Epidemiological Characterization of Hypertensive Disorders of Pregnancy in Northern Chile: Experience from the Regional Hospital of Arica (2018-2022).

UNLABELLED: Hypertensive disorders of pregnancy (HDP) are among the leading preventable causes of maternal and perinatal morbidity and mortality. In Chile, regional epidemiological data are scarce despite recognized geographic differences in demographics, comorbidities, and access to care. AIM: To determine the prevalence and clinical distribution of HDP among pregnant women attended at the Regional Hospital of Arica between 2018 and 2022. METHODS: We conducted a retrospective, observational, cross-sectional study. All women diagnosed with HDP who delivered at the Juan Noé Crevani Hospital d

A Case of Severe Pre-Eclampsia Complicated by HELLP Syndrome and Extensive Hepatic Infarction.

Extensive hepatic infarction is a rare but potentially fatal complication of HELLP syndrome and is often difficult to recognize because of nonspecific clinical manifestations. A 34-year-old woman developed HELLP syndrome shortly after cesarean delivery for severe pre-eclampsia, presenting with oliguria, marked thrombocytopenia (50 × 109/L), and rapidly rising transaminases (ALT 1077 U/L, AST 1280 U/L). Contrast-enhanced computed tomography (CT) revealed extensive hepatic infarction. Laboratory findings demonstrated microangiopathic hemolysis, severe coagulopathy, and acute kidney injury. Trans

A case of perioperative and intraoperative management of a rare vaginal paraganglioma.

BACKGROUND: We present the case of a 24-year-old woman with a vaginal paraganglioma and review management considerations, noting that only eleven cases have been previously reported in the literature. CASE PRESENTATION: Our patient was initially evaluated during a pregnancy complicated by severe pre-eclampsia and later presented with persistent catecholamine-related symptoms, including headaches, diaphoresis, and tachycardia. Laboratory testing demonstrated markedly elevated normetanephrines, and imaging subsequently revealed a paravaginal mass. Given the clinical presentation, a paragangliom

Calcium supplementation for prevention of pre-eclampsia in high-risk women: study protocol for a randomised triple-blind placebo-controlled trial (CaPE).

BACKGROUND: Pre-eclampsia is a multisystem disorder affecting 2.8% of pregnancies in the UK. It usually presents after 20-week gestation with new-onset high blood pressure and proteinuria. Complications include eclampsia, stroke, and HELLP syndrome for the mother and preterm birth, fetal growth restriction, and stillbirth for the baby. Delivery is the only definitive cure, with antenatal care focusing on early detection and management of complications and optimising timing of delivery. Previous studies have shown calcium supplementation may reduce the risk of pre-eclampsia, but findings are dr

Effects of early vs. late administration of labetalol on maternal cerebral perfusion and fetal growth in severe pre-eclampsia.

BACKGROUND: Severe pre-eclampsia is a major contributor to maternal and perinatal morbidity and mortality worldwide. Antihypertensive therapy is essential, yet the impact of the timing of intravenous labetalol initiation on maternal cerebral hemodynamics and clinical outcomes remains uncertain, particularly in Chinese populations. METHODS: In this retrospective cohort study, we evaluated women with severe pre-eclampsia treated at the Department of Obstetrics and Gynecology, The First Affiliated Hospital of Soochow University, Jiangsu Province, China, between March 2023 and July 2025. A Case Report of Subcapsular Hepatic Hematoma in a Multiparous Pregnant Woman at 21 Weeks of Gestation: A Fatal Complication of HELLP Syndrome.

HELLP syndrome, characterized by hemolysis, elevated liver enzymes, and low platelets, is a severe form of preeclampsia that can result in rare life-threatening rupture of a hepatic hematoma, causing intra-abdominal hemorrhage. This complication has been reported in up to 1.6% of HELLP syndrome cases. Here, we report a patient who developed intra-abdominal hemorrhage due to a ruptured subcapsular hepatic hematoma at 21 weeks of gestation, representing an unusually early presentation of this complication. Despite surgical treatment, the pregnancy was lost, and 5 days after discharge, the patien

Thrombectomy Versus Anticoagulation: A Case of Pregnancy Complicated by an Extensive Pulmonary Embolism.

Pulmonary embolism remains a well-known and life-threatening complication of pregnancy. Due to the variation in presentation and infrequency of this disease, despite serious potential fetal and maternal complications, there is little data available to guide its management in pregnancy. This case highlights the importance of early detection of the condition and the benefit of an interdisciplinary approach to treatment. This case also demonstrates that in the absence of right heart strain, anticoagulation alone may be sufficient management for large pulmonary emboli.

The Unfolding Mystery of Hemophagocytic Lymphohistiocytosis in Pregnancy: A Case Report.

Hemophagocytic lymphohistiocytosis (HLH) is a rare, life-threatening hyperinflammatory syndrome characterized by uncontrolled immune activation and excessive cytokine release. Pregnancy-associated HLH is particularly uncommon and presents a significant diagnostic challenge because its clinical manifestations frequently overlap with obstetric conditions such as sepsis, hemolysis, elevated liver enzymes, and low platelet count (HELLP) syndrome, and acute fatty liver of pregnancy. We report the case of an 18-year-old primigravid woman at 34 weeks of gestation who presented with persistent fever,

Rectus Sheath Hematoma Following Cesarean Section: A Concealed Extrauterine Cause of Postpartum Hemorrhage.

Rectus sheath hematoma (RSH) is a rare but potentially life-threatening extrauterine cause of postpartum hemorrhage, particularly following cesarean delivery in high-risk patients, like those with severe preeclampsia. It usually results from injury to the epigastric vessels or rectus muscle and may be aggravated by preeclampsia-related endothelial dysfunction. Because its presentation can mimic common postoperative or postpartum complications, diagnosis is often delayed. Severe cases that go unnoticed or are missed result in concealed bleeding, shock, organ failure, or death. A 30-year-old pri

Imaging negative subcapsular liver hematoma in parturients with HELLP syndrome: a report of two cases.

INTRODUCTION AND IMPORTANCE: We report two cases of parturients with HELLP syndrome who were diagnosed with subcapsular liver hemorrhage (SLH) during an emergency cesarean section. CASE PRESENTATION: In case 1, a 30-year-old woman who was pregnant at 34+ weeks was admitted because of fetal heart rate deceleration. On the third day of hospitalization, the patient experienced upper abdominal pain. Considering the diagnosis of HELLP syndrome, an emergency cesarean section was performed under general anesthesia. Multiple SLH sites were discovered during laparoscopic exploration, but no interventi

MicroRNA-210 level as a biomarker for pre-eclampsia severity: A systematic review and meta-analysis.

BACKGROUND: Pre-eclampsia is a serious pregnancy-related syndrome lacking accurate prognostic markers. MicroRNA-210, overexpressed in the circulation of women with pre-eclampsia, has recently emerged as a promising biomarker. OBJECTIVES: We conducted a systematic review and meta-analysis to evaluate the relationship between miR-210 levels and pre-eclampsia at different degrees of severity. SEARCH STRATEGY: The study was prospectively registered on PROSPERO (CRD42024604698) and included a comprehensive systematic search of five databases conducted on November 21, 2024. Hypertensive disorders of pregnancy: A review and updates for the emergency physician.

Hypertensive disorders of pregnancy (HDPs) are a spectrum of disease contributing to immediate and lasting morbidity and mortality in pregnant patients and neonates globally. With decreasing access to obstetric care in the United States, an expected increase in patients will seek care in emergency departments. Consequently, emergency physicians will likely assume a greater share of initial care and resuscitation of patients with HDPs. HDPs represent a spectrum of disease including gestational hypertension; chronic hypertension in pregnancy; pre-eclampsia; superimposed pre-eclampsia; Hemolysis,

Diagnostic performance of serum uric acid and proteinuria for classifying pre-eclampsia severity among pregnant women at Banadir Hospital, Mogadishu, Somalia: a comparative cross-sectional study.

BACKGROUND: Pre-eclampsia is a major cause of maternal and perinatal morbidity and mortality, particularly in low-resource settings. While hypertension and proteinuria are central to diagnosis, additional biomarkers may assist in classifying disease severity at presentation. Serum uric acid (SUA) has been linked to endothelial dysfunction and oxidative stress in pre-eclampsia, but evidence from Somalia is limited. OBJECTIVE: To assess the diagnostic performance of serum uric acid in classifying pre-eclampsia severity and to compare its discriminatory ability with semi-quantitative proteinuria

Hypertensive disorders of pregnancy in the United Arab Emirates.

Hypertensive disorders of pregnancy (HDP), including chronic hypertension, gestational hypertension, pre-eclampsia, eclampsia, and HELLP syndrome, are major contributors to adverse maternal and neonatal outcomes. However, epidemiological data from the United Arab Emirates (UAE) remain limited. This study aimed to investigate the prevalence and clinical characteristics of HDP in a large tertiary center, providing evidence from an underrepresented region and a basis for future studies. This six-year retrospective cohort study (2018-2023) was conducted at a tertiary referral center in Dubai, UAE.

Hypertensive retinopathy in pre-eclampsia and its association with disease severity and neonatal outcomes: A retrospective cohort study.

INTRODUCTION: Pre-eclampsia (PE) involves systemic endothelial dysfunction and microvascular injury, yet routine obstetric care lacks noninvasive readouts of maternal microvascular health. We evaluated whether hypertensive retinopathy (HR) detected during pregnancy is associated with maternal disease severity and adverse neonatal outcomes. METHODS: We performed a retrospective cohort study of singleton pregnancies with PE and at least one ophthalmic assessment during pregnancy. Retinal findings were graded as none, mild, moderate, or severe. Primary outcomes were maternal composite adverse ou
