A Genetic Landscape of Euploid Miscarriages From Couples With Recurrent Pregnancy Loss Through Whole Exome Sequencing.

BACKGROUND: This study aimed to identify candidate genes for recurrent pregnancy loss (RPL). METHODS: Trio-whole exome sequencing (WES) analysis was performed with blood samples and euploid miscarriage tissues from 55 couples. RESULTS: Overall, 117 de novo protein-altering variants of 83 genes from 38 miscarriages were identified, with three alterations per embryo on average. A vast majority of the de novo variants were missense/indel and classified as variants of uncertain significance. The de novo variants preferentially hit the intolerant genes and impacted the functional pathways such as

Identification of key genes in recurrent pregnancy loss related to obesity.

OBJECTIVE: The underlying mechanism of recurrent pregnancy loss (RPL) is still not fully understood. We aimed to identify the key genes involved in the process by which obesity influences RPL. METHODS: RPL and obesity data were retrieved from the GEO database. The differentially expressed genes (DEGs) between disease and normal samples among RPL and obesity were selected. Through intersecting the above DEGs, important DEGs were obtained. GO and KEGG analyses were used to analyze the function of these important genes. Two algorithms (LASSO and SVM) and receiver operator characteristic (ROC) an

The role and underlying mechanism of FABP4 dysregulation mediated by m6A modification in unexplained recurrent spontaneous abortion.

BACKGROUND: Unexplained recurrent spontaneous abortion (URSA) poses a significant clinical challenge with an elusive pathogenesis. FABP4 is crucial for lipid homeostasis and placental development, but its specific role and regulatory mechanism in URSA remain unclear. METHODS: FABP4 expression was analyzed in human placental tissues from URSA patients and control groups. The functional role of FABP4 was investigated using Fabp4-knockout mouse models and trophoblast cell lines. Mechanistic studies employed Seahorse metabolic analysis, Raman spectroscopy, methylated RNA immunoprecipitation qPCR

Hemostatic resuscitation: blood product replacement during cesarean delivery.

Despite efforts to improve accurate prediction, early detection, and optimal medical and surgical treatments, obstetrical hemorrhage remains the leading cause of maternal mortality worldwide. The development and application of clinical bundles incorporating risk assessment tools focused on earlier detection and prompt intervention have demonstrated improved clinical outcomes. Many clinical bundles and obstetrical hemorrhage institutional protocols have incorporated different methods attempting to quantify intraoperative blood loss during cesarean. The addition of blood loss quantification, whe

Integration of transcriptome and DNA methylation reveals the mechanism of cilia-related genes in recurrent miscarriage.

Recurrent miscarriage (RM), a complex pregnancy disorder with largely undefined molecular mechanisms, has been associated with epigenetic abnormalities in chorionic tissue. This study aimed to elucidate methylation-dependent cilia-related genes (CRGs) implicated in RM. An integrative analysis combining RNA sequencing, public transcriptome data, and DNA methylation profiles was conducted to identify RM-related CRGs. Machine learning algorithms were applied to determine the most relevant candidates. Immune infiltration profiling, gene set enrichment analysis (GSEA), and competitive endogenous RN

NIT_CH: A study protocol for evaluating the effect of inorganic nitrate capsules in chronic hypertensive pregnancies - a triple-blind placebo-controlled randomized trial.

BACKGROUND: Hypertensive disorders of pregnancy (HDP) represent a substantial risk to maternal and fetal health, with increasing prevalence worldwide linked to factors such as advanced maternal age, rising obesity rates, and metabolic syndrome. Effective management of borderline hypertension during pregnancy remains critical. However, given the limited safety data on conventional antihypertensive drugs in this population, alternative therapeutic strategies are needed. Inorganic nitrate supplementation has demonstrated potential in reducing blood pressure and enhancing maternal uterine artery f

Severe Post-spinal Bezold-Jarisch Reflex During Emergency Caesarean Delivery Unmasking Wolff-Parkinson-White Pattern.

Spinal anaesthesia for caesarean delivery commonly causes maternal hypotension due to sympathetic blockade and may occasionally progress to profound bradycardia with haemodynamic collapse when venous return is markedly reduced. We report the case of a 34-year-old gravida 3 para 2 woman at 36 weeks of gestation who underwent emergency caesarean delivery under single-shot spinal anaesthesia. Shortly after the onset of the block, she developed significant hypotension, which rapidly progressed to profound hypotension with marked bradycardia, producing a clinical picture consistent with the Bezold-

Prenatal Origin of Childhood Overweight, Obesity and Insulin Resistance, with Special Emphasis on Maternal Diabetes, Excessive Weight, Nutrition and Hormone Imbalances.

Overweight in childhood, adolescence and adulthood is now a world-wide epidemic. A variety of preventive and therapeutic measures are being developed with partial success. Since the pioneering studies of D.J. Barker linking maternal famine in pregnancy to long-term undesirable effects on the offspring-the "metabolic syndrome" that stems from long-lasting epigenetic changes, it became evident that there are many undesirable events which affect in-utero fetal growth and development. They often persist postnatally, through childhood and adulthood. The purpose of this review is to discuss the pren

Chronic endometritis and reproductive failure: The paradigm shift from microbial eradication to ecological restoration.

Despite advancements in assisted reproductive technologies, such as preimplantation genetic testing, clinical success rates have plateaued, thereby increasing the clinical focus on the endometrial factor. Chronic endometritis (CE), a persistent and often asymptomatic mucosal inflammation, is an underdiagnosed condition associated with reproductive failure, particularly in cohorts with repeated implantation failure (RIF) and recurrent pregnancy loss (RPL). This review examines the evolving understanding of CE pathophysiology, transitioning from a traditional infection model toward a framework i

Robot-Assisted Transabdominal Cervical Cerclage With the Hugo RAS System: Surgical Technique and Perioperative Outcomes in a High-Risk Obstetric Case.

OBJECTIVE: Cervical insufficiency, the inability of uterine cervix to retain pregnancy in absence of labor, is a major cause of preterm births and stillbirths. Therapeutic options include expectant management, progesterone, pessaries, or surgical cerclage [1]. Minimally invasive approaches provide several advantages over laparotomy, including decreased blood loss, lower post-operative pain, shorter hospital stay and faster recovery while achieving comparable or improved obstetric outcomes [2,3]. Robotassisted transabdominal cerclage (RA-TAC) is gaining popularity due to the enhanced three-dime

Can perioperative shifts in inflammatory indices predict maternal and neonatal outcomes following cervical cerclage?

BACKGROUND: Cervical insufficiency is a known risk factor for spontaneous preterm delivery and often leads to pregnancy loss in the second trimester. The aim of the study was to determine whether the Systemic Immune-Inflammation Index (SII), Systemic Inflammatory Response Index (SIRI) and Pan-Immune Inflammation Value (PIV) before and after cerclage, as well as alterations in these indices, can predict preterm birth and composite adverse perinatal outcomes (CAPOs). METHODS: This retrospective study included 104 singleton pregnancies in which cervical cerclage was performed between 12 and 24 we
