Value of colposcopy in the diagnosis of candidiasis in patients with vulvodynia.

OBJECTIVE: To evaluate the value of colposcopy in the diagnosis of chronic vulvar candidiasis in patients presenting with vulvodynia. STUDY DESIGN: A prospective study of 460 patients presenting with vulvodynia over a 24-month period was undertaken. All patients underwent colposcopy of the vulva, which was regarded as positive if acetowhite lesions with central clearing with or without petechiae were present. Vulvar skin scrapings were taken for Candida culture on all patients. Those patients with positive colposcopic findings were treated with long-term oral antimycotic therapy irrespective

Vulvar disease pearls.

The vulva is a neglected area of the skin, hidden away by ignorance, embarrassment, and taboo with the result that women with vulvar diseases, desperate for help, suffer needlessly and waste millions of dollars on inadequate or inappropriate treatment. As the skin experts, dermatologists are vital for vulvar care. This article covers a range of subjects from normal anatomy to atypical presentations of common infections, such as candidiasis and herpes simplex virus. Tips are presented on the management of common vulvar dermatoses and there is a brief overview of pearls in diagnosis and therapy.

Candida meningitis in a suspected immunosuppressive patient--a case report.

Meningitis due to fungal agents represents an AIDS-defining event and occurs typically with very low CD4+ lymphocyte count. Candida meningitis is still a rare clinical condition, although it is becoming frequently reported in the background of immune suppressive states such as: drug addicts, cancer patients, organ transplant recipients and HIV/AIDS patients. In this report we highlight a case of candida meningitis, in a 25- year old female patient. She presented with vulva swelling, vaginal discharge and fever, with rapid progression to tonic-clonic convulsions and loss of consciousness. She f

A simplified technique for evaluating the adherence of yeasts to human vaginal epithelial cells.

Thousands of women all over the world annually suffer of vulvovaginal candidiasis (VVC), an infection caused by yeasts, which mainly affect the mucosa of the vulva and vagina. The adherence of the yeasts to the mucosae is an essential step for colonization and predisposes the start of the infectious process. In this study, a technique capable of evaluating the adhesion of yeasts to human vaginal epithelial cells (HVEC) was employed. Twenty-five vaginal yeast isolates (10 Candida albicans, nine C. glabrata, two C. parapsilosis, one C. tropicalis, two Saccharomyces cerevisiae, and one Trichospor

Unusual location of vulvar basal cell carcinoma.

BACKGROUND: Basal cell carcinoma (BCC) is a rare tumor of the vulva; it accounts for approximately 2% to 4% of vulvar cancer. CASE: A 73-year-old woman was referred because of a 2-cm ulcerative lesion on her perineum, near the anus. Punch biopsy showed BCC. She underwent wide local excision of tumor without any damage to the anal sphincter. The postoperative period was uneventful and the cosmetic results were good. CONCLUSIONS: BCC most commonly occurs in sun-exposed areas, but it may be seen in rare locations, such as in perineum. In our case, chronic irritation secondary to long-term chron

Effects of intradermal foot and forearm capsaicin injections in normal and vulvodynia-afflicted women.

Cutaneous response to capsaicin has been used to assess central sensitization in pain research. This study compared the response to intradermal capsaicin in the forearm and foot of vulvar vestibulitis (vestibulodynia)-afflicted cases and controls. We hypothesized that cases will experience greater spontaneous pain, larger cutaneous areas of punctate hyperalgesia and dynamic allodynia, and greater vascular flow than controls. We also hypothesized enhanced post-injection pain in the foot compared to the forearm based on dermatome proximity of the foot and vulva. Methods. Ten vulvar vestibulitis

Vulval itch.

BACKGROUND: Vulval itch is common. Patients presenting with this symptom can have a long history involving visits to several general practitioners. Self diagnosis as thrush is common, and inappropriate use of over-the-counter antifungal preparations can lead to further irritation and distress. Excoriation, rubbing, maceration, secondary infection and the effects of topical applications frequently complicate matters. OBJECTIVE: This article identifies the common causes of vulval itch in adults and children, and highlights key features of the diagnosis and management of these conditions. Vulval

Isolated vulval splitting--is this normal or pathological?

Vulval splitting may be seen in association with clinically apparent dermatological disease but occasionally occurs in patients presenting with dyspareunia only. These patients usually have normal-looking vulvas on examination, apart from splitting. They are usually diagnosed as vulval vestibulitis. We have biopsied several patients with isolated vulval splitting with surprising results. Between the months of October 2000 and April 2002, 310 new patients were seen in the vulval clinic at Hope Hospital, which is a tertiary referral centre. Among these were nine cases of isolated vulval splittin

[Multifocal mycoses in women: prevalence, species characteristics, and some intraspecific features].

The aim of this study was to detect fungi in three ontocenoses of 947 women (aged 20-45). The prevalence of multifocal fungal infections in women varied from 62.2 +/- 2.99% to 81.0 +/- 3.92% in different ontocenoses. They assumed the following mean values: 38.4 +/- 1.77% for the vagina, 42.2 +/- 1.80% for the oral cavity, and 25.0 +/- 1.58% for the anus. The clinical manifestations of sexual organs inflammations were statistically significantly (p < 0.01) more frequent in women infected with fungi than in those without fungi in the vaginal ontocenosis. The highest convergence of the fungi pres

Management of common vulval conditions.

Community-based surveys indicate that about a fifth of women have significant vulval symptoms lasting over three months at some time in their lives. Common causes of itch or pain are dermatitis, recurrent candidiasis and the recently recognised pain syndromes--vulvar vestibular syndrome and dysaesthetic vulvodynia. Diagnosis is usually apparent after a thorough history and examination, although conditions commonly coexist and are complicated by prior treatment. Skin lesions not responding to treatment require biopsy. Treatment aims to control symptoms rather than to cure; avoiding soaps and ot

Vulvovaginal carriage of yeasts other than Candida albicans.

AIMS: We investigated the spectrum of yeasts isolated, and compared the epidemiological and laboratory characteristics of women carrying vulvovaginal Candida albicans with those carrying yeasts other than C. albicans. METHOD: Between April and June 2001, 5802 consecutively received genital swabs from women were plated onto Candida ID chromogenic media (BioMerieux). Blue colonies were reported as C. albicans; all other colonies (white and pink) were identified to species level using the Vitek YBC card (BioMerieux). In vitro susceptibility to amphotericin (AMB), fluconazole (FLU), itraconazole

Vulvar disease.

Unique embryologic and immunologic aspects of the vulva contribute to the diagnostic and therapeutic challenges of managing vulvar problems. Individual variations in care of the genital region, defined by personal and societal "norms," may at times exacerbate vulvar problems. Three dimensions are considered in the evaluation of a vulvar problem: 1) lesion type, 2) lesion location, and 3) associated systemic and laboratory findings. This review of vulvar disease highlights a number of common and problematic vulvar conditions. Treatment options for vulvar conditions are covered with an expanded

Characterization of women with a history of recurrent vulvovaginal candidosis.

BACKGROUND: To characterize history, signs, and symptoms in women with a history of recurrent vulvovaginal candidosis (RVVC) and who had consulted with symptoms generally associated with the condition. METHODS: Eighty-three women with a history consistent with RVVC were interviewed regarding 32 parameters and 10 signs found at the clinical examination were noted. Candida cultures were made from the introitus and the posterior vaginal fornix. RESULTS: Only in a few of the 43 women with and the 40 without a positive yeast culture could any of the many etiological factors that have been associa

Lichen Sclerosus and Other Conditions Mimicking Vulvovaginal Candidiasis.

For many women with chronic vulvovaginal symptoms, overdiagnosis of vulvovaginal candiasis (VVC) is an unfortunate tendency. In women with chronic vulvar itching or burning, a vulvar non-neoplastic epithelial disorder is a relatively frequent diagnosis. Although controversy persists about the nomenclature for these disorders, there seems to be a relatively clear consensus with lichen sclerosus. This chronic inflammatory skin disease affects primarily the female vulvar and perianal areas. Left untreated, it may lead to chronic scarring of the vulva with an associated loss of architecture.
