Associations of Urinary Triclosan Concentrations Across Gestation and Childhood with the Adolescent Gut Microbiome.

Research on the influence of repeated exposure to antimicrobial chemicals, such as triclosan, on the microbiome is lacking. The goal of this study was to elucidate the associations of time-varying urinary triclosan concentrations with the adolescent gut microbiome. We used data from a prospective cohort based in Cincinnati, OH (enrolled 2003-2006, n = 146), to assess the time-varying associations of urinary triclosan concentrations with adolescent (age ∼ 12 years) gut microbiome diversity and composition. Individuals ≥ 18-years-old were recruited during the second trimester of pregnancy, and u

Obstetric Pharmacomicrobiomics.

Pregnancy is characterized by physiologic, metabolic, immunologic, and microbial adaptation. Medication use during pregnancy is common. Yet, therapeutic response often remains unpredictable because obstetric pharmacology has historically focused on pharmacokinetic changes while paying little attention to host-microbiome-drug interactions. Pharmacomicrobiomics, the study of bidirectional interactions between the microbiome and therapeutic agents, has emerged as an important frontier in precision medicine. Increasing evidence demonstrates that the maternal microbiome may influence drug bioavaila

The contribution of circadian, sleep and hormonal rhythms to pregnancy outcomes: an opportunity for improved management of women during pregnancy.

Circadian rhythms are fundamental regulators of physiology and play a critical role in reproductive success. During pregnancy, precise temporal coordination of endocrine, metabolic, and immune processes is required to support maternal adaptation, placental function, and foetal development. Disruption of circadian rhythms through environmental factors such as light at night, shift work, or sleep disorders has emerged as a significant risk factor for adverse pregnancy outcomes, including preterm birth, gestational diabetes, and preeclampsia. In this review, we synthesize current evidence linking

Experiences and perceived determinants of healthcare-related time toxicity among women with gestational diabetes mellitus: a qualitative study.

BACKGROUND: Gestational diabetes mellitus (GDM) requires women to attend follow-up visits and complete repeated self-management tasks during pregnancy. These tasks include blood glucose monitoring, meal planning, physical activity, health education and medication when needed. They may create a time-related burden in daily life. However, little is known about women's experiences of healthcare-related time toxicity during GDM management. This study aimed to explore women's experiences of healthcare-related time toxicity and the perceived factors shaping these experiences. METHODS: A qualitative

Case Report: Treatment of fulminant paraquat poisoning in a 32-week pregnant patient: a case report exploring the potential role of estrogen.

Paraquat (PQ) poisoning is a life-threatening condition characterized by severe toxicity and the absence of a specific antidote. Prognosis is strongly correlated with the ingested dose, with high-dose exposure frequently resulting in multi-organ failure and fatal outcomes. This case report presents the clinical management of a 32-week pregnant patient with fulminant PQ poisoning, a scenario typically associated with near-certain mortality. Remarkably, the patient survived, supported by a markedly elevated peak serum estradiol level of 27,000 pg/mL, which may suggest a potential cytoprotective

Association of maternal height with pregnancy complications: Analyses of two cohorts.

BACKGROUND: Height is inversely associated with risk of cardiovascular and metabolic conditions in nonpregnant populations. Whether maternal height is independently associated with pregnancy complications, after accounting for prepregnancy weight, remains less clear. OBJECTIVE: To investigate associations of maternal height with pregnancy complications, independent of prepregnancy weight, and to examine the role of infant sex and race/ethnicity in these associations. METHODS: We conducted a secondary analysis of data from the Omega and Hyperglycemia and Adverse Pregnancy Outcome (HAPO) prosp

Preeclampsia and Gestational Diabetes in the Texas Panhandle: A Five-Year Epidemiologic Study.

BACKGROUND: Maternal care deserts, areas with limited or no access to maternity care, pose a significant risk for pregnancy complications. The VIBRANT MOMS coalition, developed to address maternal morbidity and mortality in the Texas Panhandle, conducted a needs and asset assessment of six counties, four of which have low or no access to maternal care. This study aimed to calculate the prevalence of preeclampsia and gestational diabetes, as well as associated complications, among hospital deliveries to establish a baseline estimate of maternal morbidity in the region. METHODS: An epidemiologi

Insufficient Implementation of Recommendations in Pregnancies after Bariatric Surgery: Results from a Nationwide Real-World Study in Germany (PABS Study).

Introduction Bariatric metabolic surgery (MBS) is increasingly performed in women of reproductive age. Although MBS reduces obesity-related pregnancy complications, altered metabolism and nutritional deficiencies may adversely affect maternal and neonatal outcomes. This study evaluated (1) adherence to current guideline recommendations and (2) pregnancy outcomes in women after MBS in Germany. Materials and Methods In this multicenter cohort study, 410 pregnancies after MBS were collected between 2023 - 2025; 380 cases with complete data on BMI, MBS type, and birth weight were finally analyzed.

Hypertensive disorders of pregnancy and gestational diabetes mellitus by birth order: A descriptive study across lifetime reproductive history.

INTRODUCTION: Hypertensive disorders of pregnancy and gestational diabetes mellitus are known to be strongly related, and underlying maternal cardiometabolic dysfunction has been proposed as a mechanism for both complications. A comparison of the trends by birth order for the two complications may reveal insights into their shared or different causes. MATERIAL AND METHODS: We used data from the Medical Birth Registry of Norway from 1999 to 2020 with an aim to compare risk by birth order for hypertensive disorders of pregnancy and gestational diabetes mellitus. Visualizations of cross-sectiona

Genetically Predicted IgG N-Glycosylation Traits and Risk of Pre-Eclampsia or Eclampsia: A Two-Sample and Multivariable Mendelian Randomization Study.

BACKGROUND: Maternal immunoglobulin G (IgG) has been implicated in placental and vascular dysfunction across pre-eclampsia and eclampsia. While clinical evidence supports the relevance of IgG-mediated immune regulation, whether specific IgG N-glycosylation traits-which critically regulate IgG concentration and effector function-are associated with the combined outcome of pre-eclampsia or eclampsia remains unclear. METHODS: A two-sample Mendelian randomization (MR) study evaluated the causal relevance of 77 IgG N-glycosylation traits for the combined outcome of pre-eclampsia or eclampsia. Prim

Adjunctive screening and risk stratification performance of neutrophil-to-lymphocyte ratio for gestational diabetes mellitus: A systematic review and meta-analysis.

The neutrophil-to-lymphocyte ratio (NLR) is an inexpensive inflammatory marker with uncertain value for gestational diabetes mellitus (GDM) risk assessment. This systematic review and meta-analysis evaluated NLR for first-trimester risk stratification and second-trimester adjunctive screening of GDM. PubMed, Embase, Web of Science, and the Cochrane Library were searched from inception to April 15, 2026. Risk of bias was assessed using the Quality Assessment of Diagnostic Accuracy Studies 2 (QUADAS-2) tool. Sensitivity and specificity were pooled separately by trimester, and summary receiver op

Gestational exposure to organophosphate esters (OPEs) and adiposity during middle childhood in the MIREC Cohort.

Toxicological evidence suggests that organophosphate esters (OPEs) may disrupt metabolic processes involved in energy balance and adiposity, yet epidemiological findings remain inconsistent. Among 283 mother-child dyads in the Maternal-Infant Research on Environmental Chemicals (MIREC) cohort, we examined associations between gestational urinary OPE metabolites, including bis(2-chloroethyl) phosphate (BCEP), bis(1-chloro-2-propyl) phosphate (BCIPP), bis(1,3-dichloro-2-propyl) phosphate (BDCIPP), di-n-butyl phosphate (DNBP), and diphenyl phosphate (DPHP), measured at 6-13 and 16-21 weeks of ges
