Antenatal interventions to support newborn survival: updates for the Lives Saved Tool.

BACKGROUND: Antenatal care strategies encompass a series of interventions delivered to expectant mothers to optimise both their health outcomes and those of their neonate. We summarise the most recent effect estimates for maternal immunisation, screening and management of infections, pre-existing chronic diseases such as hypertension and diabetes, nutrition, psychosocial interventions for smoking cessation, pre-term pre-labour rupture of membranes, administration of corticosteroids for foetal lung maturation, and induction of post-term labour. METHODS: We synthesised effect estimates for ante

Clinical and Molecular Characterization of Children with Neonatal Diabetes: Experience from Bangladesh.

Neonatal diabetes mellitus (NDM) is a rare monogenic form of diabetes presenting before six months of age which can be permanent (PNDM) or transient (TNDM). In rare cases, NDM can be diagnosed between 6 and 12 months of age. Whilst TNDM is commonly associated with chromosome 6q24 methylation defects, the commonest causes of PNDM are KCNJ11 and ABCC8 gene mutations. This observational study's aim was to assess the clinical features and genetic aetiologies in patients with NDM diagnosed in Bangladesh. The clinical characteristics of 28 patients who presented with NDM from 2001 to 2018 in the in

Association between first-trimester serum 25(OH)D and EPDS-positive postpartum depressive symptoms in women with gestational diabetes mellitus: a retrospective cohort study.

INTRODUCTION: Gestational diabetes mellitus (GDM) has been associated with an increased risk of postpartum depressive symptoms, while maternal vitamin D status may also be related to perinatal mood disturbances. This study investigated the association between first-trimester serum 25-hydroxyvitamin D [25(OH)D] concentrations and Edinburgh Postnatal Depression Scale (EPDS)-positive postpartum depressive symptoms in women with GDM. METHODS: This retrospective cohort study included 242 women with GDM. Prenatal depressive and anxiety symptoms were assessed using the Patient Health Questionnaire-9

Drug-induced hyperglycemia in obstetrics: key offending drugs, mechanisms, and practical management.

INTRODUCTION: Drug-induced hyperglycemia is an important metabolic complication that may arise when medications disrupt insulin secretion, insulin sensitivity, or glucose homeostasis. In obstetric practice, several commonly used drugs, including glucocorticoids, progesterone, antihypertensive agents, tocolytic drugs, and immunosuppressants, may impair glycemic control to varying degrees, and prolonged exposure may even lead to drug-induced diabetes mellitus. These effects are particularly important in pregnant women with pregestational diabetes, gestational diabetes, obesity, or other risk fac

Progress and Practice of Insulin Resistance in Pregnancy: From Pathogenesis to Management.

Evidence suggests that the degree of insulin resistance in pregnancy (IRiP) seems to affect pregnancy outcomes regardless of the presence of gestational diabetes. A physiological moderate increase in IRiP is a protective factor for fetal development, but excessive enhancement can lead to maternal metabolic disorders and placental dysfunction, increasing the risk of gestational diabetes and adverse pregnancy outcomes, as well as having adverse effects on the long-term health of the offspring. This review briefly explores the key factors influencing IRiP, including pregnancy-specific and non-pre

Impact of country income status on the relationship between interpregnancy interval and maternal complications of pregnancy: a systematic review and meta-analysis protocol.

BACKGROUND: Both short and long interpregnancy intervals (IPIs) have been associated with adverse maternal outcomes, including uterine rupture, gestational diabetes mellitus (GDM), gestational hypertension and postpartum haemorrhage (PPH). However, the strength and consistency of these relationships remain uncertain, and there is limited information on how these associations differ according to country income status. This review is designed to examine the impact of the income status of a country on the relationship between IPI and maternal complications of pregnancy. METHODS AND ANALYSIS: CIN

Associations of mid-pregnancy plasma fatty acid patterns with gestational diabetes mellitus risk: A 1:2 matched case-control study.

BACKGROUND AND AIMS: Gestational diabetes mellitus (GDM) is a common metabolic complication of pregnancy, closely related to insulin resistance and altered fatty acid metabolism. However, evidence on mid-pregnancy fatty acid profiles and GDM risk remains limited and inconsistent. We comprehensively characterized mid-pregnancy circulating fatty acid profiles and their derived patterns in relation to GDM risk in a matched case-control study. METHODS: Based on a prospective cohort study conducted in Shenzhen, China, we performed a 1:2 matched case-control study including 290 GDM cases and 580 no

Sleep Patterns During Pregnancy, Genetic Susceptibility and Progression From Gestational Diabetes Mellitus to Postpartum Glucose Intolerance: A Prospective Cohort Study.

AIMS: Gestational diabetes mellitus (GDM) is related to postpartum glucose intolerance (PGI) and future type 2 diabetes mellitus (T2DM). Sleep is a modifiable factor, yet its role in the GDM-to-PGI progression and its interaction with genetic susceptibility remain uncertain. We examined relationships between prenatal sleep patterns and PGI risk and their joint effects with genetic risk among women with GDM. MATERIALS AND METHODS: This prospective cohort study recruited 897 women with GDM in Guangzhou, China (2022-2024). A healthy sleep score was constructed by integrating prenatal sleep durat

Chronic Hypertension and Cardiovascular Disease After Preeclampsia: A Danish Nationwide Register-Based Cohort Study Identifying Women at High Risk.

AIMS: Preeclampsia is associated with future cardiovascular disease (CVD), yet systematic approaches to identify those requiring intensified prevention are lacking. We aimed to characterise these women and identify subgroups at particularly high risk of later chronic hypertension or CVD. METHODS: Using Danish nationwide registries, we followed primiparous women with preeclampsia who gave birth during 2004-2018 until December 2022. Using Poisson regression models we estimated incidence rate ratios (IRR) of later chronic hypertension or CVD. RESULTS: Of 17,434 women with preeclampsia, 2,372 de

Streamlining intrapartum glycemic control: Subcutaneous insulin for intrapartum diabetes management.

OBJECTIVE: Optimal strategies for intrapartum glycemic control among patients with gestational diabetes mellitus (GDM) and type 2 diabetes mellitus (T2DM) remain uncertain. In 2024, our labor and delivery unit implemented a subcutaneous (SC) insulin protocol to replace routine intravenous (IV) insulin initiation when glucose exceeded a treatment threshold. We hypothesized that this change would not worsen neonatal outcomes and would improve nursing (RN) satisfaction. METHODS: We conducted a retrospective cohort quality improvement study of patients with GDM or T2DM who delivered at ≥36 weeks

Long-term metabolic consequences of in-utero gestational diabetes exposure and maternal treatment interventions: a systematic review and meta-analysis.

BACKGROUND: Gestational diabetes mellitus (GDM) affects 14-17% of pregnancies globally, imposing long-term public health burdens beyond pregnancy. Maternal hyperglycaemia increases offspring risk of metabolic disorders through epigenetic, hormonal and developmental programming. Growing evidence connects prenatal GDM exposure to childhood and adult metabolic diseases, yet the consistency and magnitude of these associations across different metabolic conditions remains unclear. METHODS: We systematically searched PubMed, Cochrane CENTRAL and Embase. Eligible studies included observational cohor

Association of the GLM7 composite metabolic index with gestational diabetes mellitus in early pregnancy: a secondary analysis of a Korean prospective pregnancy cohort.

BACKGROUND: Surrogate indices such as the triglyceride-glucose (TyG) index are widely used to estimate insulin resistance (IR), yet their ability to discriminate incident gestational diabetes mellitus (GDM) in early gestation remains suboptimal. This study evaluated whether the previously developed composite metabolic index GLM7 was correlated with subsequent GDM in Korean singleton pregnant women. METHODS: In this study, we analyzed 582 singleton pregnancies from a Korean pregnancy registry. Multivariable logistic regression and restricted cubic splines (RCS) were used to evaluate the associ
