Serial insulin and C-peptide concentrations following high-dose insulin for the treatment of drug poisoning: a consecutive case series.

INTRODUCTION: High-dose insulin is a treatment for poisoning-induced cardiogenic shock. There is a risk of hypoglycaemia after discontinuing high-dose insulin due to persistent hyperinsulinaemia. The pharmacokinetics of insulin after discontinuing high-dose insulin are poorly defined, with previous publications reporting prolonged elimination half-lives (up to 18.8 h). METHODS: We conducted a consecutive case series in poisoned patients treated with high-dose insulin. Serial insulin and C-peptide concentrations were measured after discontinuing high-dose insulin. The apparent elimination half

Continuous Glucose Monitoring for Postkidney Transplantation in Patient With Pre-existing Type 2 Diabetes.

BACKGROUND: Kidney transplantation poses glycemic control challenges for type 2 diabetes mellitus due to immunosuppressive therapy and metabolic changes. In this pilot study, we aimed to assess glucometrics achieved with a continuous glucose monitor (CGM)-based insulin titration protocol during prednisone taper postkidney transplant. METHODS: Kidney transplant recipients with pre-existing type 2 diabetes mellitus on insulin therapy were enrolled. CGM metrics were used to adjust insulin dose for 3 months. Data up to 6 months were included. CGM metrics, insulin and prednisone doses, kidney func

Association of Diabetes Mellitus and COVID-19-Related Pancreatic and Biliary Inflammatory Diseases.

Background/Objectives: The COVID-19 pandemic has brought about significant clinical challenges in regard to digestive systems, as well as causing complications such as pancreatitis and biliary infections. Whether diabetes mellitus (DM) contributes to both an increased risk for these complications and mortality amongst COVID-19 patients remains to be investigated. This study aimed to illuminate any possible outcomes, including pancreatitis, cholangitis, cholecystitis and all-cause mortality, among COVID-19 patients with and without pre-existing type 2 diabetes mellitus (T2DM), using real-world

Postpartum immune thrombotic thrombocytopenic purpura masquerading as haemolysis, elevated liver enzymes and low platelets syndrome: overcoming a diagnostic dilemma.

Pregnancy-associated thrombotic microangiopathy poses a significant diagnostic challenge because of the overlap between hemolysis, elevated liver enzymes, and low platelet count syndrome (HELLP syndrome) and thrombotic thrombocytopenic purpura (TTP). We report a case of postpartum immune TTP in a 30-week gravida 3 para 1 woman with pre-existing type 2 diabetes mellitus who initially presented with features consistent with HELLP syndrome and imminent eclampsia. Despite emergency delivery, she developed persistent severe thrombocytopenia and microangiopathic haemolytic anaemia. Further evaluatio

Consensus approach to differential abundance analysis detects few differences in the oral microbiome of pregnant women due to pre-existing type 2 diabetes mellitus.

Oral microbiome dysbiosis has been proposed as a potential contributing factor to rising rates of diabetes in pregnancy, with oral health previously associated with an increased risk of numerous chronic diseases and complications in pregnancy, including gestational diabetes mellitus (GDM). However, whilst most studies examining the relationship between GDM and the oral microbiome identify significant differences, these differences are highly variable between studies. Additionally, no previous research has examined the oral microbiome of women with pre-existing type 2 diabetes mellitus (T2DM),

Subcutaneous Insulin Aspart Every 4 Hours in the Treatment of COVID-19 Patients With Mild-to-Moderate Diabetic Ketoacidosis: A Case Series.

This case series evaluates the efficacy and safety of subcutaneous (SC) insulin Aspart administered every 4 h for managing mild-to-moderate diabetic ketoacidosis (DKA) in COVID-19 patients, addressing a current evidence gap. We conducted a retrospective review of confirmed COVID-19 patients over 15 years old who developed mild to moderate DKA between July 2020 and October 2021. Insulin Aspart was administered at 0.4 units/kg SC every 4 h, reduced to 0.2 units/kg when blood glucose (BG) decreased to <250 mg/dL, and SC basal insulin was initiated at 0.15-0.2 units/kg at DKA diagnosis. Diabetes is an Increasingly Common Issue After Heart Transplantation: A Case for Integrated Diabetes Care.

BACKGROUND: Orthotopic heart transplantation (OHT) survival rates have improved with advances in immunosuppression over the last 20 years. With these improvements, there has been a greater focus on post-transplant care. Diabetes is common after transplantation and may be pre-existing (type 2 diabetes mellitus [T2DM]) or develop after transplant (post-transplant diabetes mellitus [PTDM]). The aim of this study was to compare the incidence and prevalence of diabetes in OHT recipients in two cohorts separated by 20 years. METHODS: Retrospective audit comparing the prevalence of T2DM and cumulati

IL-6 Polymorphism as a Predisposing Genetic Factor for Gestational Diabetes or Preeclampsia Development in Pregnancy with Obesity in Relation to VEGF and VEGFF Receptor Gene Expression Modalities.

The increased prevalence of obesity worldwide has been implicated in the alarming rise of the incidence of gestational diabetes and preeclampsia, which are both considered threatening conditions for both mother and fetus. We studied gene polymorphisms of the proinflammatory cytokine Interleukin 6 (IL-6) and the gene expression levels of VEGF (vascular endothelial growth factor) and VEGF-R (endothelial growth factor receptor), all known to be involved in pregnancy complications, aiming to identify possible predisposing risk factors in pregnancies with obesity. The G allele of IL-6 was found to

Impact of Breast Cancer on Cardiometabolic Health in Spanish Women ≥50 Years with Pre-Existing Type 2 Diabetes Mellitus.

During breast cancer (BC), cardiometabolic disorders can worsen prognosis, particularly in women with type 2 diabetes mellitus (T2DM). This study aimed to determine the impact of BC diagnosis on cardiometabolic parameters and the incidence of complication in women over 50 years of age (90% aged ≥ 65 years) with pre-existing T2DM. Using primary care registries from Asturias (Spain), a total of 106 women diagnosed with T2DM followed by BC were selected and matched with women with T2DM (n = 212) in a cohort study. Indicators of cardiometabolic health and microvascular complications associated wit

Atezolizumab-Induced Type 1 Diabetic Ketoacidosis in a Patient With Small Cell Lung Cancer and Pre-existing Type 2 Diabetes Mellitus.

In this report, we present a case of a 70-year-old male with small cell lung cancer (SCLC) and pre-existing type 2 diabetes mellitus (T2DM) who developed type 1 diabetic ketoacidosis (DKA) following treatment with atezolizumab plus chemotherapy. Despite well-controlled T2DM with oral hypoglycemic agents, the initiation of immune checkpoint inhibitors (ICIs) led to rapid deterioration into insulin-dependent status due to ICI-induced type 1 diabetes mellitus (T1DM). Vigilant monitoring for hyperglycemia and timely intervention is crucial during ICI therapy, considering the potentially life-threa

Corticosteroids plus metformin versus corticosteroids as front-line treatment for patients with newly diagnosed ITP and pre-existing type 2 diabetes mellitus: A multicentre propensity score-matched study.

Corticosteroids are the standard first-line treatment for primary immune thrombocytopenia (ITP), with a high initial response but unsatisfactory sustained response (SR). Additionally, corticosteroids usually lead to hyperglycaemia especially in patients with pre-existing type 2 diabetes mellitus (T2DM). Besides reducing the blood glucose levels, metformin was found to have immunomodulatory effects. We hereby conducted a multicentre propensity score matching analysis of corticosteroids plus metformin versus corticosteroids for newly diagnosed ITP patients with pre-existing T2DM. After matching
