Language-supervised electrocardiogram interpretation with vision transformer.

PURPOSE: Most artificial intelligence models for electrocardiogram (ECG) interpretation rely on task-specific supervised learning, requiring dedicated labeled datasets for each diagnostic task and limiting scalability across diverse cardiac conditions. This study investigated whether vision-language learning from paired ECG recordings and summary reports could enable transferable ECG representations without task-specific supervised training. METHODS: We developed ECG-VLM, a contrastive vision-language model consisting of a Transformer-based image encoder and a text encoder, trained on 800,034

Strengthening an Ethical Framework and Research Practice to Better Link Global Health Research to Health Equity : A Case Study of the SPOT-Impact Consortium.

An ethical framework titled "Research for Health Justice" has been developed that provides global health researchers with guidance on how to design their research to promote health equity and global justice. This study sought to test the framework's guidance by comparing it to the governance and experiences of the "Severe Pre-eclampsia adverse Outcome Triage" (SPOT)-Impact consortium-a transdisciplinary maternal health research consortium. The aim was to identify ways in which the "Research for Health Justice" framework could learn from the consortium's insights and practices and to identify h

Reversible High-Degree Atrioventricular Block Associated With Magnesium Toxicity in Severe Pre-Eclampsia.

We report a 35-year-old woman at 31 weeks and 5 days' gestation with severe pre-eclampsia and acute kidney injury who developed symptomatic hypermagnesemia (4.49 mmol/L) and 2:1 second-degree atrioventricular block after receiving a standard Pritchard magnesium sulfate regimen. Magnesium sulfate was discontinued, intravenous calcium gluconate was administered, and emergency cesarean delivery was performed. The atrioventricular block resolved completely, and the mother recovered well. This case highlights a rare but reversible manifestation of magnesium toxicity and the importance of early reco

Recurrent Episodes of New-Onset Supraventricular Tachycardia in Severe Early-Onset Pre-eclampsia and Fetal Growth Restriction: A Report of a Complex Cardio-Obstetric Case.

Supraventricular tachycardia (SVT) is one of the commonly reported sustained arrhythmias in pregnancy. The rapid progression from early-onset fetal growth restriction (FGR) to severe pre-eclampsia is a well-recognised manifestation of placental dysfunction. The coexistence of both leads to significant maternal and fetal morbidity, adding substantial complexity to management. We present the case of a 27-year-old primigravida of Caucasian origin with an otherwise uncomplicated pregnancy who attended at 28 weeks' gestation solely because of a maternal concern that her baby appeared small. This se

Venoarterial extracorporeal membrane oxygenation for peripartum cardiomyopathy complicated by severe preeclampsia: a case report with 5-year follow-up.

BACKGROUND: Peripartum cardiomyopathy (PPCM) coexisting with severe preeclampsia is uncommon and diagnostically challenging because dyspnea, pulmonary edema, and cardiac dysfunction may be attributed to either condition. Long-term outcomes after rescue mechanical support remain poorly documented. CASE PRESENTATION: A 28-year-old primigravida at 37 + 6 weeks presented with 2 days of worsening orthopnea, dyspnea, and cough. At 15 + 3 weeks, electrocardiography had shown complete left bundle branch block, although left ventricular ejection fraction (LVEF) was 61%. On admission, blood pressure wa

The effects of spatial structure on a host-parasite model with horizontal and vertical transmission.

Different strategies have been developed to refine the prediction of the risk of preterm delivery in asymptomatic patients. Transvaginal sonography has been used for this reason to measure and examine the length and shape of the cervix. In this review, we focus on clinical studies involving transvaginal sonographic assessment of the cervix in asymptomatic women at high risk of preterm delivery and in the general pregnant population. Three ultrasound signs are suggestive of cervical incompetence, namely, dilatation of the internal os, sacculation or prolapse of the membranes into the cervix (wi

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Variations in emergency care for severe pre-eclampsia in Uganda: a national evaluation study.

BACKGROUND: Worldwide, 70% of maternal deaths occur in Sub-Saharan Africa. Approximately 10% are attributable to hypertensive disorders of pregnancy, primarily complications of pre-eclampsia. Timely and effective care improves outcomes, but this is not consistently available, particularly in low-resource settings such as Uganda. OBJECTIVES: We conducted a national evaluation of the provision of prompt and safe care for women with severe pre-eclampsia across all regions of Uganda. We explored the wider health system-related factors, eg supply availability, facilities, and emergency training dr

Predicting complications in hypertensive disorders of pregnancy: external validation of a prognostic model for adverse perinatal outcomes.

BACKGROUND: Prediction models can be used as simple evidence-based tools to identify fetuses at risk of perinatal death. Payne et al developed a prognostic model for perinatal death in women with hypertensive disorders of pregnancy, a leading cause of maternal/fetal morbidity and mortality. OBJECTIVE: This study aimed to externally validate the predictive performance of this model in pregnant women with hypertensive disorders of pregnancy admitted between 26 and 34 weeks of gestation in Ghana. STUDY DESIGN: The perinatal model was applied in the SPOT (Severe Pre-eclampsia adverse Outcome Tri
