AA and Pregnancy.

AA is a crucial fatty acid in pregnancy, especially for the growth and development of the fetus and its central nervous system (CNS). As already discussed in the previous chapters, AA regulates inflammation, immune response, and cell signaling. AA has a modulatory action on gene expression, serves as a mechanotransducer, and regulates cell membrane fluidity. By regulating cell membrane fluidity, AA modulates the expression and binding of several receptors located on the cell membrane. Eicosanoids derived from AA have both beneficial and harmful effects on pregnancy. For instance, (i) AA metabo

A nomogram integrating first-trimester metabolic and angiogenic markers for predicting adverse pregnancy outcomes in gestational diabetes mellitus: development and internal validation.

BACKGROUND: Gestational diabetes mellitus (GDM) is associated with a significantly increased risk of adverse pregnancy outcomes. This study aimed to develop and internally validate a nomogram integrating integrating first-trimester metabolic and angiogenic markers to predict composite adverse pregnancy outcomes in GDM. METHODS: We conducted a retrospective cohort study of 1,000 GDM patients (training set n=700, validation set n=300). Multivariable logistic regression was used to identify independent predictors and construct a nomogram. Model performance was assessed by discrimination (area un

Maternal PFOS Exposure Impairs β‑Cell Insulin Secretion and Glucose Tolerance in Rats.

Perfluorooctanesulfonic acid (PFOS) is epidemiologically linked to gestational diabetes mellitus (GDM), but the mechanisms remain undefined. We hypothesized that PFOS exposure impairs pancreatic β-cell function through mitochondrial dysfunction and deficits in ATP/cAMP signaling. Pregnant and non-pregnant rats received PFOS (50 μg/mL in drinking water) for 17 days (gestation day [GD] 4-20 in pregnant dams). PFOS-exposed pregnant dams exhibited fasting hypoinsulinemia (65% decrease) with normal fasting glucose. Glucose tolerance was impaired (31% increase in glucose AUC), with blunted insulin s

Recent Advances in the Diagnosis and Management of Gestational Diabetes Mellitus: A Narrative Review.

Gestational diabetes mellitus (GDM) is one of the most common metabolic disorders of pregnancy and is associated with adverse short- and long-term health outcomes in both mothers and offspring. Its increasing global prevalence and substantial heterogeneity in pathogenesis and clinical presentation underscore the limitations of conventional glucose-centered approaches to risk assessment and management. Although the oral glucose tolerance test (OGTT) remains the established diagnostic standard, its relatively late diagnostic window and limited ability to predict disease development have stimulat

Gestational diabetes mellitus in Italian diabetes clinics: clinical characteristics, screening practices and management from the amd annals registry.

BACKGROUND: Gestational diabetes mellitus (GDM) is one of the most common complications of pregnancy and is associated with adverse maternal and neonatal outcomes. Nationwide real-world data describing characteristics , screening practices, clinical management, and quality of care of women with GDM attending diabetes clinics in Italy are limited. METHODS: We conducted a retrospective observational study using data from the AMD Annals Initiative, a well-established large quality-of-care monitoring program. Anonymized clinical data from women with GDM attending participating centers during 2025

The CKM-GDM multi-organ precision model: reframing gestational diabetes as an early cardiovascular-kidney-metabolic event in Southeast Asia.

BACKGROUND: Gestational diabetes mellitus (GDM) is amongst the commonest complications of pregnancy and an early marker of lifetime cardiovascular-kidney-metabolic (CKM) risk. Its burden is heavy in Southeast Asia, where prevalence ranges from about 8% to nearly 30% and a lean phenotype - with marked over-representation of the insulin-secretion-limited subtype - diverges from Western cohorts, yet its definition, timing, and treatment remain contested. AIM: This clinically oriented narrative review synthesises the diagnostic landscape, the shift to early diagnosis, the vascular mechanism, and

Perinatal Outcomes and Determinants of Elective Embryo Reduction in Trichorionic Triplet Pregnancies.

OBJECTIVE: To evaluate whether elective embryo reduction (ER) reduces maternal obstetric complications in trichorionic triamniotic triplet pregnancies, to identify the maternal characteristics associated with the decision to undergo ER and its extent, and to describe associated perinatal outcomes. METHODS: This retrospective study included 120 trichorionic triamniotic triplet pregnancies managed at two tertiary hospitals in Spain (2002-2024). ER to twins or a singleton was offered before 14 weeks of gestation. Maternal characteristics, obstetric complications, and perinatal outcomes were comp

Effects of different gestational diabetes treatments on myo-inositol levels and placental insulin signalling.

Myo-inositol (MI) is a precursor of inositol-3-phosphate, which participates in the insulin receptor pathway. Glucose enhances urinary MI excretion, which could affect gestational diabetes mellitus (GDM) management. The relationship between MI levels and GDM treatment remains unclear, raising questions about potential MI supplementation. The aim of this study was to evaluate MI levels in GDM women treated with either diet or insulin, and their associations with glycaemia and placental insulin signalling. MI was quantified in 23 women with GDM treated with diet (GDM-Diet), 20 GDM treated with i

Elevated lipoprotein(a) and adverse pregnancy outcomes.

Lipoprotein(a) [Lp(a)] is gaining attention as a potential marker for adverse pregnancy outcomes, in addition to its well-established role in cardiovascular disease. This review explores the evolving understanding of Lp(a) in pregnancy, focusing on its links to hypertensive disorders of pregnancy, gestational diabetes mellitus, fetal growth restriction, and preterm birth, while also addressing diagnostic and management challenges. During pregnancy, Lp(a) levels rise substantially, nearly doubling between weeks 10 and 35. Although this increase is universal, its clinical implications are unclea

Glycaemic Management and Pregnancy Outcomes in HNF1A-MODY.

INTRODUCTION: Birthweight of HNF1A-affected offspring is largely similar to the background population with rarely reported cases of large for gestational age (LGA) and neonatal hypoglycaemia (NH). Conventional clinical management of pre-gestational diabetes appears to be sufficient in managing HNF1A-affected women in pregnancy. This study aims to evaluate pregnancy outcomes and review glucose management in a cohort with HNF1A-MODY. METHODOLOGY: This is a retrospective review of 183 offspring (142 maternal and 41 paternal) of 73 parents with confirmed HNF1A-MODY. Data were obtained from matern

The AMPK/mTOR axis is linked to placental ferroptosis in human gestational diabetes mellitus.

BACKGROUND: Ferroptosis has been closely implicated in placenta-related complications. Nevertheless, its specific role in the placenta of gestational diabetes mellitus (GDM) remains poorly understood. We hypothesized that ferroptosis contributes to impaired placental cell function in GDM and that the adenosine 5'-monophosphate (AMP)-activated protein kinase (AMPK)/mammalian target of rapamycin (mTOR) axis acts as an potential regulator of ferroptosis. METHODS: Term placental tissues from pregnant women with GDM and healthy controls were analyzed, and the human chorionic trophoblast line (HTR8

Impact of excessive weight gain and pre-gestational obesity on maternal and neonatal outcomes.

Excessive gestational weight gain and prepregnancy obesity have been associated with adverse maternal and neonatal outcomes, although their associations with specific pregnancy outcomes may differ. This retrospective, cross-sectional study included 972 women who delivered at a public referral hospital in Brazil. Maternal and neonatal data were obtained from electronic medical records. Continuous and categorical variables were compared using appropriate statistical tests, and multivariable logistic regression analyses were performed to evaluate the associations between excessive gestational wei

Weight management interventions for pregnant women from minority ethnic populations living in the United Kingdom: a scoping review.

BACKGROUND: Maternal obesity disproportionately affects women from minority ethnic backgrounds in the United Kingdom, who face substantially higher risks of gestational diabetes mellitus, hypertensive disorders, and maternal mortality than White British women. Despite well-documented disparities and relevant National Institute for Health and Care Excellence guidance, the evidence base for culturally appropriate weight management during pregnancy remains poorly mapped. AIM: To map and synthesize available evidence on weight management interventions during pregnancy among minority ethnic women

Health behavior profiles in early pregnancy are associated with adverse pregnancy outcomes and maternal blood pressure 2-7 years after delivery.

BACKGROUND: The joint effects of individual health behaviors on pregnancy health are rarely considered. We examined associations between combinations of first trimester health behaviors with adverse pregnancy outcomes (APOs) and blood pressure (BP) 2-7 years after delivery, and estimated the proportion of associations with later BP mediated by APOs. METHODS: Participants in the nuMoM2b Heart Health Study prospective cohort were included. Physical activity, diet, sleep duration, and smoking were scored using the Life's Essential 8 framework. APOs included hypertensive disorders of pregnancy, p
