A prognostic model for use at labour onset to estimate the risk of severe fetal compromise at birth: a retrospective, model development and validation study.

BACKGROUND: Absence of reliable risk assessment and monitoring of fetal wellbeing during labour remains a major clinical challenge worldwide. Cardiotocography (CTG) is commonly used to monitor the fetal heart rate, but its misinterpretation or delays in clinical escalation contribute to poor birth outcomes. We aimed to develop a novel prognostic model for the prediction of severe fetal compromise at birth, combining known clinical risk factors with the first 20 min of CTG monitoring. METHODS: In this retrospective, model development and validation study, we used routinely collected, fully ano

Women's maternity and birth experiences in the PREVENT-PE trial: A qualitative investigation.

BACKGROUND: Pre-eclampsia (PE) continues to be a major risk to maternal and fetal health, complicating up to 5% of all pregnancies, with no proven pharmacological intervention to reduce its incidence at term. Screening for pre-eclampsia risk at 35-36 weeks' gestation has been associated with a 30% reduction in the incidence of the primary outcome of PE, in The PREVENT-PE Trial. METHODS: We aimed to understand women's maternity and birth experiences in The PREVENT-PE Trial. Semi-structured interviews were conducted with women (N = 80) from The PREVENT-PE Trial including those who were screened

Predictors of preeclampsia with severe features: A multi-center analysis of maternal and health system factors in Thai tertiary hospitals.

BACKGROUND: Preeclampsia (PE) with severe features threatens maternal and neonatal health. This study examined maternal and health-service factors associated with severe features and compared maternal and neonatal outcomes in Thailand. METHODS: This case-control study enrolled 156 postpartum women with PE (52 with and 104 without severe features) and 111 nurse-midwives in antenatal care, labor, and postpartum units across four Thai tertiary hospitals. Data were collected using postpartum and nurse-midwife questionnaires, and a maternal-neonatal health record form. Group differences were asses

[Thrombocytopenia and pregnancy : a practical approach].

Thrombocytopenia affects approximately 10% of pregnancies. It is most often benign (gestational thrombocytopenia), but some causes, particularly immune thrombocytopenia or preeclampsia, require specific management. The diagnostic approach and practical management depend on the medical history, background, gestational age, and severity of thrombocytopenia. A targeted work-up helps exclude serious causes, especially preeclampsia and other microangiopathies, that constitute a clinical emergency. Treatment, only required for severe or symptomatic thrombocytopenia, is tailored on the underlying eti

Association between adverse pregnancy outcomes and long-term postpartum blood pressure trajectories: a community-based cohort study.

BACKGROUND: Adverse pregnancy outcomes (APOs) increase the future risk of hypertension, but their impact on long-term postpartum blood pressure (BP) trajectories remains unclear. We aimed to characterize these trajectories and compare trajectory membership between women with and without APOs. METHODS: This community-based cohort study included 1,346 women with documented deliveries (2004-2020) from the Kailuan Study who underwent at least three postpartum health examinations between 2006 and 2022. APOs included hypertensive disorders of pregnancy (gestational hypertension and preeclampsia/ecl

Association of adverse pregnancy outcomes with coronary microvascular dysfunction: a systematic review and meta-analysis.

BACKGROUND: Adverse pregnancy outcomes (APOs) are recognized cardiovascular risk enhancers, yet their association with coronary microvascular dysfunction (CMD) has not been systematically quantified. OBJECTIVE: To evaluate the association between APOs and CMD and to provide pooled estimates of CMD prevalence and severity in women with APO history. METHODS: PubMed, Embase, Cochrane Library, and Web of Science were searched through December 2025. Observational studies evaluating coronary microvascular function using PET, Doppler echocardiography, stress CMR, or invasive physiology in women wit

Aspirin in preeclampsia prevention: a narrative Review of clinical and mechanistic evidence.

BACKGROUND: Low-dose aspirin reduces preterm preeclampsia, but its mechanisms remain uncertain because circulating concentrations of unchanged aspirin are substantially lower than those used in many experimental studies. This narrative review evaluates proposed mechanisms according to their clinical relevance and compatibility with prophylactic exposure. METHODS: Evidence concerning platelet inhibition, placentation, inflammatory resolution, angiogenic regulation and epigenetic modification was narratively reviewed, with particular attention to experimental concentrations, model limitations a

Race and ethnicity-specific differences in incidence of pre-eclampsia with severe features: Assessment per socioeconomic status.

In this cross-sectional study of 18,969,287 hospital deliveries identified in Healthcare Cost and Utilization Project's National Inpatient Sample from 2016 to 2022, Black pregnant people had higher incidence rates of developing pre-eclampsia with severe features compared to White pregnant people across all the socioeconomic strata. The effect sizes for Black pregnant people comparing to White pregnant people were similar and consistent across census-level household income and insurance status. These data suggest that the development of pre-eclampsia with severe features may be driven by a comb

Management of pregnancy in patients with Graves' disease secondary to alemtuzumab: a case series and literature review.

INTRODUCTION: Alemtuzumab treatment for multiple sclerosis frequently induces Graves' disease (GD), characterized by common coexistence of stimulating and blocking anti-thyrotropin receptor autoantibodies (TRAb), causing fluctuations of thyroid function. Alemtuzumab-induced TRAb-positive hypothyroidism is also common. These conditions are challenging to manage during pregnancy, considering the imperative necessity to maintain euthyroidism to minimize risks for both pregnancy and fetal/neonatal outcomes. METHODS: We reviewed the clinical course and therapeutic management of four pregnant women

Spontaneous retroperitoneal haematoma in pregnancy secondary to ruptured ovarian artery aneurysm: a case report.

Rupture of an ovarian artery aneurysm is a rare but potentially life-threatening cause of retroperitoneal haemorrhage during pregnancy. Most reported cases have occurred postpartum in multiparous women. This report describes an unusual case in a 26-year-old primigravid woman with symptom onset in the second trimester. She presented with recurrent left-sided abdominal pain from 21 + 6 weeks of gestation. Following repeated normal initial assessments, CT angiography at 31 + 6 weeks revealed a ruptured left ovarian artery aneurysm with retroperitoneal haemorrhage. She underwent a successful trans

A Rare Event Causing Blindness in an Unrecognised Pregnancy: A Case Report.

Purtscher-like retinopathy (PLR) is a rare, non-traumatic microembolic angiopathy characteristically associated with systemic endothelial conditions, including pre-eclampsia. While PLR typically exhibits a self-limiting course with a highly favourable visual prognosis, severe cases result in permanent, treatment-refractory blindness. We present a catastrophic, atypical case of bilateral PLR in an unrecognised pregnancy without any antenatal care. A 31-year-old primigravida without prior antenatal screening presented with a three-day history of sudden, bilateral central blurry vision and was ne

Maternal Delivery Outcomes Associated With Acute Sickle Cell Crisis: A National Inpatient Analysis.

BACKGROUND: Pregnancies complicated by sickle cell disease (SCD) carry substantial maternal and perinatal risk, but the specific association between acute sickle cell crisis (SCC) and adverse obstetric outcomes at delivery remains incompletely characterized. This study evaluated whether acute sickle cell crisis was associated with hypertensive disorders of pregnancy and cesarean delivery among delivery hospitalizations involving SCD. METHODS: We performed a retrospective cross-sectional analysis of the 2021 Healthcare Cost and Utilization Project National Inpatient Sample (HCUP-NIS). Delivery
