Restricting Placental Transfer of Angiotensin Receptor Blockers to Mitigate Fetal Toxicity: A Potential Therapeutic Strategy for Preeclampsia.

BACKGROUND: Enhanced AT1R (angiotensin II type 1 receptor) signaling contributes to preeclampsia development; however, angiotensin receptor blockers are contraindicated during pregnancy because of associated fetal toxicity. We aimed to determine whether chemical modification of an angiotensin receptor blocker restricts placental transfer while preserving maternal pharmacological activity. METHODS: Polyethylene glycol-modified olmesartan derivatives were synthesized to increase molecular size and hydrophilicity and evaluated in vitro for AT1R inhibitory activity. Placental transfer was assesse

Spatio-temporal expression of E-cadherin mediates trophoblast lineage transitions during human placentation.

Human placentation relies on the precise differentiation of embryo-derived trophoblasts into specialized lineages, a process strictly regulated by the spatio-temporal expression of key genes. Herein, we report that the expression of the transmembrane glycoprotein, known as E-cadherin, fluctuates dynamically across distinct trophoblast subpopulations rather than acting as a static structural tether. Along the villous pathway, E-cadherin expression initially increases to facilitate the aggregation of cytotrophoblast cells (CTs) and is subsequently downregulated via transcriptional repression, en

Genetically Predicted IgG N-Glycosylation Traits and Risk of Pre-Eclampsia or Eclampsia: A Two-Sample and Multivariable Mendelian Randomization Study.

BACKGROUND: Maternal immunoglobulin G (IgG) has been implicated in placental and vascular dysfunction across pre-eclampsia and eclampsia. While clinical evidence supports the relevance of IgG-mediated immune regulation, whether specific IgG N-glycosylation traits-which critically regulate IgG concentration and effector function-are associated with the combined outcome of pre-eclampsia or eclampsia remains unclear. METHODS: A two-sample Mendelian randomization (MR) study evaluated the causal relevance of 77 IgG N-glycosylation traits for the combined outcome of pre-eclampsia or eclampsia. Prim

Development of antenatal risk prediction model for preeclampsia with severe features in Lagos, Nigeria (PreSev study): protocol of a prospective cohort study.

BACKGROUND: Preeclampsia with severe features is a major contributor to maternal and perinatal morbidity and mortality, particularly in low- and middle-income countries (LMICs). Despite improvements in obstetric care, early identification of women at highest risk remains limited, and existing prediction tools lack contextual specificity for African populations. OBJECTIVE: This study describes the protocol for the PreSev Study, which aims to develop and validate a machine-learning risk prediction model for preeclampsia with severe features among pregnant women in Lagos, Nigeria. METHODS: This

First-trimester ophthalmic artery Doppler for prediction of early-onset and preterm preeclampsia: a systematic review and critical appraisal.

BACKGROUND: First-trimester screening for preterm preeclampsia (PE) is often performed using the Fetal Medicine Foundation (FMF) Bayes' theorem-based competing-risks model, which combines maternal factors with mean arterial pressure, uterine artery pulsatility index and placental growth factor, with pregnancy-associated plasma protein-A included in some models. Ophthalmic artery (OA) Doppler has been proposed as an additional marker of maternal vascular adaptation, but its clinical contribution remains uncertain. This review critically evaluates the predictive performance of first-trimester OA

Maternal and neonatal outcomes in a reduced visit antenatal care program for women at low risk for complications.

BACKGROUND: Public healthcare operates with constrained resources, creating a need to reallocate capacity toward the growing population of pregnant women at high risk for complications. Therefore, a modified antenatal care program with a reduced number of in-person visits supplemented with telemedicine consultations was introduced for low-risk women in Southeast Sweden. OBJECTIVE: To evaluate whether a modified antenatal care program is non-inferior to a standard antenatal care program regarding maternal and neonatal outcomes. STUDY DESIGN: A modified antenatal care program was evaluated at

Late-onset preeclampsia is characterized by accelerated placental aging.

Late-onset preeclampsia (LOPE) is a major pregnancy complication characterized by hypertension and placental dysfunction, resolving only upon delivery. Here, we show that LOPE placentae undergo accelerated molecular aging, marked by telomere attrition, DNA damage, and trophoblast senescence. Using primary placental tissue and trophoblast organoids, we demonstrate oxidative stress as a driver of telomere shortening and angiogenic imbalance. Inflammation did not alter placental aging trajectories. Antioxidant treatment (superoxide dismutase) preserved telomere length, reduced DNA damage, and res

Periconceptional neighborhood deprivation and risk of gestational and postpartum hypertensive disorders in a population-based cohort.

Hypertensive disorders during pregnancy and postpartum are major causes of maternal morbidity and mortality, yet associations with social determinants of health, including neighborhood deprivation, remain understudied. We examined the association of periconceptional neighborhood deprivation with the risk of gestational and postpartum hypertensive disorders in a population-based cohort of 281,621 pregnant individuals aged 15-44 years without chronic hypertension who delivered at Kaiser Permanente Northern California from 2011 to 2020. Neighborhood deprivation index (NDI) quartiles were assigned

Obstetric and thrombotic outcomes among pregnant patients with myeloproliferative neoplasms: a propensity score matched real-world analysis.

Pregnancies complicated by BCR::ABL1-negative myeloproliferative neoplasms (MPNs) are associated with increased maternal and thrombotic risk; however, large real-world comparative data remain limited. We performed a multicenter retrospective propensity score-matched cohort study using the TriNetX database to evaluate maternal, obstetric, and thrombotic outcomes among pregnant patients with MPN compared with matched non-MPN controls. Baseline demographics, comorbidities, and antithrombotic medication use were balanced after matching (standardized mean difference < 0.1). Among 4,467,127 pregnanc

Maternal PM2.5 exposure induces preeclampsia-like phenotypes in mice through placental angiogenic dysfunction and lncRNA-RUNX1 dysregulation.

Preeclampsia (PE) is an idiopathic gestational disorder defined by placental structural abnormalities, systemic inflammatory activation, angiogenic factor imbalance, and endothelial dysfunction. While existing epidemiological studies have established a correlational association between maternal fine particulate matter (PM2.5) exposure and elevated PE risk, the specific underlying molecular mechanisms driving this association have not been fully elucidated. Using a C57BL/6J mouse model of maternal PM2.5 exposure via intranasal instillation (3 mg/kg body weight, every other days from gestational

Evolving Fetal Biometry in Fetal Intrahepatic Portosystemic Shunt With Late-Onset Preeclampsia and Focal Umbilical Cord Hypercoiling: A Case Report.

Fetal intrahepatic portosystemic shunt may complicate fetal growth assessment, particularly when maternal and umbilical cord abnormalities coexist. We report a pregnancy first evaluated at our center at 26 + 3 weeks for suspected severe early-onset fetal growth restriction, with estimated fetal weight below the 1st percentile and abdominal circumference below the 3rd percentile. Prenatal ultrasound and fetal magnetic resonance imaging findings were consistent with fetal intrahepatic portosystemic shunt. Subsequent serial assessments showed improved fetal biometry. Routine umbilical artery and

Prenatal exposure to hypertensive disorders and adolescent brain white matter development.

BACKGROUND: Hypertensive disorders of pregnancy (HDP) are associated with altered in utero neurodevelopment, yet long-term impact on adolescent white matter architecture remains unclear, particularly by HDP subtype and sex. While preeclampsia/eclampsia (P/E) is clinically more severe, pregnancy-related hypertension (PR-HTN) may involve longer in-utero exposure to hemodynamic stress. This study examined how distinct HDP exposures influence longitudinal white matter measures in adolescents. METHODS: Using a longitudinal cohort with two MRI timepoints (baseline ages 9-11 years; 2-year follow-up
