Delayed versus Early Umbilical Cord Clamping in Preterm Multiple Gestation: A Randomized Controlled Trial.

OBJECTIVE: The objective of this study is to evaluate the effects of delayed cord clamping (DCC) on hematocrit at 6 hours and 6 weeks of age, hospital stay, and other clinical outcomes at discharge. STUDY DESIGN: This randomized controlled trial was conducted in a tertiary care center in Western India. Preterm multiple gestations were randomized to DCC at 60 seconds of birth or early cord clamping (ECC) within 30 seconds of birth. The primary outcome was hematocrit at 6 hours of age. RESULTS: A total of 76 mothers and 155 preterm infants of multiple gestation (73 twins and 3 triplets) were r

Cervical ectopic pregnancy at 22 weeks: A case with subsequent pregnancy outcome.

INTRODUCTION: Cervical ectopic pregnancies are rare ectopic pregnancies inherently associated with abnormal placentation. Fertility-sparing management of cervical ectopic pregnancy can have benefits for the patient, but such treatment strategies are less successful in the second trimester. CASE: We report a case of fertility-sparing management of a cervical ectopic pregnancy at 22 weeks' gestation with a successful subsequent pregnancy. A misdiagnosis of threatened abortion on first-trimester ultrasonography delayed accurate diagnosis of the cervical ectopic pregnancy until 20 weeks' gestatio

Drug-induced hyperglycemia in obstetrics: key offending drugs, mechanisms, and practical management.

INTRODUCTION: Drug-induced hyperglycemia is an important metabolic complication that may arise when medications disrupt insulin secretion, insulin sensitivity, or glucose homeostasis. In obstetric practice, several commonly used drugs, including glucocorticoids, progesterone, antihypertensive agents, tocolytic drugs, and immunosuppressants, may impair glycemic control to varying degrees, and prolonged exposure may even lead to drug-induced diabetes mellitus. These effects are particularly important in pregnant women with pregestational diabetes, gestational diabetes, obesity, or other risk fac

Multidimensional molecular profiling reveals decidual and circulating NK cell dynamics in early and late pregnancy.

BACKGROUND: Human pregnancy presents a unique physiological state that allows for growth of an antigenically dissimilar foetus. The immune system plays a key role in establishing and maintaining successful pregnancy, yet detailed understanding of immunological responses in pregnancy is lacking. METHODS: We defined the immunological landscape in pregnancy by screening >350 markers on NK cells, CD4+, CD8+ and γδ T cells, B cells and monocytes in third-trimester pregnancy peripheral blood, cord blood and decidua, and by performing single-cell RNA sequencing (scRNAseq), including 130 CITE-seq ant

Prenatal factors shaping adolescent cardiovascular function and cardiometabolic risk trajectories.

Cardiovascular disease remains the leading cause of mortality worldwide, with early risk markers, including elevated blood pressure, arterial stiffness, and adverse lipid profiles, often emerging during adolescence. These phenotypes are clinically important, as adolescent cardiovascular risk strongly predicts adult subclinical atherosclerosis and future cardiovascular events. Increasing evidence from developmental origins of health and disease (DOHaD) research suggests that cardiovascular outcomes in adolescence are associated, in part, with prenatal exposures. This review synthesizes current

Care of the Newborn and Cord During the Third Stage of Labour (Scientific Impact Paper No. 78).

The third stage of labour happens after the baby or babies are born and when the placenta (afterbirth) is delivered. This Scientific Impact Paper reviews new evidence related to clamping of the umbilical cord, and other interventions that impact the birthing woman and newborn during this part of labour. The paper summarises recent additional information on anatomy (structure of the circulation system) and physiology (how the system functions) of the newborn baby's circulation before the umbilical cord blood vessels close, whether naturally or after a clamp is placed on the umbilical cord. The

Higher PFAS Exposure Associated with Lower Aldosterone in Pregnant Women - the potential missing link with fetoplacental interaction? The Odense Child Cohort.

BACKGROUND: Perfluoroalkyl substances (PFAS) are endocrine disrupting chemicals that may interfere with steroid hormone synthesis. Aldosterone is an essential steroid hormone during pregnancy and an independent determinant of placental and birth weight. OBJECTIVE: To investigate associations between PFAS exposure in early pregnancy and 24hour urine (U-) and plasma (P-) aldosterone concentrations in late pregnancy. METHODS: In 467 pregnant women from Odense Child Cohort, serum concentrations of five PFAS - perfluorohexane sulfonic acid (PFHxS), perfluorooctane sulfonic acid (PFOS), perfluoroo

Primary prevention of maternal anaemia to prevent preterm delivery and other adverse outcomes (PANDA): a protocol for a double-blind placebo-controlled randomised trial of a daily iron supplement during pregnancy.

UNLABELLED: Iron deficiency anaemia (IDA) affects up to 30% of all pregnant women often causing symptoms including malaise and lethargy which can significantly impact daily life and well-being. Additionally, IDA is associated with serious complications, including an increased risk of bleeding, preterm birth, small for gestational age babies and rarely, but significantly, stillbirth. The most common current management approach reactively treats IDA once it is diagnosed, usually with an oral iron preparation such as ferrous sulphate or another ferrous salt. There have been calls to investigate a

Spatial Transcriptomics of Human Decidua Identifies Molecular Signatures in Recurrent Pregnancy Loss.

The human decidua establishes immune tolerance at the maternal-fetal interface and is essential for successful embryo implantation and development. Here, we conducted a spatial transcriptomic analysis of human decidua from early pregnancies in both healthy donors and patients with recurrent pregnancy loss (RPL). Our analysis revealed two distinct spatial domains, named implantation zone (IZ) and glandular-secretory zone (GZ), corresponding to the layers of decidua compacta and spongiosa, respectively. The decidual natural killer cell subset (dNK1) and the decidual macrophage subset (dM2), both

Targeted nanoparticles for placenta-specific drug delivery in pregnant rhesus macaques.

Rationale: To reduce the risks associated with systemic drug administration in pregnancy and create new therapies for placental dysfunction, we have previously developed targeted liposomes capable of selective delivery of payloads to the placentas of pregnant mice. In this study, we assessed whether these liposomes selectively accumulated in the placentas of nonhuman primates and evaluated the maternal systemic response to liposome infusion. Methods: Liposomes were produced using the thin-film method, decorated with the placental homing peptide CRGDKGPDC (iRGD) or non-targeting peptide ARALPSQ

Brain Volumes After Hypertensive Pregnancy and Postpartum Blood Pressure Management: A POP-HT Randomized Clinical Trial Imaging Substudy.

IMPORTANCE: Hypertensive pregnancy increases risk of cognitive decline, stroke, and dementia, especially after preeclampsia. Women with prior hypertensive pregnancy show lower brain volumes, but it was unknown whether early postpartum blood pressure optimization could alter these outcomes. OBJECTIVE: To evaluate whether an intervention designed to achieve better postpartum blood pressure control after a hypertensive pregnancy is associated with differences in brain volumes around 9 months post partum compared with usual care. DESIGN, SETTING, AND PARTICIPANTS: This randomized clinical trial,

Maternal copy number variations detected by noninvasive prenatal testing in Japanese women.

Non-invasive prenatal testing (NIPT) is a screening method that detects fetal chromosomal trisomies from cell-free DNA in maternal blood. Because NIPT uses whole-genome sequencing with next-generation sequencing for data processing, it can also detect maternal genomic information. Although most copy number variations (CNVs) are benign, some have been reported to be associated with pathological phenotypes and are attracting increasing attention. However, most CNV studies have been conducted in Western populations, and large-scale studies in Japanese cohorts remain scarce. This study represents

An Unbiased Molecular Characterization of Peripartum Cardiomyopathy Hearts Identifies Mast Cell Chymase as a New Diagnostic Candidate.

Peripartum cardiomyopathy (PPCM) is a rare form of acute heart failure that develops in women toward the end of pregnancy or early postpartum. No effective, specific treatment for PPCM is available and heart transplantation or mechanical circulatory support may be required in severe cases where drug treatment for heart failure is insufficient. The mechanisms through which the disease progresses are not well understood, and despite similar clinical characteristics to dilated cardiomyopathy of other etiologies (nonperipartum cardiomyopathy; NPCM) it is not known how the molecular remodeling diff
