Association of quantitative sputum Candida load with respiratory dysbiosis and airway inflammation in suspected invasive pulmonary candidiasis.

UNLABELLED: The clinical significance of Candida species isolation from sputum remains ambiguous due to the colonization-infection dilemma. Integrating quantitative culture with microbiome profiling may clarify its role, yet systematic evidence is scarce. In this exploratory study (June 2023-June 2026), we enrolled 140 hospitalized patients suspected of invasive pulmonary candidiasis (IPC) and stratified them into possible IPC (n = 36), probable IPC (n = 71), and negative control (n = 33) groups based on clinical criteria. All underwent quantitative fungal culture of sputum and metagenomic nex

Prophylactic potential of Astragalus membranaceus extract in experimental pulmonary Candidiasis in Albino rats.

Invasive candidiasis is a fungal infection with potentially serious outcomes, requiring novel therapies to overcome drug resistance. Astragalus membranaceus (AM) dried root extract has been shown to have anti-inflammatory and immunomodulatory properties. This study aimed to examine the anticandidal potential of AM versus fluconazole and explore the mechanisms underlying the antifungal effect in a model of lung candidiasis. Four groups (n = 6, in each) of twenty-four mature male Wistar albino rats were created: non-infected control, Candida-infected, AM-treated, and fluconazole-treated groups.

Breaking the Mold: A Rare Case of Invasive Colonic Aspergillosis Causing Toxic Megacolon and Perforation in a Patient Without Classic Immunosuppression.

Invasive gastrointestinal aspergillosis is rare and typically occurs in immunocompromised hosts. We present a case of a 24-year-old man with intermittent corticosteroid use presenting with diarrhea and abdominal pain found to have toxic megacolon on imaging. He developed colonic necrosis and perforation requiring emergent colectomy. Histopathology revealed invasive Aspergillus with transmural necrosis. His course was complicated by septic shock, acute kidney injury requiring dialysis, and pulmonary candidiasis. Primary colonic aspergillosis in immunocompetent patients is exceedingly uncommon.

[Chinese expert consensus on the diagnosis and treatment of severe pneumonia in immunocompromised hosts].

The population of immunocompromised hosts has steadily increased in recent years. Clinical evidence indicates that these individuals are at a significantly higher risk of developing severe pneumonia following infection, with the mortality rates markedly higher than those observed in immunocompetent individuals. However, there is currently a lack of specific national or international guidelines or expert consensus addressing the management of severe pneumonia in this population. To address this unmet need, the Chinese Thoracic Society (CTS) of the Chinese Medical Association and the Oncology Re

Concurrent pulmonary candidiasis and tuberculosis in type 2 diabetes mellitus: immune pathogenesis and multidisciplinary management challenges: a case report.

BACKGROUND: Type 2 diabetes mellitus predisposes to opportunistic pulmonary infections. We report sequential invasive pulmonary candidiasis followed by tuberculoma in a patient with newly diagnosed type 2 diabetes mellitus and summarize diagnostic pitfalls and management lessons. CASE PRESENTATION: A 67-year-old Han Chinese man with no previously known diabetes presented with cough and severe hyperglycemia (random glucose 36.3 mmol/L; HbA1c 12.7%). Initial chest computed tomography showed right‑upper‑lobe consolidation. Bronchoalveolar lavage metagenomic next‑generation sequencing detected ab

Candida glabrata associated primary pleuro-pulmonary infections: A report of two cases.

Candida glabrata, a respiratory tract commensal, is an opportunistic pathogen that rarely causes respiratory disease. Primary pleuropulmonary infection caused by Candida glabrata without systemic involvement is extremely uncommon. We report two cases of proven Candida glabrata-associated pleuropulmonary infections in poorly controlled diabetics. A 51-year-old lady with a lung abscess had Candida glabrata identified from a biopsy specimen. Another 42-year-old man had necrotizing pneumonia and empyema due to Candida glabrata identified from repeated pleural fluid cultures. Both patients showed r

Development of a novel model for accurate prediction of secondary Candida pneumonia in patients with acute exacerbation of chronic obstructive pulmonary disease.

BACKGROUND: Given the increased risk factors such as the wide application of various dose forms of corticosteroids and broad-spectrum antibiotics in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) in recent years, the incidence of invasive Candida pneumonia secondary to AECOPD tends to increase. However, Candida infections secondary to AECOPD are often neglected in clinical practice, or even misdiagnosed as bacterial infections, resulting in disease deterioration due to delayed diagnosis. Knowing that early diagnosis and timely treatment can obviously improve

Tomographic presentation with miliary appearance in an immunocompromised patient with pulmonary candidiasis: A case report.

INTRODUCTION: Candida albicans infection shows a wide range of patterns on chest computed tomography, including ground glass opacities and consolidation, with the miliary pattern being the least frequent. The miliary pattern is not exclusive to tuberculosis. Fungal lung infections are rare and potentially fatal. Colonization by Candida albicans, associated with impaired immunity, requires early empirical antifungal therapy to improve survival. CASE REPORT: 48-year-old male patient with persistent productive cough, mucopurulent expectoration, dyspnea on moderate exertion, night sweats, elevate

Invasive candidiasis presenting bronchiectatic cavity as chest radiological findings: A case report.

Invasive candidiasis is rare but is associated with high mortality in immunocompromised or critically ill patients. Here, we present a case of a 55-year-old man with untreated diabetes who was diagnosed with coronavirus disease 2019 and subsequently developed invasive candidiasis. The patient presented with fever, tachycardia, and tachypnea. Chest computed tomography revealed multiple consolidations mainly distributed around the bronchovascular bundles with bronchiectatic cavity formation, which initially raised suspicion for invasive pulmonary aspergillosis. However, subsequent testing confir

Clinical impact of Candida respiratory tract colonization and acute lung infections in critically ill patients with COVID-19 pneumonia.

Coronavirus disease 2019 (COVID-19), which is attributable to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has been causing a worldwide health issue. Airways colonization by Candida spp. is prevalent among patients on automatic ventilation in intensive care units (ICUs). This research aimed to ascertain the risk factors and roles of Candida spp. respiratory tract colonization, and Candida lung infection during the progression of COVID-19 pneumonia in critically ill patients. In total, Candida spp. were recovered in 69 from 100 immunosuppressed patients with COVID-19. Bronchosc

Invasive Fungal Pneumonia in Immunocompromised Patients.

Fungal pneumonia is the most frequent presentation of invasive fungal infections (IFIs) in patients with hematologic malignancies (HM) and hematopoietic stem cell transplantation (HSCT) recipients. The most common causes include Aspergillus, Mucor, Fusarium, and Candida species. The high incidence and high morbidity and mortality rate of fungal pneumonias in HM/HSCT populations arise from severe immune dysfunction that may be caused by both the underlying disease and/or its treatment. CT is routinely used when pulmonary complications are suspected after HSCT. Appropriate image interpretation o
