The HELLP syndrome--H for haemolysis, EL for elevated liver enzymes and LP for low platelet count, is a serious complication of hypertension of pregnancy. We report five cases of this syndrome. The haemolysis, which is the consequence of a microangiopathic haemolytic anemia, is not always present: we report four such cases. The recognition of the HELLP syndrome even in the absence of hypertension is important and must be thought of when epigastric or dorsal pain occur in the second half of pregnancy. No liver biopsy was done in this small series.

An increasing number of patients with hemolysis, elevated liver enzymes, and low platelet count (HELLP) have been noted in recent years. We report 11 patients at the Department of Obstetrics (University of Bonn), who were admitted during the past 3 years at between the 24th and 37th week of gestation, showing the typical symptoms of the HELLP syndrome with epigastrical pain and characteristic changes in the blood serum, partly associated with severe EPH gestosis or preeclampsia. No predisposition was found in the medical or pregnancy history. The aggravation of symptoms in combination with ins

Report on a severe Hellp-Syndrome. The Hellp-Syndrome fully develops in the postpartal period accompanied by a breakdown of the kidney and liver functions. Therapeutical success has been achieved only by plasmapheresis.

To clarify the role of disseminated intravascular coagulation (DIC) in women with the hemolysis, elevated liver enzymes, and low platelets (HELLP) syndrome, serial coagulation studies were performed prospectively in 18 patients. A semiquantitative DIC scoring system was used retrospectively to augment the diagnostic confidence of coagulopathy. At the time of admission to the hospital, three patients showed no evidence of DIC, eight had suspected DIC, and seven had manifest DIC. The intravascular coagulation process was progressive in all patients; upon delivery, eight patients proved to have s

A 34-year-old quadripara was hospitalized in the 33rd gestational week due to an acute hypertensive crisis, headache, upper abdominal pain, icterus, and proteinuria. Laboratory testing revealed hemolysis, hepatic dysfunction, and thrombopenia. The values returned toward normal after delivery by cesarean section. Diffuse bleeding in the surgical wound and acute renal failure necessitated two relaparotomies, intensive hemotherapy, and hemodialysis. Mother and child were released in good condition. Anesthesiological and obstetrical aspects of the HELLP syndrome are discussed.

In 1982 Weinstein reported a syndrome marked by Hemolysis, Elevated Liver enzymes, Low Platelets (HELLP Syndrome). Following that report a critical review by MacKenna & Coll. reported that typical signs of the syndrome (observed in 223 cases of preeclampsia) were related to the preeclampsia so denying any nosografic autonomy to it. This report was confirmed by Killan that in 1974 described the classic HELLP syndrome as a complication of hypertensive syndrome in pregnancy. Our case report presents some patterns that may contribute to acquire new data in order to place the HELLP syndrome in its

Recently described by Weinstein, the "HELLP syndrome" is an entity of the pre-toxemic syndrome; its precise physiopathological mechanism, related to a thrombotic micro-angiopathy, is still not clearly established. Therefore, the treatment remains symptomatic, associated with the treatment of the toxemia. Two cases occurring in twin pregnancies are reported.

A case of HELLP (haemolysis, elevated liver enzymes, and low platelet count) and its implications for the country family practitioner- obstetrician is described. The author discusses pre-eclampsia as a multiorgan pathophysiological process, which can present as variants not necessarily in a chronological order of severity. He recommends that it would be reasonable to screen at risk patients with a full blood examination and multiple biochemical analysis from 32 weeks.

Case history of a patient is reported who developed signs of preeclampsia in 28th week of her first gravidity. Within one day severe haemolysis and diffuse haemorrhage occurred and an emergency section was performed. Subsequently, supported respiration became necessary because of diffuse infiltrations of the lung. Laboratory data then were conclusive for a thrombotic thrombocytopenic purpura (Moschcowitz's disease) and therefore the patient was subjected to plasma exchange during seven days: this measure induced rapid and full clinical and laboratory remission. The case history is discussed on

A case is presented of cerebral infarction secondary to carotid artery thrombosis in a patient with hemolysis, elevated liver function, and low platelets (HELLP) syndrome. The patient's rebound thrombocytosis may have contributed to a hypercoagulable state. Severe preeclamptic patients with HELLP syndrome who experience a reactive thrombocytosis may represent a subset of women who have postpartum thrombosis.

Four cases of severe preeclamptic patients with the findings of hemolysis (H), elevated liver enzymes (EL) and a low platelet count (LP) were presented. Maternal mortality occurred in two of the four cases with one experiencing liver rupture. Intrauterine fetal death occurred in 3 cases. The entity has been termed the HELLP syndrome. The high maternal and perinatal mortality of our cases was mainly due to the severe coagulopathy associated with the HELLP syndrome. The practicing obstetrician must be knowledgeable about this severe consequence of toxemia in pregnancy and aggressive management i

On the basis of a literature review the extremely severe form of EPH gestosis--the HELLP syndrome is described. In the clinical manifestations of the syndrome a symptom triad predominates: haemolytic anaemia, raised values of hepatic enzymes and thrombocytopenia. The only causative treatment saving the life of the mother and fetus is possibly early termination of pregnancy and adequate supplementation of blood deficiency. The pathophysiology, clinical manifestations, differential diagnosis and treatment of the HELLP syndrome are discussed.

Neutropenia occurs often among the newborns of women with hypertension, but its cause, mechanism, and clinical consequences have not been adequately studied. Of 72 infants whose mothers had hypertension during pregnancy, 35 (49 percent) had neutropenia, which persisted from 1 hour to 30 days. The disorder was more prevalent among newborns whose growth had been retarded in utero (P less than 0.01), those who had been delivered prematurely (P less than 0.001), and those whose mothers had had severe hypertension (P less than 0.002) or hypertension and the HELLP syndrome (hemolysis, elevated liver

A case of HELLP-syndrome is reported and attention called to the importance of early recognition of the syndrome whose clinical characteristics at times delay intervention that can be important for the wellbeing of mother and foetus.

This is a case report on a nulliparous women, who developed the HELLP syndrome following intravenous tocolysis. Contrary to our expectations, neither schistocytes nor signs of intravasal coagulation were apparent; however, we found Fenoterol-associated anti-erythrocyte antibodies. However, the low titre of these antibodies does not explain the massive haemolysis. It is suggested, that these antibodies represent a causal factor in developing the HELLP syndrome.

A case of 30 year-old female with HELLP syndrome, who had undergone emergency caesarean section under general anesthesia, was reported. HELLP syndrome is characterized by hemolysis, liver dysfunction and thrombocytopenia, besides symptoms of severe toxemia of pregnancy. After an awake orotracheal intubation, anesthesia was maintained with nitrous oxide, oxygen and muscle relaxant. Blood pressure was controlled with intravenous administration of nitroglycerin. Though the eclampsia was recognized several times during and after the operation, the patient and her baby had no complication nor seque

We have reported a case of subcapsular hematoma of the liver, diagnosed and observed by ultrasonography, in association with the HELLP syndrome. The patient's condition improved with conservative management, but the hematoma was clearly shown to last for more than 12 weeks, which shows the need for extended postpartum follow-up and appropriate patient consultation.
