Presenting a prediction model for HELLP syndrome through data mining.

BACKGROUND: The HELLP syndrome represents three complications: hemolysis, elevated liver enzymes, and low platelet count. Since the causes and pathogenesis of HELLP syndrome are not yet fully known and well understood, distinguishing it from other pregnancy-related disorders is complicated. Furthermore, late diagnosis leads to a delay in treatment, which challenges disease management. The present study aimed to present a machine learning (ML) attitude for diagnosing HELLP syndrome based on non-invasive parameters. METHOD: This cross-sectional study was conducted on 384 patients in Tajrish Hos

Increased circulating miR-155 identifies a subtype of preeclamptic patients.

INTRODUCTION: Preeclampsia is a common and severe pregnancy complication. The syndrome is highly heterogeneous, making accurate classification difficult, which is not conductive to find ways to predict and prevent this syndrome. Recently, we reported that high placental miR-155 defined a new subtype of preeclampsia. Here, we aimed to examine whether high maternal sero-miR-155 could be a marker to identify this subtype. METHODS: To explore whether the patients with high sero-miR-155 no matter in first and third trimester, we conducted a case-control and a longitudinal cohort study. We measured

Trends in adverse pregnancy outcomes in Louisiana, 2017 to 2022.

Natural disasters can lead to more adverse pregnancy outcomes (APO). It is unclear if the extended COVID-19 pandemic has impacted APOs and pre-existing conditions among perinatal populations with increased risk of severe maternal morbidity and mortality.A retrospective chart review was conducted of hospital records and birth certificates in the largest birth hospital in Louisiana from 2017 to 2022. Amongst 27,877 births (50.9% White, 38.3% Black, 28.9 ± 5.6 years), gestational diabetes (GDM) was lowest in pre-pandemic conceptions (11.0%, June 2017-May 2019) and rose to 16.4% early pandemic (Oc

Uterine torsion during cesarean section coinciding with HELLP syndrome.

Uterine torsion belongs to one of the most dangerous labor complications also because of its rarity. As it is not common for an obstetrician to come across this state, it is usually not at the very top of the differential diagnostics list when solving acute child delivery complications. However, it is serious enough to pose a lethal threat to both mother and child. In this case, the term gravidity was ended by acute cesarean section because of HELLP syndrome. During the operation, as well as after a complicated delivery and hysterotomy suture uterine torsion of 120 degrees to the right, the pa

Hepatic Infarct: Uncommon Causes of an Uncommon Vascular Complication Revealed at Nontransplant Medical Autopsies.

BACKGROUND AND AIMS: Hepatic infarcts are uncommon vascular lesions due to dual vascular supply of the liver characterized by the peripherally-located wedge-shaped large zones of necrosis in the hepatic parenchyma. They are commonly seen as a complication of liver transplant. MATERIAL AND METHODS: The cases of hepatic infarcts of the native liver detected in medical autopsies were searched in the archives and their clinical, biochemical, gross, and histomorphological details were retrieved. RESULTS: Three cases of hepatic infarct of the native liver were detected from among 1865 consecutive

Hypertensive pregnancy deaths: a three-year review at Ghana's largest referral hospital.

BACKGROUND: Maternal mortality continues to be a significant public health challenge in sub-Saharan Africa. Even though the leading causes, such as hypertensive disorders in pregnancy and obstetric hemorrhage have remained unchanged, associated factors continue to significantly impact reduction efforts in the maternal mortality ratio. Despite efforts, Ghana's maternal mortality ratio has not significantly decreased since 2015, the end of the Millennium Development Goals (MDGs). To achieve Sustainable Development Goal (SDG) targets, it is essential to understand and focus on reducing deaths fro

Mitochondrial genetic landscape and its correlation with immune cell infiltration in preeclampsia: Insights from bioinformatics.

BACKGROUND: Preeclampsia (PE), a hypertensive pregnancy disorder, remains a leading cause of maternal and perinatal morbidity and mortality. Mitochondria-related placental metabolic dysfunction is implicated in PE, but its mechanistic role is unclear. This study aimed to identify mitochondria-related genes (MRGs) and their possible regulatory mechanisms in PE. METHODS: Differentially expressed mitochondria-related genes (MRGs) of PE were identified from Gene Expression Omnibus (GEO) dataset GSE114691 and GSE190971. LASSO regression analysis was used to screen key MRGs. Datasets GSE75010 and G

Subcapsular Hepatic Hematoma Secondary to Preeclampsia With a Successful Maternal Outcome: A Case Report.

Subcapsular hepatic hematoma (SHH) is a rare and life-threatening condition, often leading to severe maternal complications. Its diagnosis is frequently challenging due to nonspecific symptoms and a reliance on ultrasound findings. Case reports for this type of pregnancy complication are limited due to its low incidence. A 30-year-old pregnant woman at 32.3 weeks with preeclampsia with severe features presented with epigastric pain and high blood pressure, requiring an emergency C-section. She developed a peri-hepatic subcapsular hematoma, leading to hypovolemic shock and multiple surgeries, i

ULK2 deficiency stratifies autophagy-driven molecular subtypes and exacerbates trophoblasts apoptosis in preeclampsia.

INTRODUCTION: Preeclampsia (PE), a placenta-originated hypertensive disorder of pregnancy, lacks targeted therapies despite its significant contribution to maternal and fetal morbidity. Emerging evidence implicates autophagy dysregulation in PE pathogenesis, though its molecular heterogeneity remains unexplored. This study aims to investigate autophagy-related molecular subtypes in PE and identify ULK2 key regulators linking autophagic imbalance to placental dysfunction. METHODS: Placental transcriptomic datasets were analyzed to identify autophagy-related differentially expressed genes (ARGs

Recombinant a disintegrin-like and metalloproteinase with thrombospondin type 1 motifs 13 (ADAMTS-13) for acute and prophylactic treatment of congenital thrombotic thrombocytopenic purpura in pregnancy.

Thrombotic thrombocytopenic purpura (TTP) is a disorder of the absence or severe depletion of a disintegrin-like and metalloproteinase with thrombospondin type 1 motifs 13 (ADAMTS-13) protease, which cleaves von Willebrand factor multimers. In its absence, exposed platelet binding sites on large von Willebrand factor multimers bind and aggregate platelets, causing microangiopathic hemolytic anemia and thrombosis. The rare congenital form of TTP (cTTP) is caused by mutations in the ADAMTS-13 gene. Pregnant cTTP patients are at very high risk of developing complications. Current recommendations

Quantifying prenatal and postnatal mortality in extreme early onset fetal growth restriction: A systematic review and incidence analysis.

BACKGROUND AND OBJECTIVE: Extreme early-onset fetal growth restriction (EE-FGR) is associated with significant perinatal morbidity and mortality. As short- and long-term prognoses are influenced by both gestational age and severity of FGR, quantifying risk is inherently challenging. We hypothesized that a meta-analysis would provide quantifiable estimates of perinatal outcomes in pregnancies complicated by EE-FGR. METHODS: We searched PubMed, Scopus, Embase, and Web of Science databases for relevant studies published from inception to March 2024. We included all original studies involving sin

Development of a Risk Score for the Prediction and Management of Pre-Eclampsia in Low-Resource Settings.

Background: Pre-eclampsia is a significant hypertensive disorder affecting 2-8% of pregnancies globally, significantly contributing to maternal/perinatal deaths. Early identification of at-risk patients is crucial for reducing these mortalities, yet first-trimester screening remains inaccessible in many low-resource settings. This study aims to develop a second-trimester risk stratification model based on clinical parameters to assist in managing pre-eclampsia in diverse healthcare contexts. Methods: This retrospective cohort study analyzed medical records from 700 pregnancies (350 with preecl

[The new S2k guidelines on hypertensive diseases during pregnancy (HES): diagnosis and treatment-What do anesthetists need to know?].

Hypertensive diseases of pregnancy (HES) affect approximately 6-8% of all pregnancies and contribute to 20-25% of perinatal mortality globally. Among these, preeclampsia represents the most severe manifestation, contributing to over 70,000 maternal and 500,000 perinatal deaths anually worldwide [1]. In July 2024, the updated S2k guidelines on hypertensive diseases during pregnancy (HES) were published, presenting updated recommendations for improved diagnosis and therapeutic strategies. Notable advancements in the revised guidelines include an emphasis on the early identification of patients a
