Maternal antibody transfer confounds type 1 diabetes screening in infancy: results from the French TRAKR cohort.

AIM: The optimal age to initiate islet autoantibody (IAb) screening in first-degree relatives of individuals with type 1 diabetes (T1D) remains uncertain. The TRAKR cohort prospectively characterized IAb patterns in newborns of parents with T1D to assess the interpretability of early-life screening. METHODS: We analyzed 148 children with available follow-up. Child blood samples were collected at birth and at 8, 18, 30, and 42 months, and parental samples during pregnancy. Insulin (IAA), GAD (GADA), IA-2 (IA-2A), and ZnT8 (ZnT8A) IAbs were measured using bridging ELISAs. Child-mother IAb speci

Small Molecules Prescribed On- and Off-Label for Thoracic Cancers During Pregnancy: A Combined VigiBase and Systematic Review Analysis.

BACKGROUND: Thoracic cancers during pregnancy are rare but increasingly encountered because of delayed childbearing and the rising incidence of cancer in younger individuals. Despite guideline recommendations, exposure to targeted small-molecule therapies occurs across all trimesters, but safety data remain limited. OBJECTIVE: To investigate the safety of targeted small-molecule therapies for thoracic cancers during pregnancy by combining VigiBase data with a systematic review. PATIENTS AND METHODS: We performed a pharmacovigilance disproportionality analysis based on the reporting odds rati

Blood and Cervical Immune Biomarkers Associated With HIV Susceptibility Are Compromised Rapidly After Sexual Trauma.

PROBLEM: Sexual violence compromises women's health and increases susceptibility to pathogens. Yet immunobiological consequences of acute sexual trauma remain incompletely understood. METHODS: We conducted a cross-sectional study among women without HIV, comparing women exposed to forced vaginal penetration in the past 4 days (Acute Cases, n = 10), within the past 12 weeks (Cases, n = 10) and never exposed (Controls, n = 12). Sixteen immune biomarkers were measured by ELISA in plasma and cervical swabs. Blood CD4 and CD8 T cells were quantified by flow cytometry. Group differences were analyz

Human chorionic gonadotropin levels of cesarean scar pregnancy compared with those of other ectopic pregnancies: a retrospective cohort study.

BACKGROUND: A cesarean scar pregnancy (CSP) is an abnormal implantation of an embryo within a previous cesarean section scar and considered a relatively rare form of ectopic pregnancy. We aimed to clarify the factors associated with CSP and evaluate whether differences in serum human chorionic gonadotropin (hCG) levels according to implantation site could contribute to early detection of CSP. METHODS: We investigated 27,528 freeze-thawed embryo transfer cycles performed at our hospital between January 2016 and March 2025. RESULTS: Forty-four patients were diagnosed with ectopic pregnancy: 26

Case Report: Treatment of subthreshold bipolar disorder in pregnancy: a report on the effectiveness of the Zamar Protocol® in three treatment-resistant depression cases.

INTRODUCTION: Subthreshold bipolar disorder (SBD), also referred to as unspecified bipolar disorder, is frequently encountered in patients with treatment-resistant depression. It presents with depressive and subthreshold manic and mixed symptoms. Sustained remission with full functional recovery is the aim of treatment in mood disorders as residual symptoms may cause disability, while relapse may be associated with increased morbidity or mortality. To date, evidence for achieving sustained remission in SBD is absent, with no studies reporting sustained remission outcomes. The only available ev

The Maternal Blood Transcriptome Reflects Molecular Signatures Associated With Large Offspring Syndrome and Other Adverse Pregnancy Outcomes in Cattle.

Large offspring syndrome (LOS) in cattle is a phenomenon characterized by fetal overgrowth that is largely associated with calves derived from in vitro produced embryos. Here it was tested whether occurrence of LOS as well as two other adverse pregnancy outcomes, stillbirth and abortion, could be predicted using changes in the maternal blood transcriptome early in gestation. Blood samples were collected from Holstein and crossbred heifers at 40-50 days of gestation following embryo transfer of in vitro produced embryos. A total of 203 samples were selected for RNA-sequencing and used to compar

Effects of prenatal early food and micronutrient supplementations on body size and cardiometabolic risk factors in the 15-year-old offspring - the MINIMat randomised trial.

BACKGROUND: Evidence on the effects of maternal nutrition during pregnancy on offspring cardiometabolic risk is mixed. We hypothesized that 15-year-olds, whose mothers were randomised to maternal early pregnancy protein-energy supplementation, are taller and have lower low-density lipoprotein cholesterol than controls. We also assessed effects of prenatal multiple micronutrient supplementation (MMS) on adolescent body size and cardiometabolic risk factors. METHODS: In MINIMat trial (Maternal and Infant Nutrition Interventions in Matlab; ISRCTN16581394) in rural Bangladesh, pregnant women were

A risk assessment model for gestational diabetes mellitus integrating maternal demographics and hematological indices in Thai women.

BACKGROUND: Universal screening for Gestational Diabetes Mellitus (GDM) is the gold standard, but it can be difficult and expensive in resource-limited settings. Many current prediction models rely on specialized tests that are not available in primary care. This study aimed to create a cost-effective risk assessment model using common maternal demographic and blood test data to help identify GDM risk in Thai pregnant women. METHODS: We conducted a cross-sectional study with 200 pregnant women (24-28 weeks of gestation) at a secondary care hospital in Southern Thailand. We analyzed routine an

The Evolving Paradigm of Autism: Folate Receptor Autoimmunity deranging Epigenetic Programming in fetal and postnatal Brain Development.

This review on autism provides a summary of the multiple genetic, epigenetic and environmental changes setting the stage for disturbed brain development due to aberrant synaptogenesis and corticogenesis during the prenatal and post-natal periods. The emphasis is placed on nonmendelian transmission driven by epigenetics. Folate deficiency appears to play a critical role in influencing many of the structural changes contributing to developmental deficits. Impaired passage of folate across the placental and the blood-brain barriers to the fetal and postnatal brain because of folate receptor autoa

The association between prenatal maternal selenium concentration and mental health in early childhood, a mother-child cohort study.

BACKGROUND: Adequate selenium (Se) status is important for biological mechanisms involved in cognitive functioning and mental health. However, there are few studies that have examined the association between prenatal maternal Se and mental health of preschool children in low-and middle-income countries. The primary aim of this study was to measure associations between maternal plasma Se concentration during pregnancy and child mental health at four years. The secondary aim was to assess the psychometric properties of the parent reported Nepalese version of the Strengths and Difficulties Questi

Diagnostic Terminology and Clinical Practice in Interstitial Ectopic Pregnancy: A Five-Year Clinical Audit.

Background Interstitial ectopic pregnancy is rare and carries risks of rupture and severe haemorrhage. Accurate terminology is important because interstitial and cornual pregnancy are often used interchangeably despite representing distinct implantation sites. The primary objective of this five-year audit was to assess compliance with anatomically precise diagnostic terminology. Secondary objectives evaluated ultrasound assessment, escalation of diagnostically uncertain findings, and documented individualisation of management. Methods We retrospectively audited confirmed cases diagnosed betwee

Narrative review: The role of maternal nutrition in recurrent miscarriage: From molecular pathways to clinical care gaps.

Recurrent miscarriage (RM), defined as three or more consecutive pregnancy losses before 24 weeks' gestation, represents a profound clinical and psychological challenge, affecting approximately 1-2% of couples. Despite standard investigation, up to 50% of cases remain unexplained, shifting focus towards modifiable environmental factors. Maternal nutrition, particularly micronutrient status, is a critical yet underexplored component influencing the biological pathways essential for early pregnancy success. This narrative review synthesises evidence on the role of nutrition in RM, focusing on mo

Detection of MOG-IgG and MOG-IgA in Maternal Breast Milk and Infant Blood in MOGAD: A Case Report.

BACKGROUND AND OBJECTIVES: Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) is an inflammatory demyelinating disorder of the CNS characterized by pathogenic MOG autoantibodies. Data regarding maternal-infant transfer of MOG autoantibodies, particularly in breast milk, are limited. We aimed to characterize maternal MOG autoantibodies longitudinally and assess their presence in maternal plasma, breast milk, and infant blood, as well as their clinical relevance in the infant. METHODS: MOG-specific IgG, IgA, and IgM, including selected subclass analyses, were assessed by ce
