Values and preferences towards the use of prophylactic low-molecular-weight heparin during pregnancy: a convergent mixed-methods secondary analysis of data from the decision analysis in shared decision making for thromboprophylaxis during pregnancy (DASH-TOP) study.

BACKGROUND: Venous thromboembolism (VTE) in pregnancy is a major cause of maternal morbidity and mortality, and the use of preventive low-molecular-weight heparin (LMWH) can be challenging. Clinical guidelines recommend eliciting pregnant individuals' preferences towards the use of daily injections of LMWH and discussing the best option through a shared decision-making (SDM) approach. Our aim was to identify individuals' preferences concerning each of the main clinical outcomes, and categorize attributes influencing the use of LMWH during pregnancy. METHODS: Design: Convergent mixed-methods.

[Women-specific aspects of venous thromboembolism].

Women have a higher lifetime risk of venous thromboembolism (VTE) than men. Hormone-associated risk factors such as pregnancy, contraception and hormone replacement therapy contribute significantly to this. Contraception with combined hormonal contraception increases the risk of VTE in young women, with the extent of the increase in risk being determined by the level of the estrogen dose and the progestin component. After hormone associated VTE, temporary anticoagulation is sufficient in many cases, provided there are no additional persistent risk factors. Affected women should be informed tha

Venous Thromboembolism in Pregnancy: A Review of Diagnosis, Management, and Prevention.

Venous thromboembolism (VTE), which includes deep vein thrombosis (DVT) and pulmonary embolism, is a leading cause of maternal morbidity and mortality worldwide. Physiological changes that occur in a normal pregnancy increase the risk for VTE by 4-5-fold in the antepartum period and 30-60-fold in the immediate postpartum period. Compressive ultrasonography is the diagnostic test of choice for deep vein thrombosis. Both ventilation/perfusion scanning and computed tomography pulmonary angiography can reliably diagnose pulmonary embolism. Anticoagulation for a minimum of 3 months, typically with

Mechanical thrombectomy for the management of iliofemoral deep venous thrombosis in the second trimester of pregnancy secondary to May-Thurner syndrome.

Treatment of pregnancy-related venous thromboembolism is limited by considerations of the health risks to both the patient and fetus. Anticoagulation is the cornerstone treatment for pregnancy-related venous thromboembolism; however, early thrombus removal may be preferred for prompt symptom resolution and to decrease the risk of post-thrombotic syndrome. We report the successful treatment of a patient in the second trimester of pregnancy with symptomatic iliofemoral deep venous thrombosis and May-Thurner syndrome using percutaneous mechanical thrombectomy.

Advances in Thromboprophylaxis for High-Risk Pregnancies: A Comprehensive Review of Current Strategies and Emerging Approaches.

Thrombosis during pregnancy poses a significant clinical challenge due to its potential for severe maternal and fetal complications. The incidence of thromboembolic events in pregnant women is heightened by pregnancy-associated hypercoagulability, venous stasis, and endothelial changes, all of which contribute to an elevated risk. Effective thromboprophylaxis is essential to mitigate these risks and improve outcomes for both mother and child. This review provides a comprehensive evaluation of current thromboprophylaxis strategies, including pharmacologic interventions such as low-molecular-wei

Anxiety in patients with venous thromboembolism: quantification and risk factors in a prospective cohort study.

BACKGROUND: Patients with venous thromboembolism (VTE) are at risk of psychological consequences. However, as opposed to physical sequelae of VTE, mental health issues are understudied. OBJECTIVES: To assess anxiety after VTE and investigate associated clinical characteristics. METHODS: We conducted a prospective cohort study, including patients with acute deep vein thrombosis (DVT) and/or pulmonary embolism. Patients with cancer, pregnancy, or puerperium were excluded. Anxiety was assessed with the Patient-Reported Outcome Measurement Information System short form 8a. Standardized T-scores

Hereditary Angioedema in Pregnancy Complicated by Upper Extremity Deep Venous Thrombosis: A Case Report.

Hereditary angioedema (HAE) is a rare disorder that causes episodes of angioedema due to a mutation in the C1 esterase inhibitor gene (C1-INH). Complications of HAE include intestinal obstruction, asphyxiation, and venous thromboembolism (VTE). In this case, we report a 34-year-old G4P2011 female with HAE at 24 weeks gestation presenting with acute right upper extremity pain and swelling following a peripherally inserted central catheter (PICC) line for HAE treatment infusion, revealing a right upper extremity VTE. Early treatment with Lovenox, PICC line removal, and continuation of HAE therap

Examining postpartum anticoagulation practices: An international survey of healthcare providers.

BACKGROUND: The risk of venous thromboembolism (VTE) is 15 to 35-fold higher in the postpartum period compared to non-pregnant individuals. Clinical practice guidelines recommend the use of postpartum thromboprophylaxis with low molecular weight heparin (LMWH) for 6 weeks in individuals at high risk of developing VTE. However, a marked reduction in the risk of VTE risk occurs beyond the third week of the postpartum period. OBJECTIVE: We sought to characterize practice patterns of clinicians who manage postpartum individuals at high risk of VTE. METHODS: We conducted a cross-sectional study u

Clinical implications of genetic testing for congenital protein C deficiency in pregnancy.

Congenital protein C (PC) deficiency is a mostly autosomal dominant hereditary thrombophilia associated with early onset arterial and venous thrombotic diseases. In newborns, PC deficiency results in severe complications such as cerebral hemorrhage, cerebral infarction, and purpura fulminans, leading to death in some cases. We report two cases of deep vein thrombosis diagnosed during pregnancy that prompted genetic testing confirming definitive congenital PC deficiency. One patient with deep vein thrombosis at 30 weeks of gestation underwent anticoagulation therapy with the placement of an inf

The Basic Principles of Pathophysiology of Venous Thrombosis.

The past few decades have brought tremendous insight into the molecular and pathophysiological mechanisms responsible for thrombus generation. For a clinician, it is usually sufficient to explain the incident of deep vein thrombosis (DVT) with provoking factors such as trauma with vascular injury, immobilization, hormonal factors, or inherited or acquired coagulation defects. About half of DVTs are, however, lacking such triggers and are called unprovoked. Venous stasis and hypoxia at the valve sinus level may start a chain of reactions. The concept of immunothrombosis has added a new dimensio

"Inpatient pharmacological thromboprophylaxis in the antepartum period: an argument for universal thromboprophylaxis".

Venous thromboembolism (VTE), a largely preventable condition, accounts for almost 15% of maternal mortalities. The physiologic changes of pregnancy, including quantitative changes in coagulation factors and compression of vasculature by the gravid uterus, cause an increase in risk of VTE, including deep vein thromboembolism (DVT), pulmonary embolism, and stroke (CVA). Long term antepartum admission for preeclampsia, preterm prelabor rupture of membranes (PPROM) or other high-risk pregnancy needs present additional risk factors for VTE due to the patient's medical condition and their inpatient

Evaluation of risk factors for postpartum cerebral venous sinus thrombosis, a multicenter retrospective observational study.

The postpartum period is a well-defined risk factor for cerebral venous sinus thrombosis (CVST). However, it is unclear which patients are at risk for CVST in the postpartum period. Thus, determining some CVST risk factors in postpartum patients may be useful for preventing the disorder in this population. Previous studies have shown that preeclampsia (PE) is a risk factor for pulmonary thromboembolism and deep vein thrombosis, but whether it is related to postpartum CVST has not yet been evaluated. This study aimed to determine if this relationship exists. This study was a case-control study

Risk of venous thromboembolic complications in pregnant trauma patients: A matched cohort study.

BACKGROUND: Trauma and pregnancy are both risk factors for venous thromboembolism (VTE). We hypothesized that pregnant blunt trauma patients would have a higher incidence of VTE complications compared with matched nonpregnant females. METHODS: We conducted a retrospective cohort study using National Trauma Data Bank data from 2017 to 2022. Female patients with blunt mechanism, age between 15 and 50 years old, were eligible for inclusion. Patients who presented as transfers, hospitalized for less than 72 h, discharged against medical advice, injury severity score <9, or abbreviated injury scal

Risk Factors and Outcomes of Acute Pulmonary Embolism in African Patients: A Systematic Review.

Pulmonary embolism is a common cause of morbidity and mortality. Numerous risk factors have been identified that predispose patients to this disease. This study aims to identify these risk factors and the possible outcomes (recovery or mortality) after receiving treatment from any hospital. Healthcare is expensive in Africa, hence hindering its easy accessibility.  PubMed, Scopus, and African Journals Online were searched from the database inception to October 2024 to identify relevant studies. A total of 719 articles were identified, for which 172 duplicate articles were removed. After screen

Selective Fetal Growth Restriction Leading to Cerebral Injury in Monochorionic Twins: A Case Report.

Monochorionic twin pregnancies carry a risk of perinatal complications due to shared placental anastomoses, which can cause uneven blood distribution and lead to conditions like selective fetal growth restriction (sFGR). This case describes a monochorionic pregnancy complicated by preeclampsia and late-onset sFGR of twin B. Labor was prematurely induced and a 45% weight discordance between the twins was confirmed. Twin A adapted well to extrauterine life, but a routine cerebral ultrasound on the second day revealed a periventricular venous infarction. Subsequent brain magnetic resonance imagin
