Liver Embolization for Subcapsular Hematoma in HELLP (Hemolysis, Elevated Liver Enzymes, and Low Platelets) Syndrome: A Case Report.

HELLP syndrome is a severe obstetric complication characterized by hemolysis, elevated liver enzymes, and low platelet count, typically occurring in the third trimester or postpartum period. One of its rare but life-threatening complications is subcapsular hepatic hematoma, which may rupture and cause catastrophic hemorrhage. We report the case of a 38-year-old woman with type 2 diabetes mellitus who developed severe right upper quadrant pain three days after a cesarean delivery complicated by eclampsia. Laboratory findings were consistent with HELLP syndrome. Abdominal ultrasound and contrast

Preeclampsia: Contemporary Concepts in Pathophysiology, Risk Stratification, Prevention and Monitoring.

Preeclampsia is a heterogeneous disorder affecting approximately 2-5% of pregnancies and remains a major cause of maternal and perinatal morbidity and mortality worldwide. Its clinical presentation ranges from mild, nearly asymptomatic forms to severe conditions progressing to eclampsia or HELLP syndrome. Despite significant advances in understanding its pathophysiology, preeclampsia continues to pose diagnostic and therapeutic challenges. In recent years, intensive research efforts have focused on developing comprehensive diagnostic criteria, identifying novel biomarkers, improving risk predi

Control of HELLP syndrome and pre-eclampsia with microbiota.

Various physiological and pathophysiological conditions, including metabolism, immunological tolerance, behavior, and cardiovascular disease, can be studied and manipulated by the complex human microbiome. It plays a significant role in maternal-fetal well-being and can affect complex pregnancy-related disorders such as HELLP Syndrome and pre-eclampsia (PE). Evidence continues to accumulate regarding the delicate balance of gut microbiota (GM) functions, including the potential role in immunologic conditions such as immune thrombocytopenic purpura, where the microbiota may influence platelet d

Hypocholesterolaemia as an early feature of presumed acute fatty liver of pregnancy.

Differentiating between acute fatty liver of pregnancy (AFLP) and haemolysis, elevated liver enzyme and low platelet (HELLP) syndrome may be difficult. The treatment for both conditions is expedited delivery, with the infrequent exception of expectant management with close monitoring and consideration of dexamethasone with HELLP in the setting of severe prematurity. Correct diagnosis may have implications for maternal morbidity and mortality, risk of recurrence, risk to the child, and the risk of long-term maternal disease. The Swansea criteria lack specificity, a significant proportion of wom

Epidemiological Risk Factor Analysis for Maternal Mortality Associated with Eclampsia: A Single-Center Study from Tertiary Hospital Garut.

BACKGROUND: Maternal mortality remains a major global health challenge, particularly in low- and middle-income countries. Eclampsia, a severe complication of hypertensive disorders in pregnancy, continues to contribute substantially to maternal and perinatal deaths despite advances in obstetric care. Previous studies have highlighted the role of clinical and laboratory abnormalities in worsening outcomes; however, evidence from developing settings remains limited. A clearer understanding of maternal mortality risk factors in eclampsia is essential to improve early detection, risk stratificatio

A simple bedside technique for managing a non-deflating Foley catheter balloon in a resource-limited setting - a case report.

BACKGROUND: Urethral catheterization is a frequently performed procedure that can sometimes result in complications such as failure to deflate a Foley catheter balloon. This can present significant challenges in managing a patient. CASE PRESENTATION: We describe a case involving a 37-year-old woman diagnosed with severe cerebral malaria, eclampsia, and HELLP syndrome whose Foley catheter balloon failed to deflate despite the use of standard techniques. An easy bedside technique was used by cutting the catheter to a short length and using a spinal needle stylet to unblock the existing inflation

The risk for the development of hypertensive complications in oocyte donation pregnancy: a systematic review and individual participant data meta-analysis (DONOR IPD).

BACKGROUND: Oocyte donation (OD) is an established ART involving an oocyte donor and recipient with a rising number of treatments. Previous meta-analyses highlight increased risks of hypertensive complications compared to naturally conceived (NC) and IVF/ICSI pregnancies, including pregnancy-induced hypertension (PIH) and preeclampsia (PE), but limitations exist due to study quality and heterogeneity. OBJECTIVE AND RATIONALE: The DONOR (DONation of Oocytes in Reproduction) individual participant data (IPD) meta-analysis aims to generate clinically relevant and robust evidence regarding the de

A Prospective Observational Study on Serum Lactate Dehydrogenase and Uric Acid Levels in Hypertensive Disorders of Pregnancy and Prediction of Fetomaternal Outcome.

Introduction Hypertensive disorders of pregnancy are a major cause of maternal and perinatal morbidity and mortality, particularly in low- and middle-income countries. Early identification of women at risk using simple biochemical markers may aid in timely intervention and improved outcomes. Serum lactate dehydrogenase (LDH) and uric acid have been proposed as indicators of disease severity in hypertensive pregnancies. The study aimed to evaluate the association of serum LDH and uric acid levels with the severity of hypertensive disorders of pregnancy and their relationship with maternal and f

Endothelial glycocalyx damage underlies the generalized endothelial dysfunction in preeclampsia.

In pregnancy, endothelial dysfunction serves as a key mechanism linking maternal-fetal immune maladaptation and soluble antiangiogenic factors to the clinical presentation of preeclampsia. Although most of these aspects have been elucidated in isolation in humans and in animal models, a complete picture of their relationship with the clinical manifestations of preeclampsia has been elusive. Perturbation of endothelial glycocalyx results from conflicting inflammatory messages and leads to a paradoxical situation involving tissue distress and stimulation of maternal syndrome. Increased endotheli

Successful management of amniotic fluid embolism with cardiac arrest and liver rupture: a case report.

Amniotic fluid embolism (AFE) is an unpredictable and critical obstetric emergency. Although infrequent, it carries a significant risk of mortality. AFE is typically characterized by abrupt circulatory and respiratory failures, along with disseminated intravascular coagulation (DIC). Another rare complication during pregnancy is spontaneous liver rupture, which poses a serious danger to both the mother and the fetus. AFE generally manifests suddenly during labor, delivery, or shortly after childbirth and is believed to be triggered by the entry of amniotic fluid components into the mother's bl

Liver Conditions Specific to Pregnancy: Optimizing Management and Outcomes.

Pregnancy is accompanied by complex physiological adaptations that influence hepatic metabolism, bile transport, coagulation, and immune tolerance, which may precipitate the development of pregnancy-specific liver disorders. Gestational liver disease comprises a distinct group of conditions such as hyperemesis gravidarum (HG), intrahepatic cholestasis of pregnancy (ICP), acute fatty liver of pregnancy (AFLP), and the spectrum of preeclampsia that includes hepatic involvement and Hemolysis, elevated liver enzymes, and low platelets (HELLP) syndrome. Although heterogeneous in presentation and pa

Does nocturnal hypertension reclassify maternal-fetal risk in women with white-coat hypertension? A cohort study in the second half of pregnancy.

Hypertensive disorders of pregnancy are defined according to office blood pressure (BP), yet out-of-office BP phenotypes may carry different prognostic implications. White-coat hypertension (WCH) is traditionally defined using 24-h ambulatory BP monitoring (ABPM) without considering nighttime BP, despite growing evidence supporting the prognostic relevance of nocturnal hypertension. This study aimed to evaluate the maternal-fetal risk associated with WCH according to the presence or absence of nighttime hypertension during the second half of pregnancy. We conducted a retrospective cohort study

Experiences of discrimination, paternal support, and hypertensive disorders of pregnancy (HDP) in the MIEHR study, Houston, Texas.

BACKGROUND: Discrimination is a psychosocial stressor that, through activation of stress-response pathways, may influence risk of adverse perinatal health outcomes. However, few studies have evaluated the association between discrimination and hypertensive disorders of pregnancy (HDP), and none have explored effect modification by race or paternal support, which may buffer the adverse effects of discrimination. METHODS: We included 1,847 non-Hispanic Black and White participants from the Maternal and Infant Environmental Health Riskscape (MIEHR) study. Krieger's validated Experiences of Discr
