Predictors of Shoulder Dystocia: A Retrospective Case-Control Study.

INTRODUCTION: Shoulder dystocia (SD) is rare, but it has serious consequences. Antepartum risk stratification for SD has demonstrated variability across populations, reflecting differences in assessment methodologies. METHODS: We conducted a retrospective analysis using a prospectively audited electronic database from a single center. All singleton vertex deliveries between January 2012 and December 2017 were reviewed. SD cases were identified by the recorded use of ancillary maneuvers during shoulder delivery. Case-control matching was informed by previously published odds ratios for key var

Application of the guidewire anchoring technique in lateral approach transforaminal endoscopic lumbar foraminoplasty: a propensity score-matched retrospective study.

OBJECTIVE: Safe and rapid bony localization for intervertebral foraminoplasty is essential in lateral transforaminal endoscopic lumbar discectomy (TELD). This study introduces a novel guidewire anchoring puncture technique and compares it with conventional puncture to evaluate operative efficiency, safety, and clinical outcomes. METHODS: A retrospective 1:1 propensity score-matched (PSM) analysis was performed on 60 patients with single-level L4/5 or L5/S1 disc herniation receiving TELD (anchoring group = 30, conventional group = 30). Baseline demographics, intraoperative metrics, neurologica

Rationale and protocol for the Time to Move Randomized Crossover Trial: morning versus evening time physical activity and CGM-assessed glucose levels in individuals with pregnancy hyperglycemia.

BACKGROUND: For most patients with pregnancy hyperglycemia, treatment includes lifestyle behavioral counseling for a healthy diet and physical activity (PA). Outside of pregnancy, emerging evidence suggests that the timing of PA (e.g., in the morning vs. evening) may modify its glucose-lowering effects. PA is an evidence-based, non-pharmacological strategy for managing glucose levels, and recommendations for PA timing could improve glucose levels in individuals with pregnancy hyperglycemia. OBJECTIVE: To describe the rationale and protocol of the Time to Move Randomized Crossover Trial, which

The award gap affecting international medical graduates in general practice training: results of a scoping review.

BACKGROUND: General practice in the UK is under significant pressure, resulting in a drive to increase training capacity. These additional training posts have historically been filled by international medical graduates (IMGs) who accounted for 50% of UK GP residents in 2025. There is however a significant 'award gap' (previously differential attainment) affecting IMGs in postgraduate GP training. AIM: To identify literature addressing the factors underlying the award gap. DESIGN & SETTING: The scoping review followed the process outlined in the Joanna Briggs Institute (JBI) guidance. Studies

A retrospective study on the efficacy and safety of anlotinib combined with limb salvage therapy for osteosarcoma after chemotherapy failure.

This study aimed to evaluate the efficacy and safety of anlotinib combined with limb salvage therapy in patients with osteosarcoma after chemotherapy failure. A retrospective study was conducted on 46 eligible osteosarcoma patients who failed chemotherapy, admitted to our hospital from October 2021 to December 2022. They were divided into 2 groups: 28 patients in the anlotinib combined with limb salvage surgery group (combined group) and 18 patients in the limb salvage surgery alone group (surgery group). The primary endpoint was progression-free survival (PFS); secondary endpoints included ob

Slugging-induced Mechanistic Coupling of Microcrystalline Cellulose-starch Disintegrants for Enhanced Tablet Disintegration.

Disintegrant performance in insoluble, poorly disintegrating matrices is strongly influenced by how the excipients are assembled at the granule level. This study investigated densified microcrystalline cellulose-starch (MCC-starch, 4:1) granular systems as direct compression disintegrants. In the screening stage, co-processed MCC-starch granules prepared by slugging at 10-190 MPa were incorporated into dibasic calcium phosphate (DCP) tablets at 10-15%, w/w. Disintegration behavior exhibited a strong dependence on slugging pressure, with low-pressure, less dense granules failing to disintegrate

Subcortical Bone Cysts at the Medial Meniscus Posterior Root Are Associated with Longer Symptom Duration but Not with Healing Status or Meniscal Extrusion After Root Repair.

Background and Objectives: Subcortical bone cysts at the tibial attachment of the medial meniscus posterior root (MMPR) may reflect chronic degenerative changes; however, their clinical significance in medial meniscus posterior root tear (MMPRT) remains unclear. This study assessed whether bone cysts are associated with symptom duration and whether their presence influences healing status or medial meniscus extrusion (MME) after transtibial pullout repair (TPR). Materials and Methods: Seventy-four consecutive patients who underwent arthroscopic TPR for MMPRT between January 2022 and December 2

Macrophage Proteomic Profiling Reveals Divergent TLR4-Dependent and -Independent Responses to Kdo2-Lipid A and Lipid IVa.

Macrophages harness pattern recognition receptors (PRRs) to detect conserved bacterial components and mount effective immune responses. Many Gram-negative bacteria modify their lipid A structures to limit recognition by Toll-like receptor 4 (TLR4) and cytosolic Caspase-11 lipopolysaccharide sensors. One common evasion strategy is to reduce the lipid A acylation state from hexa- to tetra-acylation. This alteration can limit binding to receptors and dampen subsequent immune signaling responses, yet the proteomic alterations associated with this altered immunogenicity remain incompletely understo

Association of Slope-Reducing Osteotomy With Return to Preinjury Level of Sport After Failed Anterior Cruciate Ligament Reconstruction in Elite Athletes.

BACKGROUND: Increased posterior tibial slope (PTS) has been identified as a major risk factor for failure after anterior cruciate ligament reconstruction (ACLR). In athletes with high PTS undergoing revision ACLR, slope-reducing osteotomy (SRO) has been proposed to reduce anterior cruciate ligament graft forces and improve outcomes, but data regarding return to sport in elite athletes remain limited. PURPOSE: To evaluate whether combining SRO with revision ACLR would allow elite athletes with increased PTS to return to their preinjury level of sport and, further, to assess clinical, functiona

Antibiotics as infrastructure: Labour, precarity and healthcare ecology in a Nepali industrial town.

In industrial work environments with limited labour protection and frequent injuries, medicines may function to sustain work capacity rather than facilitate recovery. Focusing on steel production in Nepal, we explored how the local healthcare ecology framed care pathways for steel workers. The analysis draws on concepts of pharmaceuticalisation, local healthcare ecology and the political economy of labour to examine how medicines-particularly antibiotics-enable continued productivity under exploitative work conditions. Ethnographic fieldwork in a steel factory included survey and qualitative i

Autophagy impairs the sensitivity of Ewing sarcoma cells to PARP inhibitors.

PURPOSE: PARP inhibitors (PARPi) proved effective in Ewing sarcoma cells in preclinical studies. In clinical evaluation, however, the PARPi olaparib failed to elicit substantial responses, suggesting an unknown mechanism of resistance to PARPi in Ewing sarcoma. Since autophagy has been identified as a PARPi resistance mechanism in other tumours, this study aimed at exploring the impact of autophagy on PARPi effectiveness in Ewing sarcoma cells. METHODS: Effects of the PARPi olaparib and veliparib were assessed by flow cytometry in the Ewing sarcoma cell lines WE-68 and SK-ES-1. Autophagy leve

Melatonin improves osteogenic differentiation in a high-glucose environment by activating NRF2 to promote autophagy through the regulation of cross-talk between macrophages and bone marrow mesenchymal stem cells.

BACKGROUND: Melatonin (MT) can regulate the osteogenic differentiation of bone marrow mesenchymal stem cells (BMSCs), but its effect on the osteogenic differentiation of BMSCs under high glucose (HG) conditions is unclear. Therefore, in this study, the effect of MT on the osteogenic differentiation of BMSCs under HG conditions was investigated. METHODS: A mouse model of diabetic osteoporosis (DOP) was induced by the intraperitoneal injection of streptozotocin (STZ), and macrophages or BMSCs were cultured with 25 mM glucose to construct an in vitro cell model. Different doses of MT were used t

Clavicular Distraction Osteoplasty With Sternoclavicular Reconstruction for a Medial Clavicle Fracture: A Case Report.

CASE: An 80-year-old man with osteoporosis sustained a complex, comminuted medial clavicle (MC) fracture that failed nonoperatively. A novel reconstruction involved MC resection, midclavicle distraction osteoplasty (DO) with iliac crest bone graft, and sternoclavicular (SC) joint reconstruction using a semitendinosus tendon allograft. By 1 year, the patient achieved successful healing and functional recovery. CONCLUSION: Patients with excessive MC bone loss may require a clavicular lengthening to approximate the MC to the manubrium. Restoring clavicular length through a DO in conjunction with

A reactive oxygen species-responsive diclofenac nanogel coating improves the anti-degeneration performance of bioprosthetic heart valves.

The growing burden of valvular heart disease has led to increased use of bioprosthetic heart valves (BHVs), yet limited durability remains a major challenge. Although calcification, chronic inflammation and mechanical injury are recognized contributors to BHVs degeneration, their relative contributions and interactions within implanted BHVs remain insufficiently resolved, partly because direct pathological evidence from long-term clinical explants is limited. Here, comparative pathological analysis of long-term failed BHVs and procedurally retrieved non-degenerated valves confirmed pronounced

Patellar Tendon Reconstruction After Failed Patellar Tendon Repair With a Concomitant Double-Bundle Posterior Cruciate Ligament Reconstruction and Medial Collateral Ligament Reconstruction.

BACKGROUND: Patellar tendon ruptures are a relatively uncommon injury that results from a contraction of the quadriceps against a fixed eccentric load during partial knee flexion. The medial collateral ligament (MCL) and posterior cruciate ligament (PCL) are frequently injured together, and their reconstitution should follow anatomic reconstruction techniques. INDICATIONS: In a previously failed patellar tendon repair, a reconstruction with an allograft is indicated due to poor tissue quality. The failed patellar tendon repair was further complicated by a chronic PCL tear and a chronic high-g

Revision total hip arthroplasty with limb shortening for symptomatic leg length discrepancy: survivorship and clinical outcomes at mean seven-year follow-up.

PURPOSE: This study evaluates the survivorship and clinical outcomes of revision total hip arthroplasty (THA) to treat symptomatic leg length discrepancy (LLD) via limb shortening. METHODS: A total of ten patients were included who underwent revision THA to treat LLD via limb shortening at a single arthroplasty center from January 1, 2000, to December 31, 2022, representing 0.4% (ten of 2763) of all revision THA during this period. Among these cases, six of ten (60.0%) were men, mean age at index surgery was 62 years (range, 39 to 81), and mean follow-up was 7.63 years (range, 3.51 to 15.73).

3D craniofacial generative model for surgical planning in mandibular reconstruction.

Mandibular reconstruction following segmental resection for oral tumors is a complex procedure necessitating precise restoration of both masticatory function and facial aesthetics. Current Computer-Assisted Surgery (CAS) workflows remain fragmented, relying on subjective manual mirroring for shape completion and labor-intensive, non-standardized CAD operations for fibula osteotomy planning. Furthermore, existing deep learning approaches predominantly address mandibular shape completion in isolation, failing to integrate surgical feasibility or predict postoperative soft-tissue outcomes. Anterior Deltoid to Biceps Transfer for Restoration of Elbow Flexion.

Major injuries to the infraclavicular or axillary region requiring axillary artery repair or reconstruction, a replantation carried out at the proximal third of the arm, or an infraclavicular brachial plexus palsy with failed nerve reconstruction pose a challenge to a reconstructive surgeon with only the shoulder remaining functional. We observed that patients with these injuries exhibited a 'trick' movement causing elbow flexion by abducting the shoulder beyond 90 degrees, which led us to use the anterior fibers of the deltoid extended with fascia lata graft as a donor to restore elbow flexio

Protocol for a prospective cohort study for the assessment of sarcopenia in gestational diabetes: the SiGnal-D study.

INTRODUCTION: Sarcopenia is characterised by loss of muscle mass and strength. Although ageing is the most likely risk factor of sarcopenia, sarcopenia is prevalent even in non-elderly people. Type 2 diabetes (T2D) is a risk factor for sarcopenia, as T2D shares with sarcopenia several aetiological factors. Meanwhile, gestational diabetes mellitus (GDM) is characterised by metabolic alterations that resemble those observed in T2D, including increased insulin resistance (present even in physiologic pregnancies). Hence, GDM presents two major risk factors for sarcopenia, that is, dysglycaemia and
