrome are serious pregnancy-related complications associated with significant morbidity and mortality. These pathological conditions are related to placental dysfunction, systemic endothelial dysfunction and various clinical and biochemical abnormalities. Exudative retinal detachment as a rare but clinically significant ophthalmologic complication of preeclampsia occurs in 0.2-2% of cases, more frequently affecting primigravidas and often presenting bilaterally. This article presents two case reports of patients with preeclampsia complicated by exudative re

Circulating tumor cells in myeloma are a compound biomarker for bone marrow high-risk genomic alterations and tumor load.

Preeclampsia (PE) is a pregnancy-specific complication that typically occurs in the mid to late stages of gestation. Its main clinical features include gestational hypertension, proteinuria, and multiple organ dysfunction. In severe cases, it may progress to eclampsia, HELLP syndrome, and even pose life-threatening risks to both mother and fetus. Recent studies have found that serum uric acid (UA) levels are significantly elevated in patients with PE and closely correlated with disease severity, suggesting that UA may not only serve as a metabolic marker but also play an active role in the pat
