Antiphospholipid syndrome: epidemiology and outcomes in the evolving criteria era.

Antiphospholipid syndrome (APS) is a heterogeneous autoimmune thrombo-inflammatory disorder characterized by thrombosis, pregnancy morbidity and an expanding spectrum of microvascular, cardiac, haematological and other systemic manifestations. Although research has provided considerable insights into the pathogenesis and clinical phenotypes of APS, its epidemiology remains difficult to define because estimates depend on case definitions, antibody testing, repeat-testing requirements, testing practices and data sources. Population-based studies suggest that APS is rare, and catastrophic APS is

Graded Epidural Anaesthesia for Caesarean Section in the Presence of an Untreated Spetzler-Martin Grade V Cerebral Arteriovenous Malformation With Radiological Progression Over Six Years: A Case Report.

Cerebral arteriovenous malformations (AVMs) are rare during pregnancy, when the haemodynamic and hormonal changes of gestation may increase the risk of rupture. Lesions that are large, located in eloquent regions, and drained by deep venous channels represent a surgically high-risk subtype, carry substantial surgical morbidity, and are frequently managed conservatively when unruptured. We describe a 33-year-old G3P1L1A1 parturient at 36 weeks and six days of gestation with an untreated right parieto-temporo-occipital AVM, who declined intervention across three encounters and underwent an elect

Prenatal Diagnosis of an Isolated Persistent Left Superior Vena Cava With Absent Right Superior Vena Cava: A Case Report.

Persistent left superior vena cava (PLSVC) with absent right superior vena cava (RSVC) is a rare congenital venous anomaly resulting from the failure of the left anterior cardinal vein to regress. Although often asymptomatic in isolated cases, prenatal diagnosis has clinical benefits, including the exclusion of coexisting congenital heart disease (CHD) and awareness of possible complications during central venous access or pacemaker implantation. We report the case of a 34-year-old G2P1 woman who underwent a fetal echocardiography at 34 weeks of gestation. Examination of the four-chamber view

Recurrent Left Abducens Nerve Palsy and Idiopathic Intracranial Hypertension During Pregnancy: A Diagnostic Challenge.

Idiopathic intracranial hypertension (IIH) is characterized by elevated intracranial pressure without an identifiable mass lesion, hydrocephalus, cerebral venous sinus thrombosis, or abnormal cerebrospinal fluid composition. Diagnosis during pregnancy can be challenging because headache and visual symptoms overlap with several neurologic and obstetric disorders. We report a previously healthy 20-year-old primigravida with a body mass index of approximately 33 kg/m² who presented at 17 weeks' gestation with intermittent headache, photophobia, blurred vision, horizontal diplopia, nasal deviation

Optimising gravimetric method interpretation using population-specific blood densities among Indonesian pregnant women: A cross-sectional study.

BACKGROUND: The diagnosis of postpartum haemorrhage (PPH) depends heavily on accurate and timely blood loss assessment, yet no gold standard assessment method is available. Gravimetric methods are recommended for low-resource settings, but they require an accurate blood density value for interpretation. Currently, there are no data on the blood density of pregnant or labour women. OBJECTIVE: This study measured the blood density of Indonesian pregnant women to improve the reliability of the gravimetric results in estimating postpartum blood loss. METHODS: This cross-sectional study was condu

Acute Abdominal Pain in Pregnancy Revealing a Wandering Spleen With Infarction: A Case Report.

INTRODUCTION AND IMPORTANCE: Wandering spleen (WS) is a rare condition caused by the absence or laxity of splenic suspensory ligaments, predisposing the spleen to displacement and potential complications. While torsion is the most commonly reported issue, infarction due to vascular compromise is a serious and rare complication, particularly in pregnancy. We present a case of splenic infarction in a WS during pregnancy, emphasizing diagnostic challenges and management strategies. CASE PRESENTATION: A 19-year-old primigravida at 5 months of gestation presented with progressively worsening abdom

The influence of thrombophilia and low molecular weight heparin on D-Dimer levels in women with high-risk pregnancy.

INTRODUCTION: Pregnancy is a hypercoagulable state that raises the risk of venous thromboembolism VTE), particularly in women with thrombophilia. D-Dimer levels increase during the activation of the coagulation cascade and are used to exclude VTE in non-pregnant women. This study aimed to analyze the influence of thrombophilia and the use of low molecular weight heparin (LMWH) on D-dimer levels in pregnant women. METHODS: We retrospectively analyzed data from pregnant women presenting at the hemostaseology outpatient clinic of the University Hospital of Leipzig between July 2016 and June 2023

Ovarian vein thrombophlebitis as a complication of second trimester abortion.

Ovarian vein thrombophlebitis (OVT) is a rare but serious complication, most often described postpartum but also possible after abortion or pelvic surgery. It represents part of a spectrum that includes ovarian vein thrombosis, OVT and septic pelvic thrombophlebitis (SPT). Clinical features are non-specific, with pelvic pain and persistent fever often mimicking endometritis, urinary tract infection or other abdominal conditions, which contributes to delayed diagnosis. Imaging, particularly contrast-enhanced CT, is crucial for confirmation. We report the case of a woman in her 20s who developed

Maternal antenatal depression and offspring DNA methylation.

OBJECTIVE: Research on the link between antenatal depression and alterations in offspring DNA methylation is sparse and inconsistent. This study aimed to provide a robust and rigorous test of the association between maternal antenatal depression and offspring DNA methylation in neonatal and middle childhood (8-10 years) periods. METHODS: Moderate to severe maternal antenatal depression was identified via a combination of diagnosis codes from outpatient and inpatient encounters during pregnancy and self-reported symptom severity on birth certificates. Offspring DNA methylation was quantified f

Cardiovascular Complications During Delivery Hospitalizations in Patients With Infective Endocarditis.

Infective endocarditis (IE) in pregnancy is a rare but serious infection that increases morbidity and mortality. We aimed to assess the risk factors and outcomes in pregnant patients with IE as compared to pregnancy without IE. The National Inpatient Sample was used to identify hospitalizations for delivery from 2011 to 2020. Propensity score matching was performed to study the association of IE with the primary outcomes of in-hospital medical and obstetric complications. A total of 37,482,206 weighted delivery hospitalizations in women ≥18 years were identified, of which 2020 patients had IE.

First-Trimester Serum Omentin Levels as a Predictor of Gestational Diabetes Mellitus.

BACKGROUND: Gestational diabetes mellitus (GDM) is a frequent metabolic complication of pregnancy associated with significant maternal and neonatal morbidity. Its prevalence is notably higher among South Asian women, likely due to increased baseline insulin resistance and genetic predisposition. Conventional screening using the oral glucose tolerance test (OGTT), typically performed at 24-28 weeks of gestation, often identifies GDM after metabolic alterations are already established, thereby limiting opportunities for early intervention. This has led to growing interest in identifying reliable

Hemodynamic effects of umbilical cord milking versus early cord clamping in nonvigorous intrauterine growth restricted neonates: a randomized controlled trial.

UNLABELLED: The effect of placental transfusion on the nonvigorous intrauterine growth restricted (IUGR) neonates with pre-existing subclinical myocardial dysfunction and altered cerebral hemodynamics is largely unknown. The present study was planned to evaluate the effect of umbilical cord milking (UCM) on left ventricular output (LVO) of nonvigorous IUGR neonates of > 28-weeks' gestation. In the present randomized control trial, 74 nonvigorous IUGR neonates were randomly allocated to UCM (4-times squeezing of 20-cm intact cord, n = 37) and ECC (cord clamping within 30 s, n = 37). The primary

Recurrent Pregnancy Loss Despite Therapeutic Anticoagulation in a Patient With May-Thurner Syndrome: A Case Report.

Pregnancy is a hypercoagulable state associated with an increased risk of venous thromboembolism. May-Thurner syndrome (MTS), an anatomic compression of the left common iliac vein by the right common iliac artery, predisposes affected individuals to left-sided deep vein thrombosis (DVT) and is often underrecognized in pregnant patients. While maternal thrombotic complications related to MTS have been described, its potential impact on fetal outcomes remains poorly characterized. We report the case of a 29-year-old gravida 7, para 2 woman with confirmed MTS and recurrent pregnancy-associated le

During pregnancy, anticoagulant effect of low molecular weight heparin can be determined with both thrombelastography and anti-factor Xa activity, or not?

Venous thromboembolism is a common cause of maternal mortality. Many women need regional anaesthesia during vaginal delivery or caesarean section. However, the risk of epidural or spinal haematoma is increased if regional anaesthesia is administered during ongoing thromboprophylaxis with low molecular weight heparin (LMWH). The aims of this pilot study are to evaluate the anticoagulant effect of LMWH during pregnancy by thrombelastography and anti-factor Xa (FXa) activity and their correlations. Sixteen pregnant women were administered dalteparin or tinzaparin from early pregnancy until six we
