AA and Pregnancy.

AA is a crucial fatty acid in pregnancy, especially for the growth and development of the fetus and its central nervous system (CNS). As already discussed in the previous chapters, AA regulates inflammation, immune response, and cell signaling. AA has a modulatory action on gene expression, serves as a mechanotransducer, and regulates cell membrane fluidity. By regulating cell membrane fluidity, AA modulates the expression and binding of several receptors located on the cell membrane. Eicosanoids derived from AA have both beneficial and harmful effects on pregnancy. For instance, (i) AA metabo

Effects of Mindfulness Therapy on Stress Biomarkers and Psychological Symptoms in Women With Recurrent Pregnancy Loss: A Randomized Clinical Trial.

Women with recurrent pregnancy loss (RPL) exhibit high levels of depression, anxiety, and stress, which negatively impact their quality of life. This randomized clinical trial evaluated the effects of an eight-week online Mindfulness-Based Intervention (MBI), with weekly one-hour sessions, on psychological and biological markers of stress in 35 women with RPL. Participants were randomly assigned to either the MBI or standard care (control group) and assessed before and after the intervention through salivary biomarkers (cortisol, alpha-amylase, and Immunoglobulin A-IgA) and the validated quest

Advanced Rheumatic Heart Disease Complicated by Pregnancy, Mitral Valve Disease, Pulmonary Hypertension, and Left Atrial Thrombus: A Multidisciplinary Case Report from Resource-Limited Settings.

INTRODUCTION: Rheumatic heart disease (RHD) remains a leading cause of cardiovascular morbidity and mortality among pregnant women in resource-limited settings. Pregnancy induces significant hemodynamic changes that can decompensate pre-existing valvular heart disease, leading to life-threatening complications for both mother and fetus. CASE PRESENTATION: A 27-year-old pregnant woman (gravida 6, para 5) at approximately 34-38 weeks presented with progressive shortness of breath, palpitations. Based on the modified World Health Organization (mWHO) classification, she was categorized as Class I

Narrative review: The role of maternal nutrition in recurrent miscarriage: From molecular pathways to clinical care gaps.

Recurrent miscarriage (RM), defined as three or more consecutive pregnancy losses before 24 weeks' gestation, represents a profound clinical and psychological challenge, affecting approximately 1-2% of couples. Despite standard investigation, up to 50% of cases remain unexplained, shifting focus towards modifiable environmental factors. Maternal nutrition, particularly micronutrient status, is a critical yet underexplored component influencing the biological pathways essential for early pregnancy success. This narrative review synthesises evidence on the role of nutrition in RM, focusing on mo

A 20-year follow-up of a patient with Klippel-Trenaunay syndrome: case report.

INTRODUCTION: Klippel-Trenaunay syndrome (KTS) is a rare congenital malformation characterized by a clinical triad: capillary malformations, varicose veins, and hypertrophy of soft tissues or bones in one or more limbs. Differential diagnosis from Parkes Weber syndrome is crucial; the primary distinguishing feature is the presence of arteriovenous fistulas. KTS is progressive. Management of KTS is predominantly conservative, involving lifelong elastic compression therapy and regular courses of venotonics. This case report presents the clinical follow-up of a patient with KTS over a period of m

Suspected Mesenteric Malperfusion in Acute Type A Aortic Dissection During Pregnancy: A Case Report and Review of Reperfusion-First Strategies.

Aortic dissection during pregnancy is rare but potentially catastrophic. Acute Type A aortic dissection generally requires urgent surgical repair. When visceral malperfusion is present, however, the optimal sequence of interventions may be complex, particularly when pregnancy and recent cesarean delivery increase operative risk. Endovascular reperfusion followed by delayed aortic repair has been proposed in selected patients, although evidence supporting this strategy in pregnancy remains limited. A 37-year-old woman at 31 weeks' gestation presented with chest and back pain. Initial CT pulmona

Pregnancy Complicated by Citrin Deficiency and Gestational Diabetes Mellitus: Perioperative Metabolic Management and Postoperative Analgesic Challenges Following Caesarean Delivery.

Citrin deficiency is a rare autosomal recessive metabolic disorder caused by pathogenic variants in the SLC25A13 gene. Pregnancy in affected women is uncommon and presents unique challenges because conventional obstetric management may conflict with metabolic requirements. The coexistence of gestational diabetes mellitus (GDM) introduces additional complexity. A 25-year-old, gravida 2 para 0+1 woman with genetically confirmed citrin deficiency (homozygous SLC25A13 c.1763G>A mutation) was managed jointly by obstetric and inherited metabolic disease (IMD) teams. Optimal cutoffs for the Montreal Cognitive Assessment among English and Spanish speaking Latinos.

Commonly applied cutoff scores of the Montreal Cognitive Assessment (MoCA) perform poorly among US Latinos. Prior studies relied on small samples, combining persons with mild cognitive impairment (MCI) and dementia into a single group, or failing to account for multiple intersecting demographic factors. We identified optimal MoCA cutoffs for MCI and dementia while addressing these limitations, cross-sectionally, among 1,780 English- and Spanish-speaking Latinos. Research diagnostic groups consisted of cognitively unimpaired (CU), MCI, and dementia. Area under the curve (AUC) values for CU vs.

Adjunctive screening and risk stratification performance of neutrophil-to-lymphocyte ratio for gestational diabetes mellitus: A systematic review and meta-analysis.

The neutrophil-to-lymphocyte ratio (NLR) is an inexpensive inflammatory marker with uncertain value for gestational diabetes mellitus (GDM) risk assessment. This systematic review and meta-analysis evaluated NLR for first-trimester risk stratification and second-trimester adjunctive screening of GDM. PubMed, Embase, Web of Science, and the Cochrane Library were searched from inception to April 15, 2026. Risk of bias was assessed using the Quality Assessment of Diagnostic Accuracy Studies 2 (QUADAS-2) tool. Sensitivity and specificity were pooled separately by trimester, and summary receiver op

Gestational exposure to organophosphate esters (OPEs) and adiposity during middle childhood in the MIREC Cohort.

Toxicological evidence suggests that organophosphate esters (OPEs) may disrupt metabolic processes involved in energy balance and adiposity, yet epidemiological findings remain inconsistent. Among 283 mother-child dyads in the Maternal-Infant Research on Environmental Chemicals (MIREC) cohort, we examined associations between gestational urinary OPE metabolites, including bis(2-chloroethyl) phosphate (BCEP), bis(1-chloro-2-propyl) phosphate (BCIPP), bis(1,3-dichloro-2-propyl) phosphate (BDCIPP), di-n-butyl phosphate (DNBP), and diphenyl phosphate (DPHP), measured at 6-13 and 16-21 weeks of ges

Strengthening gestational diabetes mellitus prevention, diagnosis, and care in Africa: evidence and policy priorities from a systematic review, meta-analysis, and bibliometric analysis.

Gestational diabetes mellitus (GDM) is an increasing public health challenge in Africa, contributing substantially to maternal and neonatal morbidity. Evidence from our systematic review, meta-analysis, and bibliometric analysis shows that inconsistent screening practices, reliance on selective rather than universal testing, and limited health system capacity contribute to underdiagnosis and poor pregnancy outcomes. GDM is associated with adverse maternal and neonatal complications and increases the long-term risk of type-2 diabetes for mothers and offspring. Our bibliometric findings reveal u

IMPACT OF GESTATIONAL DIABETES MELLITUS ON NEONATAL BIRTH WEIGHT, LENGTH, AND GESTATIONAL AGE: A RETROSPECTIVE CASE-CONTROL STUDY.

Gestational diabetes (GDM) in pregnant women leads to the development of macrosomia in newborns and also influences the characteristics of neonatal length development. It is worth noting that findings regarding the association between GDM and gestational age at birth vary, and there is no evidence to support treating GDM as an independent risk factor for determining gestational age at birth. The aim of this study was to determine the impact of GDM on neonatal physical parameters, including birth weight and length, as well as gestational age at birth. Data were collected using a standardized qu

Sex-informed interpretation of coronary artery calcium in primary prevention.

Coronary artery calcium (CAC) scoring is selectively used to refine cardiovascular risk assessment in primary prevention, but interpretation still relies largely on absolute thresholds applied similarly to women and men. This approach may overlook relevant sex-related differences in coronary atherosclerosis. Women tend to develop detectable CAC later in life and have lower absolute CAC values for a given age, while sex-related differences in coronary plaque composition may not be fully captured by calcium scoring alone. Consequently, a low absolute CAC score may still represent a high age- and
