An Increased Plasma D-Dimer Levels During Different Stages of Normal and Complicated Pregnancies Among Women in Bisha, Saudi Arabia.

INTRODUCTION: Hypercoagulability is one of the most reported state among Pregnant women. During pregnancy, the concentrations of D-dimer increase in a trimester-dependent- manner. The presence of gestational diabetes (GD) and gestational hypertension (GH) lead to further elevation in D-dimer levels. Pregnant women are at an increased risk of developing Venous thromboembolism (VTE), which can be fatal for both the mother and fetus. Elevated D-dimer levels during pregnancy could result in misdiagnosing other thrombotic diseases, emphasizing the need for further confirmatory testing for VTE. It i

Association between thrombophilic gene variants and thrombosis in the Iranian population: a systematic review and meta-analysis.

Thrombophilia is influenced by genetic variants, such as Factor V Leiden (FVL) and the prothrombin G20210A mutation. In clinical settings, assessing numerous genetic factors can lead to diagnostic errors and unnecessary treatments. This meta-analysis examines gene variants associated with thrombosis in the Iranian population, where their role in thrombotic disorders remains underexplored. A systematic literature search was performed across PubMed, Scopus, and Web of Science, targeting case-control studies published up to July 2025. Studies were included if they evaluated thrombophilia-related

Measuring and Managing Obesity in Pregnancy Using the Edmonton Obesity Staging System: A Scoping Review.

Emerging evidence and clinical practice guidelines have highlighted that obesity, defined as a chronic disease characterised by excess or dysfunctional adipose tissue, may not be accurately measured or understood by solely relying on body mass index (BMI) which is a measure of size not functionality. An alternative to BMI, as proposed in the Canadian Adult Obesity Management Guideline, is the use of the Edmonton Obesity Staging System (EOSS). While the EOSS has been evaluated in both adult and paediatric populations, pregnant individuals remain an underrepresented clinical group in its applica

A postpartum-adapted algorithm for safe management of suspected pulmonary embolism: a retrospective cohort study.

BACKGROUND: Pulmonary embolism presents with nonspecific clinical features, leading to excessive use of computed tomography pulmonary angiography. Although the YEARS rule-incorporating assessment of symptomatic deep vein thrombosis, hemoptysis, and whether pulmonary embolism was the most likely diagnosis-has been shown to help to reduce computed tomography pulmonary angiography exposure during pregnancy, its safety and effectiveness in the postpartum period remain to be elucidated. OBJECTIVE: To evaluate the safety and effectiveness of a postpartum-adapted YEARS algorithm incorporating elevat

Pregnancy in Women With Klippel-Trenaunay Syndrome Complicated by Uterine Vascular Malformation: A Case Report and Literature Review.

Klippel-Trenaunay syndrome (KTS) is a rare congenital vascular disorder characterized by the classical triad of cutaneous port-wine stain (capillary hemangioma), varicose veins, and hypertrophy of soft tissues and bones. KTS with uterine involvement is extremely uncommon. We present a 27-year-old pregnant woman with KTS complicated by uterine venous malformation. Comprehensive management was provided by a multidisciplinary team after 12 weeks gestation diagnosed with KTS. The patient wore full-length compression stockings and received low-molecular-weight heparin (LMWH) for thromboprophylaxis.

Strategies to Reduce Advanced Imaging in Antenatal Pulmonary Embolism Diagnostics.

IMPORTANCE: Advanced imaging to diagnose pulmonary embolism (PE) during pregnancy increases costs, ionizing radiation exposure, and the risk of overdiagnosis. D-dimer testing can exclude PE and compression ultrasonography can diagnose deep vein thrombosis (DVT), both reducing advanced imaging needs. How these 2 strategies are used in community PE diagnostics is unknown. OBJECTIVE: To analyze the use and efficiencies of D-dimer and ultrasonography for the diagnosis of antenatal PE. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study was performed in 21 US community hospitals fr

A multidisciplinary management model for severe gestational hypertriglyceridemia: using plasmapheresis to prevent pancreatitis in pregnancy.

Severe gestational hypertriglyceridemia can lead to life-threatening pancreatitis with high maternal and fetal mortality, yet there are no established guidelines for managing these patients. We report two cases of severe gestational hypertriglyceridemia managed through a multidisciplinary approach. With the collaboration of obstetricians, endocrinologists, and nephrologists, we utilized a novel plasmapheresis circuit-combining centrifugation and filtration plasmapheresis (CFPP)-and performed regular sessions via peripheral venous access without the use of plasma or albumin. This treatment effe

Clinical Characteristics and Impact of May-Thurner Syndrome in Patients With Proximal Deep Vein Thrombosis: Insights From the RIETE Registry.

May-Thurner syndrome (MTS) is an anatomical variant involving compression of the left iliac vein, predisposing to proximal deep vein thrombosis (DVT). Despite its clinical significance, population-level data remain limited. Using the RIETE (Registro Informatizado Enfermedad Trombo-Embólica) registry (2009-2024), we analyzed proximal DVT patients who underwent advanced imaging (CT-venography, contrast-venography, or MRI). Among 2872 patients with advanced imaging-confirmed proximal DVT, 124 (4.3%) had MTS. MTS patients were more likely to be female (78% vs 52%), younger (mean age 42 vs 62 years

The maternal hemostatic shift: Understanding VTE risk in pregnancy and postpartum.

Venous thromboembolism (VTE) remains a leading cause of maternal morbidity and mortality, with pregnancy increasing the risk of VTE four- to five-fold compared with the non-pregnant state and up to 60-fold in the postpartum period. The incidence of pregnancy-related VTE ranges from 0.5 to 2.0 per 1000 pregnancies, with deep vein thrombosis (DVT) and pulmonary embolism (PE) accounting for most cases. The heightened thrombotic risk during pregnancy and postpartum stems from physiological adaptations in hemostasis, creating a hypercoagulable state to mitigate hemorrhage risk at delivery. These ch
