Laparoscopic transabdominal cerclage: preconception versus postconception placement.

BACKGROUND: Cervical insufficiency is a major contributor to spontaneous preterm birth and second-trimester pregnancy loss. Transabdominal cerclage is recommended for patients with a history of failed vaginal cerclage or when vaginal cerclage is not anatomically feasible. Laparoscopic transabdominal cerclage has emerged as a minimally invasive alternative to open transabdominal cerclage. However, data comparing obstetric and surgical outcomes based on the timing of laparoscopic transabdominal cerclage placement before vs during pregnancy. OBJECTIVE: Primary aims were to evaluate whether lapar

Comparison of two strategies to prevent preterm birth in twins complicated by cervical insufficiency: a retrospective cohort study.

OBJECTIVE: The strategies of preventing preterm birth in twin pregnancies complicated by cervical insufficiency is controversial. We compared the efficacy of fetal reduction combined with prophylactic cervical cerclage and prophylactic cervical cerclage alone in preventing preterm birth in this population. METHODS: A retrospective cohort study was conducted in two tertiary hospitals in China. A total of 82 twins complicated by cervical insufficiency who received either fetal reduction combined with cervical cerclage treatment (reduction group, n = 40) or only prophylactic cervical cerclage (o

History-indicated cerclage versus ultrasound cervical length screening in women with a history of one extremely preterm birth: A comparative observational cohort study.

INTRODUCTION: In women with a history of one extremely preterm birth, the current literature suggests that ultrasound cervical length screening offers a more favorable risk-benefit ratio than history-indicated cerclage. However, some of the women included in previous studies were at a low risk of cervical insufficiency. Therefore, the efficacy of history-indicated cerclage may have been underestimated. Our objective was to compare history-indicated cerclage with ultrasound cervical length screening in women with a history of one second-trimester loss or extremely preterm birth suggestive of ce

The Experience and Outcomes of Rescue Cervical Cerclage at a Victorian Tertiary Hospital-A Retrospective Observational Study.

A rescue cervical cerclage is an emergency procedure aimed to prolong pregnancy and reduce the risk of preterm birth for women with an open cervix. This retrospective study assessed risk factors, perioperative management and outcomes for all attempted rescue cerclage over 3 years in a large tertiary hospital in Australia. Data were obtained from maternity and hospital-level records. Thirty pregnancies were included, with 27 rescue cerclage performed. Women with rescue cerclage had continuation of pregnancy for a mean of 9.5 weeks, with low rates of perioperative complications, and a mean gesta

Advanced dilation at time of cerclage placement.

OBJECTIVE: We examined cervical dilation at time of cerclage placement and its relationship to gestational age (GA) at delivery. METHODS: We conducted a retrospective cohort study of patients undergoing physical exam or ultrasound indicated cerclage in singleton pregnancies from 2014-2024 at a tertiary care facility. Patients were stratified by cervical dilation (< 1 cm, 1-2 cm, ≥ 3 cm). The primary outcome was GA at delivery. Secondary outcomes included rates of extreme prematurity and latency to delivery. Logistic regression was performed to calculate adjusted relative risk (aRR) with 95% c

Women's motivations for and experiences with transabdominal cerclage: A qualitative study.

INTRODUCTION: Transabdominal cerclage (TAC) is used to prevent pregnancy loss and preterm birth in women with previous failed transvaginal cerclage or insufficient cervical tissue. TAC requires laparoscopic or open abdominal surgery, cesarean birth, and the potential long-term effect are currently unknown. While the procedural aspects, indications, and pregnancy outcomes associated with TAC are subject to ongoing research, little is known about how women experience the decision to undergo a TAC and the pregnancy that follows. MATERIAL AND METHODS: We conducted a qualitative interview study wi

Optimal management of pregnancies presenting with membrane prolapse.

INTRODUCTION: Membrane prolapse represents a severe manifestation of cervical insufficiency, occurring in 0.3-1.0 % of pregnancies and posing significant risks for extreme prematurity and perinatal mortality. Despite decades of clinical experience, optimal management remains controversial. CONTENT: This narrative review synthesizes current evidence on pathophysiology, diagnosis, and management of membrane prolapse. We examine biomechanical and inflammatory mechanisms, diagnostic criteria, including cervical length assessment and amniocentesis, as well as therapeutic strategies, such as emerge

Cervical insufficiency revisited: a proposal for a new treatment-aligned definition.

Cervical insufficiency [CI] has traditionally been defined as painless cervical dilation resulting in recurrent second-trimester pregnancy loss in the absence of labor or placental pathology. Definitions from the American College of Obstetricians and Gynecologists and the Royal College of Obstetricians and Gynecologists emphasize historical clinical presentation. However, contemporary obstetrics relies increasingly on ultrasound-based cervical length surveillance and evidence-based cerclage indications rather than retrospective diagnosis. The objective of the review was to propose a definition

Refractory Neonatal Apnea Revealing Congenital Central Hypoventilation Syndrome: Improved Outcome through Early Multidisciplinary Intervention.

Background: Congenital central hypoventilation syndrome (CCHS) is a rare genetic disorder that causes alveolar hypoventilation because of impaired chemoreceptor response to hypercapnia and hypoxia occurring due to a dysfunctional central autonomic drive. CCHS is a result of pathogenic variants of the PHOX2B gene, which affects neural crest cell development. Consequently, CCHS can be accompanied by dysregulation of the autonomic system, Hirschsprung's disease and other gastrointestinal disorders, cardiac arrhythmias, and neural cell-derived tumors. Disease severity correlates with mutation type

The Impact of Congenital Anomalies of the Male and Female Reproductive Organs on Infertility and Recurrent Pregnancy Loss: A Review.

Congenital anomalies of the reproductive system represent a heterogeneous group of structural and functional abnormalities affecting both male and female genital organs. These anomalies typically arise during embryogenesis and may remain asymptomatic until they are incidentally identified during evaluation for infertility, recurrent pregnancy loss, or disorders of sexual development. In females, abnormalities include Müllerian duct anomalies and congenital malformations of the uterus, cervix, vagina, and ovaries, such as Mayer-Rokitansky-Küster-Hauser syndrome, septate, unicornuate, bicornuate

Antihypertensive Treatment Escalation and Maternal Outcomes During Expectant Management of Early-Onset Preeclampsia With Severe Features.

BACKGROUND: Early-onset preeclampsia with severe features is associated with substantial maternal morbidity. During expectant management, oral antihypertensive medications are frequently escalated and intravenous agents are administered for severe hypertension, yet the clinical significance of escalating antihypertensive treatment has not been well characterized. OBJECTIVE: To evaluate the association between antihypertensive treatment escalation during expectant management of early-onset preeclampsia with severe features and adverse maternal outcomes. STUDY DESIGN: This retrospective cohort

Endometrial Vitamin D Signaling and Immune Escape in Recurrent Pregnancy Loss.

Recurrent pregnancy loss (RPL) continues to be a significant challenge in reproductive medicine, particularly in women for whom standard examinations do not reveal a conclusive underlying reason. There is growing evidence that several instances may result from nuanced alterations in the endometrial milieu, particularly with decidualization and maternal-fetal immune tolerance. In recent years, vitamin D has gained recognition for its significance in early pregnancy, serving not only as a regulator of calcium metabolism but also as an active contributor to endometrial and immunological functions

Elevating blood pressure in neurogenic orthostatic hypotension: Investigating the efficacy and tolerability of rebreathing therapy.

PURPOSE: Neurogenic orthostatic hypotension (nOH) causes profound drops in blood pressure (≥20/10 mmHg). We previously showed that carbon dioxide (CO2) inhalation powerfully elevates standing blood pressure in patients. Rebreathe devices elevate CO2 and lower O2 and may offer a simple, cost-effective way to raise standing blood pressure. We tested the acute efficacy of high CO2 + low O2 (i.e., simulated rebreathing) to raise standing blood pressure. Additionally, we compared simulated rebreathing to high CO2 (+CO2) alone. METHODS: In this exploratory sub-study, ten patients with nOH (F = 3; 6

Oral antihypertensive agents in hypertensive pregnancies and adverse maternal and perinatal outcomes: study protocol for a target trial emulation using Dutch electronic health record data.

INTRODUCTION: Hypertensive disorders of pregnancy (HDP) are associated with maternal and neonatal morbidity. Although delivery is the definitive treatment, antihypertensive agents are often prescribed to control blood pressure to facilitate safe prolongation of pregnancy. International guidelines vary in their treatment recommendations and preferences for methyldopa, labetalol and nifedipine. Within the field of HDP, randomised trials are often underpowered and observational studies are limited in causal inference due to confounding. Therefore, we propose a target trial emulation with hospital

A study of the association between the early pregnancy thrombosis-thyroid axis and pregnancy outcomes in patients with recurrent spontaneous abortion.

BACKGROUND: Recurrent spontaneous abortion (RSA) is a severe, yet common, pregnancy complication. Thrombophilia and thyroid dysfunction are critical factors altering placental microcirculation and embryonic development, although the synergistic predictive value of these two conditions for RSA pregnancy outcomes remains undetermined. OBJECTIVE: To investigate the factors influencing pregnancy outcomes in patients with RSA; analyze the association between early pregnancy markers (including coagulation and fibrinolysis, platelet function, and thyroid function) and pregnancy outcomes in patients

Canine brachycephalic obstructive airway syndrome as a model for studying the effects of obstructive sleep apnea during pregnancy.

Brachycephalic obstructive airway syndrome (BOAS) in dogs shares pathophysiological features with human obstructive sleep apnea, presenting different degrees of airway obstruction. Thus, the severity of the disorder may result in negative pregnancy consequences differently. This study aimed to compare the effects of BOAS severity throughout gestation on maternal clinical status, uteroplacental hemodynamics, and fetal development. Bitches were grouped according to BOAS symptomatic manifestation as mild (n = 5), severe BOAS (n = 5), and non-BOAS groups (control; n = 5). Along gestation (early, m

Amniocentesis before cerclage in asymptomatic women with mid-trimester short cervix: time to consider?

The efficacy of cervical cerclage (CC) in cases of asymptomatic mid-trimester short cervix, or the ultrasound finding of a cervical length<25 mm within 24 weeks of gestation, has been documented in women with a previous history of preterm birth (PTB). Conversely, no robust evidence supports the role of cervical cerclage in prolonging pregnancy and improving maternal-neonatal outcomes in pregnant patients with mid-trimester short cervix detected by sonography and no prior history of PTB. However, the limited effect of CC in this population may be partially explained by the lack of preprocedure

Preoperative considerations for cervical cerclage (part 1).

Transvaginal cervical cerclage remains a central strategy for preventing spontaneous preterm birth in appropriately selected pregnancies. Although decades of research have clarified which patients benefit most from cerclage, substantial heterogeneity persists in pre-, intra-, and postoperative management. This article, the first in a series on cerclage technique, synthesizes the evidence guiding preoperative decision-making. Cerclage is classified by indication as history-indicated cerclage, ultrasound-indicated cerclage and physical examination-indicated cerclage. History-indicated cerclage i

Removal of small fibroids and polyps in patients with infertility and recurrent miscarriage: The HELP Fertility? RCT.

BACKGROUND: Infertility affects one in six of females globally, with uterine submucous fibroids and endometrial polyps being common findings. The effectiveness of surgical removal to improve fertility remains uncertain. The associated surgical risks and costs highlight the need for more robust research in this area. The HELP Fertility? Study aimed to assess the clinical and cost-effectiveness of hysteroscopic removal of endometrial polyps and submucosal fibroids, compared to no removal, in improving fertility outcomes for participants with infertility or recurrent miscarriage while also evalua

Reversible Neurological Manifestations Preceding Biochemical Deterioration in Postpartum HELLP Syndrome-A Case Report and Literature Review.

Posterior reversible encephalopathy syndrome (PRES) is a rare but severe neurological complication associated with hypertensive disorders of pregnancy and HELLP syndrome. We report a postpartum case in which neurological manifestations preceded the full biochemical expression of HELLP syndrome. A 22-year-old primigravida was admitted at 36 + 0 weeks for premature rupture of membranes and fetal growth restriction, presenting with mild thrombocytopenia and elevated lactate dehydrogenase but normal liver enzymes. On postpartum day 1, she developed headache, visual disturbances, hypertensive crisi
