Maternal hypertensive disorders of pregnancy and childhood neurodevelopmental disorders: A two-sample Mendelian randomization.

OBJECTIVE: To investigate the association between maternal hypertensive disorders of pregnancy (HDP) and childhood neurodevelopmental disorders (NDD) by Mendelian randomization (MR). METHODS: A two-sample MR analysis was conducted to evaluate the causal effects of HDP, including gestational hypertension (GH), pre-eclampsia superimposed on chronic hypertension, and pre-eclampsia or eclampsia on childhood NDD, such as cerebral palsy (CP), autism spectrum disorder (ASD), and hyperkinetic disorders (HD). The exposure and outcome data were extracted from the FinnGen database. As the genetic and ph

Neither Metabolic Dysfunction Associated Steatotic Liver Disease Presence Nor Fibrosis Severity, By FIB4, Predicts Adverse Pregnancy Outcomes.

BACKGROUND: Metabolic dysfunction associated steatotic liver disease (MASLD) has been associated with adverse pregnancy outcomes. Data are limited with regard to whether this association is solely related to MASLD versus comorbid metabolic conditions. This study aims to evaluate the independent contribution of MASLD to maternal and fetal outcomes. METHODS: This single center retrospective study included adult pregnant patients who received care between 2000 and 2021. Patients with MASLD before pregnancy were age-matched with controls without chronic liver disease. Baseline FIB-4 score, within

Preeclampsia 2.0: limitations and challenges of the two-stage hypothesis, and beyond.

The pregnancy-specific syndrome preeclampsia remains the syndrome of hypotheses. Still, there is not a single hypothesis explaining the etiology of the full spectrum of preeclampsia. This has direct consequences for the clinical management of the syndrome. So far, no single early biomarker has been identified to predict all women who will develop preeclampsia later in pregnancy. Similarly, no preventive treatment for all types of preeclampsia has been integrated into clinical routine. Interestingly, the last decade has not seen much progress in the quest of identifying the pathophysiological p

Circulating MALAT1 in Preeclampsia and Association With Cardiometabolic Risk.

BACKGROUND: Preeclampsia is a hypertensive disorder affecting 2% to 8% of pregnancies. Women with a history of preeclampsia have an increased risk of cardiovascular disease. The long noncoding RNA MALAT1 is shown to regulate inflammatory responses linked to cardiovascular disease. MALAT1 is decreased in preeclampsia placentas and may have a cis-regulatory function on neighboring RNAs. METHODS: Expression of MALAT1, NEAT1, mascRNA, SCYL1, and FRMD8 was assessed in peripheral blood mononuclear cells, and MALAT1 in plasma and extracellular vesicles, at 22 to 24 and 36 to 38 weeks of gestation in

TP53BP2 Promotes Placental Autophagy and Preeclampsia via G9a and DNMT1 Cooperatively Modulating E2F1.

Preeclampsia (PE) is a pregnancy-related disorder characterized by impaired migration and invasion of trophoblast cells. Recent studies have highlighted the critical role of autophagy in the development of PE. However, the precise mechanisms underlying the upregulation of autophagy in PE remain unclear. This study demonstrated that the expression of the tumor suppressor p53-binding protein 2 (TP53BP2) is significantly upregulated in patients with PE. Silencing of TP53BP2 not only decreases autophagy but also attenuates PE progression in rat model. Moreover, TP53BP2 expression was positively co

A novel two-sample Mendelian randomization framework integrating common and rare variants: application to assess the effect of HDL-C on preeclampsia risk.

Mendelian randomization (MR) has become an important technique for establishing causal relationships between risk factors and health outcomes. By using genetic variants as instrumental variables, it can mitigate bias due to confounding and reverse causation in observational studies. Current MR analyses have predominantly used common genetic variants as instruments, which represent only part of the genetic architecture of complex traits. Rare variants, which can have larger effect sizes and provide unique biological insights, have been understudied due to statistical and methodological challeng

Adjusting for truncated study duration in recurrent event analysis: A weighting approach for clinical trials.

BACKGROUND: In recurrent event analysis with fixed follow-up intervals, truncated follow-up due to early dropout or study termination introduces bias and reduces precision in risk estimates, particularly in clinical trials where shorter observation periods may underestimate event risks. METHODS: We propose a time-based weighting approach using the ratio of observed-to-expected follow-up duration in the Prentice-Williams-Peterson Gap Time (PWP-GT) model. The method was evaluated in simulations and applied to a double-blinded trial (N = 4000) comparing 500 mg vs. 1500 mg daily calcium supplemen

Risk of pre-eclampsia according to gestational age across three decades: population-based study.

OBJECTIVES: To determine the risk of pre-eclampsia in ongoing pregnancies according to gestational age. We also studied whether the gestational-age-specific risks of developing pre-eclampsia have changed over recent decades. METHODS: This was a population-based study using data from the Medical Birth Registry of Norway. The study population included all singleton pregnancies in Norway delivered between 21 and 43 weeks' gestation during the period 1984-2019. We calculated the proportion of pregnancies with pre-eclampsia at each week of gestation. The numerator was the number of pre-eclamptic p

Cardiac Remodeling in Preeclampsia: A Large Language Model-Assisted Meta-Analysis and Meta-Regression.

Preeclampsia is a hypertensive disorder of pregnancy associated with substantial maternal morbidity and long-term cardiovascular risk, but the consistency of echocardiographic remodeling remains unclear. We conducted a mega-meta-analysis of left ventricular function and geometry, enabled by Synthesa AI, a large language model-based suite of tools. A PROSPERO-registered review (CRD420251109103) searched PubMed, Scopus, and Embase without date limits. Synthesa AI screened more than 138,000 abstracts, extracted data, assessed risk of bias, and generated Bayesian analytic code, with all outputs va

Adverse pregnancy outcomes following COVID-19 infection or vaccination in active component U.S. military service women, 2021-2023.

Prior studies have found a higher risk of adverse pregnancy outcomes due to COVID-19 infection; however, recent literature documents few adverse impacts to younger and otherwise healthy populations, but with limited information about military members. The study population comprised active component service women with a singleton delivery between 2021 and 2023. Adverse pregnancy outcomes were evaluated by COVID-19 infection and vaccination history, as well as by demographics and pre-existing comorbidities. During the surveillance period, 39,355 active component U.S. service women had a singleto

Immune cell signatures and preeclampsia: Unveiling causal links through genome-wide association studies.

The immune-placental interaction could potentially contribute both causally and therapeutically to preeclampsia (PE). However, previous observational studies have not demonstrated a causal association between these 2 factors. Exploring the causal relationship between immune cells and PE provides genetic evidence that immune cells play a role in PE risk. Using a large dataset of genome-wide association studies, we conducted a bidirectional Mendelian randomization (MR) analysis to determine whether 731 immune cell signatures were correlated with PE, which included 4 types of immune signatures. Association between sweet food consumption and preeclampsia: Insights from an observational study and Mendelian randomization analysis.

BACKGROUND: Previous studies suggest links between sweet food consumption and metabolic conditions, but its role in preeclampsia (PE) remains unclear. This study combines a case‒control design with Mendelian randomization (MR) to explore the potential causal relationship between sweet food intake and PE. METHODS: This retrospective cohort study included 1146 pregnant women from Fujian Provincial Maternity and Children's Hospital (60 with PE, 1086 controls). Dietary habits, specifically sweet food consumption (≥3 times/week), were recorded pre-delivery. Logistic regression assessed the associa

Linking maternal and neonatal circulation in preeclampsia.

Hypertensive disorders of pregnancy, particularly early-onset preeclampsia (EOPE), are major causes of perinatal morbidity and mortality. Although impaired placentation has long been recognized as a key mechanism, increasing evidence highlights the contribution of maternal cardiovascular dysfunction. However, how maternal hemodynamics influences neonatal circulatory transition remains poorly understood. In this prospective study, mother-infant pairs from pregnancies complicated by EOPE were enrolled if the mother had undergone echocardiographic assessment within 3 wk before delivery and the ne

Pregnancy Outcomes in Patients with Type 1 Diabetes Using Continuous Glucose Monitoring.

OBJECTIVE: Continuous glucose monitoring (CGM) use among patients with type 1 diabetes mellitus (T1DM) has been associated with improved glycemic control, though improvement in non-glycemic outcomes is less consistent. We hypothesize that CGM use in patients with T1DM in a real-world clinical setting is associated with both improved glycemic and clinical outcomes. STUDY DESIGN: This was a retrospective cohort study of patients with T1DM receiving care at a large health system from 2016 to 2023. Primary outcomes included (1) glycemic control and (2) a composite comprising severe maternal morbi

Investigating preeclampsia risk factors and angiogenic profiles in low-screening area.

Preeclampsia (PE) is a complex pregnancy disorder that may cause adverse outcomes for mother and baby. Combining risk factors with clinical, laboratory, and ultrasonographic data can help identify women at risk. This study investigated the relationship between maternal risk factors, soluble fms-like tyro-sine kinase 1 (sFlt-1), placental growth factor (PlGF), their ratio, and pregnancy outcomes, involving 68 women with PE risk factors and 21 controls. There were no significant differences in the frequency of adverse outcomes (PE, foetal death, and infants with abnormal birth weight), sFlt-1, P

Immune regulation and lymphangiogenesis by lymphatic endothelial cells in the decidua in severe preeclampsia.

Lymphatic vasculature regulates lymphocyte trafficking and modulates adaptive immunity. Imbalanced immune cells at the maternal-fetal interface may contribute to severe preeclampsia (PE). Impaired placental lymphangiogenesis and immune dysregulation could contribute to PE but supporting evidence is limited. Here, we investigate the association between lymphangiogenesis and immune regulation in severe PE. First, we identified the presence of LYVE1-positive lymphatic vessels in the decidua, and then decidual lymphatic endothelial cells (dLECs) were isolated and cultured from chorioamniotic membr
