Alternative dosing regimen of magnesium sulfate for prevention of eclampsia: An observational prospective study.

OBJECTIVE: To evaluate serum magnesium concentrations in pregnant women with severe pre-eclampsia being treated with a novel magnesium sulfate regimen for seizure prevention. METHODS: This was a prospective, longitudinal study. Pregnant women aged 18 years or older with severe pre-eclampsia and an indication for treatment with magnesium sulfate were included. Participants received a magnesium sulfate loading dose of 4.5 g over 20 min, then a maintenance dose of 1.5 g/h (the standard regimen at the study center). Serum magnesium concentrations were measured at admission, 30 min, 1, 2, 4, and 6

Subcapsular hepatic haematoma in a postpartum woman with severe pre-eclampsia.

Subcapsular hepatic haematoma (SHH) is an uncommon but potentially life-threatening complication of hypertensive disorders of pregnancy, particularly Hemolysis, elevated liver enzymes, and low platelets (HELLP) syndrome and severe pre-eclampsia. We report the case of a woman in her 30s who developed a large SHH without rupture in the postpartum period following severe pre-eclampsia. Early imaging and conservative management strategy led to favourable maternal and neonatal outcomes. This case highlights the importance of maintaining a high clinical suspicion, timely imaging and multidisciplinar

ZBTB16::RARA Translocated Acute Promyelocytic Leukemia with Erythrophagocytosis and Rod-Like Crystalline Inclusions.

BACKGROUND: Diabetic retinopathy and diabetic nephropathy are major microvascular complications of type 2 diabetes mellitus and may progress silently before clinical detection. Plasma glycated CD59 has been proposed as a marker linked to complement-mediated endothelial injury. This study assessed plasma glycated CD59 levels in type 2 diabetic patients with retinopathy, nephropathy, and combined microvascular complications. METHODS: This cross-sectional observational study included 420 patients with type 2 diabetes mellitus. Participants were divided into four groups: diabetes without microvas

Evaluation of ProBNP and Troponin I in Cases of Placenta Accreta Spectrum Disorders and Placenta Praevia.

PURPOSE: Pulmonary Hypertension (PH) and Diabetes mellitus (DM) have been extensively studied independently; data on their co-occurrence are limited. This study aims to analyze trends in mortality with PH and DM recorded as contributing causes of death (PH-DM co-mortality) in the U.S from 1999 to 2024. METHOD: The CDC Wonder database was used to examine mortality trends for deaths with PH (I27.0, I27.2, I27.8, I27.9) and DM (E10-E14) recorded as contributing causes of death among adults (≥ 25 years); this age cutoff is consistent with prior CDC WONDER mortality analyses [10]. Data were strati

Management of Wernicke's encephalopathy in a pregnant woman at 27 weeks gestation complicated by pre-eclampsia: A case report.

Wernicke's Encephalopathy (WE) is a rare but severe condition primarily caused by thiamine deficiency, often seen in pregnant women who experience severe vomiting, such as in hyperemesis gravidarum. This case report details a 38-year-old woman at 27 weeks of gestation who developed altered consciousness, cerebellar ataxia, and hyperlactatemia following persistent vomiting. Brain MRI demonstrated characteristic bilateral abnormalities consistent with WE. Early recognition and prompt intravenous thiamine administration led to significant clinical improvement. This case emphasizes the importance

The conundrum of thrombotic microangiopathy in pregnancy.

Hypertensive disorders of pregnancy (HDP) including haemolysis, elevated liver enzymes and low platelet count (HELLP) and pre-eclampsia are one of the leading causes of maternal mortality. There is significant overlap between the acute clinical presentation of such HDP and other medical conditions with thrombotic microangiopathy such as acute fatty liver of pregnancy, catastrophic antiphospholipid antibody syndrome, acute systemic lupus erythematosus, atypical haemolytic uraemic syndrome and thrombocytopenic thrombotic purpura. This can make timely clinical diagnosis and treatment challenging.

Attenuated heterogeneity of hippocampal neuron subsets in response to novelty induced by amyloid-β.

Alzheimer's disease (AD) patients exhibited episodic memory impairments including location-object recognition in a spatial environment, which was also presented in animal models with amyloid-β (Aβ) accumulation. A potential cellular mechanism was the unstable representation of spatial information and lack of discrimination ability of novel stimulus in the hippocampal place cells. However, how the firing characteristics of different hippocampal subsets responding to diverse spatial information were interrupted by Aβ accumulation remains unclear. In this study, we observed impaired novel object-

Randomized Double-Blinded Comparison of Intermittent Boluses Phenylephrine and Norepinephrine for the Treatment of Postspinal Hypotension in Patients with Severe Pre-Eclampsia During Cesarean Section.

BACKGROUND: Norepinephrine has fewer negative effects on heart rate (HR) and cardiac output (CO) for treating postspinal hypotension (PSH) compared with phenylephrine during cesarean section. However, it remains unclear whether fetuses from patients with severe pre-eclampsia could benefit from the superiority of CO. The objective of this study was to compare the safety and efficacy of intermittent intravenous boluses of phenylephrine and norepinephrine used in equipotent doses for treating postspinal hypotension in patients with severe pre-eclampsia during cesarean section. METHODS: A total o

An Unusual Case of Severe Pre-eclampsia and Pulmonary Edema Following Single Fetal Demise in an In Vitro Fertilization (IVF)-Conceived Twin Gestation: A Twisted Tale of Twins.

There has been a notable rise in instances of multiple-fetus pregnancies over the last decade, attributed to the widespread adoption of assisted reproductive technologies. Moreover, these pregnancies have been associated with the use of drugs to induce ovulation. While some cases involve the loss of one twin with minimal consequences for the surviving twin, the demise of a fetus after the first trimester, especially beyond three months into the pregnancy, can significantly impact the health of both the mother and the surviving fetus. Unfavorable outcomes linked to the loss of one twin after th

A mixed methods study on continuity and care coordination based on the obstetric near miss approach.

BACKGROUND: The near-miss approach assumes that mothers facing life-threatening conditions such as severe pre-eclampsia and postpartum haemorrhage share common risk factors. Among these women, those who survive (near-miss cases) can offer insights into the determinants, providing valuable lessons for understanding underlying factors. AIM: To investigate elements of continuity and coordination leading to obstetric near misses. SETTING: A major referral hospital and its referral pathway in Kenya. METHODS: Explanatory sequential mixed-methods design. RESULTS: Near-miss survivors had lower con
