Schistocytosis, aminotransferase elevation and thrombocytopenia in preeclampsia/eclampsia.

Some preeclamptic patients have schistocytosis, abnormal liver function tests and thrombocytopenia. To determine how strongly these three abnormalities cluster with each other, a sequential series of 49 preeclamptic or eclamptic patients was analyzed for the presence of schistocytosis, serum aminotransferase elevation and thrombocytopenia. These three abnormalities were found less often together (the HELLP syndrome) than singly or in pairs. These data do not clearly separate HELLP patients from other preeclamptic patients.

[Hematologic diseases in pregnancy; a danger to mother and child or a calculable risk?].

In 1985 the authors delivered six gravidae who had blood diseases during pregnancy; they included one patient with thalassemia minor, one with Hodgkin's disease, one with Glanzmann's thromboasthenia, one with Von Willebrand-Jürgens syndrome, one with Werlhof's disease and one with HELLP syndrome. The problems which arose were managed in close collaboration with internists and pediatricians and healthy infants were born in all cases. No aggravation of the basic disease was seen in any of the mothers. Cesarean section was necessary in two cases, while vaginal deliveries were accomplished in the

Haptoglobin helps diagnose the HELLP syndrome.

In a case of the HELLP syndrome, anemia was absent, and bilirubin and lactic dehydrogenase levels were normal. In combination with minimal microangiopathic changes in red cell morphology, a decrease in serum haptoglobin was useful to corroborate the presence of intravascular hemolysis.

[Hemorrheologic findings in severe gestosis].

On the basis of the hemorrheologic and hemodynamic data of 30 severe gestoses the importance of rheology in the diagnosis and treatment of these conditions is discussed. It was found that the gestoses can occur with a variety of clinical phenomena (HELLP syndrome, hypervolemic gestosis). The classic picture comprises hemoconcentration, increased erythrocyte aggregation, low central venous pressure and low-normal cardiac output with increased blood viscosity. Depending on the condition of the microcirculation, laboratory values can fluctuate considerably. The mean antithrombin-III value was not

HELLP syndrome: a severe consequence of pregnancy-induced hypertension.

A severe consequence of pregnancy-induced hypertension can be the development of HELLP syndrome (H = hemolysis; EL = elevated liver enzymes; LP = low platelets). The etiology and clinical manifestations of HELLP syndrome are discussed through a review of research findings and a case study. Nursing interventions to facilitate recognition and management of pregnant women with this life-threatening condition are presented.

[Is it necessary to specify the HELLP syndrome?].

In 1982 Weinstein described the Hellp-Syndrome in pre-eclampsia. Two personal case histories have led the authors to discuss the need to specify this syndrome. Apart from the classical elements of pre-eclampsia, which are: arterial hypertension, proteinuria and oedema, there are bowel symptoms in 86% of cases and on the biological level there is thrombocytopenia and a rise in the transaminases as well as a haemolytic anaemia. Some cases of rupture of the liver have been described. Treatment is that of pre-eclampsia. This involves treatment with cortisone and sometimes with heparin. Delivery of

Antepartum reversal of hematologic abnormalities associated with the HELLP syndrome. A report of three cases.

The "HELLP syndrome" refers to the occurrence of thrombocytopenia, elevated liver enzymes and, at times, hemolysis in patients with preeclampsia. Although uncontrolled series have reported a reversal of thrombocytopenia and elevated liver enzymes with aspirin and colloid infusion, most authors recommend that delivery be done soon after the diagnosis is made. The abnormal laboratory parameters of the HELLP syndrome may be reversed completely with bed rest alone, and with preterm gestations, conservative management may be possible.

Hemolysis, elevated liver enzymes, and low platelet count syndrome: nursing care of the critically ill obstetric patient.

Although patients with severe preeclampsia and eclampsia are infrequently admitted to critical care areas, it is important that the critical care nurse be aware of the HELLP syndrome. The case presentations demonstrated hemolysis, liver dysfunction, and abnormal platelet consumption. Knowledge of the clinical and laboratory findings will assist in nursing assessment and intervention for these critically ill obstetric patients.

Hydralazine-induced hepatitis in pregnancy.

During 1983, 38 patients with pregnancy-induced hypertension were treated with hydralazine-apresoline (1-hydrazinophthalazine). During the course of their treatment, five of these patients showed evidence of the HELLP syndrome described by Weinstein (hemolysis, thrombocytopenia, and elevated liver enzymes). Evidence is presented on the role of hydralazine in producing hepatocellular damage. Liver function is compromised during the natural course of pregnancy-induced hypertension, and 5-10% of preeclamptic patients develop the HELLP syndrome. In many places hydralazine is the drug of choice in

[HELLP syndrome: a rare, threatening complication in pre-eclampsia].

A very rare, but severe complication, occurring together with preeclampsia, is the so-called HELLP syndrome (H for haemolysis, EL for elevated liver enzymes and LP for low platelet counts). Perinatal mortality for this syndrome is estimated at 9.5% up to 60%, maternal mortality at 3.5%. We examined retrospectively 11 patients of the last 5 years, presenting not only hypertension, proteinuria and oedema, but also pathologically elevated data concerning transaminases, indirect bilirubin and thrombocytopenia; the mean value for GI in this group was 7.7 (3-11). Thrombocyte counts showed mean value

Maternal-perinatal outcome associated with the syndrome of hemolysis, elevated liver enzymes, and low platelets in severe preeclampsia-eclampsia.

During an 8-year period, 112 severe preeclamptic-eclamptic patients with the above syndrome were studied. The incidence of this syndrome was significantly higher in white patients, in patients with delayed diagnosis of preeclampsia and/or delayed delivery, and in multiparous patients. Twenty-six patients had amniocentesis and 16 received epidural anesthetics. There was one maternal bleeding episode associated with epidural anesthetics. The use of steroids in 17 patients did not improve maternal platelet count. The overall perinatal mortality was 367 per 1000 and neonatal morbidity was signific

Pregnancy induced hypertension complicated by thrombocytopenia, haemolysis and elevated liver enzymes (HELLP) syndrome. Renal biopsies and outcome.

Twelve patients are presented with the symptomatology of haemolysis, elevated liver enzymes and low platelet count (HELLP syndrome). Only 3 patients experienced a progressive course; the rest did not differ from a group of 130 patients suffering from severe preeclampsia. There was no significant difference in the concentration of plasma fibrinogen, fibrinogen monomer complex and antithrombin III. Two patients were treated with heparin without improvement of the coagulation data or the severity of the disease. The subgrouping of very severe preeclampsia may be desirable, but it is felt that thi

Severe hypoglycemia associated with HELLP syndrome.

Presented is a case of the syndrome of hemolysis, elevated liver enzymes, and low platelet counts, in which severe maternal hypoglycemia occurred 8 hours after delivery. It is proposed that the hepatic pathologic features in this case were severe enough to interfere with glycogenolysis and glyconeogenesis.

Sonographic findings in severe preeclampsia twenty-four hours prior to clinical signs.

We describe a patient who had abnormal sonographic findings of the liver 24 hours before signs and symptoms of severe preeclampsia and HELLP syndrome (hemolysis, elevated liver enzymes, and low platelet count). The abnormal sonographic appearance of the liver prompted further investigation and was instrumental in the management of this case.

Preeclampsia/eclampsia with hemolysis, elevated liver enzymes, and thrombocytopenia.

Data are presented to define a unique group of preeclamptic/eclamptic women with the findings of hemolysis, elevated liver enzymes, and a low platelet count. This syndrome is a variant of severe preeclampsia and may develop either antepartum or postpartum. A nonobstetric diagnosis such as gastrointestinal or hematologic disease is often made. When the patient presents with the hemolysis, elevated liver enzymes, and low platelet (HELLP) syndrome, supportive therapy is used and delivery expedited to improve maternal and fetal outcome.
